Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Staff Eddie Molano and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6), a current census of (1). LPA Ramirez conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility has a swimming pool with no water and is enclosed with a locked gate inaccessible to residents in care. Outdoor shaded area is sufficient for resident activities. The facility has sufficient lighting and is maintained at a comfortable temperature. The facility is equipped with operating smoke detectors/carbon monoxide alarms, laundry equipment, and telephone service. Resident’s showers, toilets, and hand washing areas were operating properly. The hot water temperature in (1) residents bathroom measured at 112.6 degrees F. Three (3) resident’s bedrooms had beds, bed linen, chairs, dresser, storage space and sufficient lighting. The facility has sufficient linens, towels, and personal hygiene items for residents. The facility has posted in a common area, CCLD complaint poster, Ombudsman poster, facility license, emergency disaster plan with telephone numbers,facility sketch, house rules, and visiting hours. Food Service: Facility kitchen and dining areas are maintained clean. The facility has sufficient perishable food supply for residents in care. Sharps and chemicals were kept locked and inaccessible to residents in care. Continuation on LIC – 809C: Care & Supervision: Facility has 24-hour/7days a week care staff. Staff working was CPR/First aid trained and background cleared. Medical Related Services: Resident’s medications are labeled and centrally stored in a locked cabinet. Resident #1 (R1) had an incomplete medication list, Technical Violation and Deficiency issued. Record Review: Two (2) Staff files reviewed, were observed to be complete. One (1) Resident file reviewed was observed to be incomplete, needs and service plan missing, deficiency issued. Facility did not maintain liability insurance deficiency issued. Based on observations and record review deficiencies and technical violation were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted where reports (LIC809/C, LIC 9102 & LIC809-D) were discussed and copies were provided with appeal rights to Staff Eddie Molano at the conclusion of the visit.the state’s words, verbatim · CDSS document, May 19, 2026

LPA met with Nina Molano, Licensee/Administrator, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6) and a current census of (1) resident in care. LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility swimming pool is empty & enclosed with a locked gate. Outdoor shaded area is gated and sufficient for resident activities. The facility is maintained at a comfortable temperature. Resident bathroom faucets are working properly and equipped with grab rails and slip mats. The hot water temperature in resident's bathroom measured 107 degrees F. Three (3) resident’s bedrooms audited had mattresses, pillows, bedlinen, dressers, chairs, and lighting. Facility has operating carbon monoxide alarms, laundry equipment, and telephone service. The facility has sufficient linen, towels, and personal hygiene items for residents. The facility has posted in a common area, Community Care Licensing complaint poster, Ombudsman poster, Personal Rights, facility license, house rules, and emergency telephone numbers. Sharps, disinfectants, and cleaning solutions are kept in a locked drawer and cabinet. Food Service: The facility has sufficient non-perishable and perishable food and snacks for residents in care. The facility maintains clean cups, dishes, and utensils for residents. Care & Supervision: Facility has 24-hour, 7 days a week care staff. Facility staff have CPR/first aid training. Record Review: Staff files reviewed did not have annual dementia job training on file for staff#1 (S1) and staff#2 (S2). Resident #1(R1) file reviewed did not have an annual appraisal on file. The last appraisal on file for R1 was dated 1/30/23. The facility did not have record of a recent quarterly drill on file. The last drill conducted on earthquake/fire was over a year ago (7/01/24). Medical Related Services: Resident’s medications are centrally stored in a locked cabinet. The facility has a first aid kit with manual. Deficiencies were cited during today's visit, in accordance with Title 22 of the California Code of Regulations. An exit interview was conducted where this report and correction plans were discussed. Medical Related Services: Resident’s medications are labeled and centrally stored in a locked cabinet. Record Review: Three (3) Staff files reviewed, staff #1,2, and 3 had expired CPR certificates, deficiency will be cited. Three (3) Resident files reviewed were observed to be complete. Based on observations and record review deficiencies were cited per Title 22, Division 6, of the California Code of Regulations.
