On 5/27//2026 at 8:55 AM, Licensing Program Analyst (LPA) Eldin Serrano made an unannounced visit to the facility to investigate and deliver the findings of the above allegations. LPA Serrano met with administrator Delcie Mucha to explain the purpose of the visit. The investigation consisted of interviews with facility staff, outside parties, and a hospice agency, as well as review of resident records, hospice documents and physician orders. Allegation #1: Staff did not provide resident with appropriate mattress resulting in resident sustaining pressure injures: Interviews revealed that the air mattress in question was ordered, delivered, and managed by the resident's hospice agency, not the facility. Staff reported that the mattress was delivered by hospice personnel and that facility staff sought assistance from hospice when concerns arose regarding the mattress. Licensing Program Analyst (LPA) Sarina Ramirez conducted an unannounced visit to deliver findings on the allegations mentioned. LPA met with Administrator Delcie Mucha and explained the purpose of the visit. LPA's investigation involved records review, tour of the facility, interviews with staff, residents and outside parties. Regarding Allegation #1 – Staff Mismanaging Residents’ Medication: LPA interviewed eight (8) residents. Five (5) of the eight (8) residents reported that their medications are not mismanaged and stated that medications are administered consistently at the same time each day without errors. One (1) resident does not take any medication, and another (1) resident was unable to answer due to memory impairment. LPA interviewed six (6) staff members. Three (3) staff reported that medications are not mismanaged. LPA introduced herself and disclosed the purpose of the visit to Marketing Director Sneha Khalid. On August 18, 2023, it was alleged that staff handled resident in a rough manner, staff engaged in verbal altercations with resident, staff did not ensure resident was transported to appointment, and staff did not safeguard resident's personal belongings. It was also alleged that staff did not ensure resident had privacy, staff are not meeting residents dietary needs, and staff are not providing adequate food service to residents. Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced complaint visit to the facility. LPA was granted entry into the facility and met with Administrator, Delcie Mucha, who was informed of today’s visit. The investigation consisted of pertinent record review and interviews with relevant parties. Regarding the allegation, resident sustained unexplained sores while in care, interviews with four (4) staff and four (4) residents reveals not enough evidence to corroborate that a resident sustained unexplained sores while in care. Allegation investigated: Unlawful eviction
Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced complaint visit to the facility. LPA met with Administrator, Elizabeth Mahan and discussed the purpose of the visit. Regarding the allegation, unlawful eviction, On 7/18/2023, the facility issued a thirty-day eviction notice to Resident #1 (R1) for unpaid rent. LPA reviewed the eviction notice which included key elements: reason for eviction, effective date, facility referrals, and a statement informing the resident of their right to file a complaint with the licensing agency and Ombudsman. During today's visit, LPA found that R1 is still residing at the facility. Based on LPA observations, pertinent record review and interviews with relevant parties, the allegation is Unsubstantiated. Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced complaint visit to the facility. LPA met with Elizabeth Mahan, Administrator and discussed the purpose of the visit. Regarding the allegation, staff are allowing children to provide care to residents at the facility, LPA toured the facility and did not witness children providing care to residents. Administrator and (5) staff interviewed deny children are providing care to residents at the facility. Six (6) out of nine (9) residents interviewed stated that they have not witnessed children providing care to residents. Based on LPA observations and interviews with pertinent parties, the allegation is Unsubstantiated. A finding that is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Licensing Program Analyst (LPA) Anna Bueno conducted an unannounced visit to the facility to investigate the above mentioned complaint allegations and deliver findings. LPA identified herself to facility staff who phoned administrator Elizabeth Mahan and were notified of the reason for today’s visit. Administrator Mahan arrived and LPA informed Mahan of the elements of the allegation. The investigation included resident and staff interviews, facility inspection, and records review. Allegation 1: Facility staff are retaliating against Resident (R1) for filing a complaint. Interview with R1 deny that staff are retaliating against them but stated that some staff are not kind. Staff interviews reveal that all residents are treated with dignity and respect. Staff interviews further reveal that they work with more challenging residents and provide assistance to the resident's families.
