Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today February 24, 2026 for the facility’s annual inspection. LPA introduced themselves and was allowed entrance into the facility. LPA met with Administrator Jean Asignacion. Facility is licensed for 6 residents and has a current census of 3. There are 0 residents on hospice and 0 with a health care plan. Water temperature was checked in the kitchen which read at 113.1 degrees Fahrenheit and the common bathroom which read at 114.9 degrees Fahrenheit. Fire Extinguisher was purchased 4/1/2025 and is within the safety regulation period. Carbon monoxide detectors were tested and in working order. Jean Asignacion Administrator's Certification expires 11/12/2027. Staff files were reviewed, and training is not current. Resident files were reviewed, and are not current, physician report is not current for R1 & R2. Facility did not provide a current log for disaster drills that are conducted quarterly. First aid kit on site and complete. Tools and knives are not locked and accessible to residents when in the kitchen. There is a locked storage for medications. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean, properly furnished, with adequate lighting, and in good repair. Food supply is adequate for 2-day perishable and 7-day nonperishable. LPA observed the following deficiencies and cited during today's inspection per California Code of Regulations, Title 22. On duty caregivers do not have current first aid training or current training. 2 of 3 resident’s physician reports are not current Kitchen appliances have grease/food buildup on outside and inside. Dishwasher has mildew buildup inside. Microwave has food debris on the inside Refrigerator and cabinets have debris Tools in unlocked cabinet & lighters in unlocked kitchen drawer Leak under kitchen sink. Tools in unlocked cabinet in the kitchen Cleaning products accessible in staff bathroom (bathroom was unlocked) Administrator was not able to provide quarterly drill verification. R3 Centrally Stored Medication Log incomplete Raloxifene HCL 60 MG TAB not logged Med count off Raloxifene HCL 60 MG TAB started 11/29/2025- 2/24/2026 (87days), 90 count, 90-87= 3 pills should be left, 5 pills were left in the bottle. No PRN form for R3 Lorazepam Additional lock to front exterior door. LPA's requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Listed documents shall be sent to Licensing. On 03/27/2025, Licensing Program Analyst (LPA) M Vega arrived unannounced to conduct an annual inspection. LPA introduced stated the purpose of the visit and requested to meet with the Administrator. Facility staff contacted Administrator, Jean Asignacion via telephone. Administrator arrived a short time later. LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free and at a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. Resident rooms clean and had required furnishings. LPA observed an adequate supply of linen. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and tub/shower areas, non-skid mats were observed. Hot water measured 114.0 degrees F in facility restroom. Kitchen toured, clean, observed a 7-day supply of non-perishable and 2-day supply of perishable food. Exterior tour conducted, all exits open and free of obstructions. Side gate was observed to be self-latching. Fire extinguisher serviced on 04/21/2024. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. LPA reviewed staff and client records. Medications reviewed and observed to have original labels and be administered as prescribed. First Aid Kit contained the required supplies. Continuation on LIC 809C LPA is requesting the following documents be submitted to the Fresno CCL: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC 308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E) Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020A), Surety Bond LIC 400. No deficiencies issued. Exit interview conducted. On 03/27/2024, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct an annual inspection. LPA introduced stated the purpose of the visit and requested to meet with the Administrator. Facility staff contacted Administrator, Jean Asignacion via telephone. Administrator arrived a short time later. LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free and at a comfortable temperature. Common areas were furnished well with adequate seating and lighting available. LPA observed a long, wooden stick on the track of the sliding door in the dining room. Resident rooms appeared clean and had required furnishings. LPA observed medications on the counter in the bathroom in bedroom 4 and nutritional supplements on a dresser in bedroom 3. Medications and nutritional supplements were accessible to residents in care. LPA observed an adequate supply of linen. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and tub/shower areas, non-skid mats were observed. Hot water measured between 112.6 degrees F and 117.2 degrees F. Kitchen toured, appeared clean, observed a 7-day supply of non-perishable and 2-day supply of perishable food. Exterior tour conducted, all exits open and free of obstructions. Side gate was observed to be self-latching. Fire extinguisher serviced on 04/12/2023. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Facility has not conducted a fire/emergency disaster drill. LPA reviewed staff and client records. Upon review of resident records, R2 did not have an updated physician's report. LPA also found that 3 out of 4 residents receiving hospice services did not have a care plan on file. LPA observed that 3 out of 3 staff did not have training specific to each resident on file. Upon review of staff files, LPA found that 3 out of 3 staff did not have a complete personnel file, multiple documents appeared to be missing, including the health screen, employee rights, and personnel record. LPA also found that 2 out of 3 staff did not have updated training. Medications reviewed and observed to have original labels and be administered as prescribed. LPA observed multiple medications that had been discontinued stored in a cabinet near the stove. Administrator stated, the medications are "old" and need to be destroyed. CONTINUED TO 809C. LPA is requesting the following documents be submitted to the Fresno CCL office by 04/10/2024: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E) Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020A), Surety Bond Deficiencies are being cited in accordance to California Code of Regulations, Title 22 Division 6 on the attached 809D.
