Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator Anchalee Phumirat, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6), a current census of (5). LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility does not have a swimming pool or similar bodies of water. The facility has sufficient lighting and is maintained at a comfortable temperature. The facility has sufficient indoor and outdoor space for resident activities. The facility is equipped with operating smoke detectors/carbon monoxide alarms, working laundry equipment, and telephone service. Resident’s showers, toilets, and hand washing areas were operating properly. The hot water temperature in four (4) resident bathrooms measured between 110.3 and 120 degrees F. Five (5) resident’s bedrooms had beds, bed linen, chairs, dresser, storage space and sufficient lighting. The facility has sufficient linens, towels, and personal hygiene items for residents. The facility has posted in a common area, facility license, administrator certificate, facility sketch, activities calendar, emergency disaster plan and telephone numbers, CCLD complaint poster, and Ombudsman poster. Food Service: Facility kitchen and dining area are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents in care. Sharps and chemicals were kept locked and inaccessible to residents in care. Continuation on LIC – 809C: Care & Supervision: Facility has 24-hour/7days a week care staff. Facility staff working have current CPR/first aid training and background clearance. Medical Related Services: Resident’s medications are labeled and centrally stored in a locked cabinet. Three (3) residents medication was audited at random with no issues. Record Review: Three (3) Staff files reviewed were observed to be complete. Five (5) Resident files reviewed were observed to be complete. Last disaster drill was conducted on 03/20/26. Liability insurance is up to date. Based on observations and record review no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Type of visit: POC

Licensing Program Analyst (LPA) Beena Singh conducted an unannounced visit to the facility to verify clearance of Plan of Correction from visit on 10/16/2025. LPA Singh met with a Staff and was granted entry to the facility. Licensee/Administrator Ann Phumirat Voodtikon arrived at the facility during the visit. The following deficiencies were cleared during the time of the visit. The Licensee was cited on 10/16/2025, for HSC 1569.605- Facility has no Liability insurance coverage, the licensee provided the Liability Insurance certificate, Valid through 10/30/2025 to 10/30/2026, in the amount of at least one million dollars ($1,000,000) per occurrence and three million dollars ($3,000,000) in the total annual aggregate, caused by the negligent acts or omissions to act of, or neglect by, the licensee or its employees. The Plan Correction was cleared and a copy of clearance letter was provided to Licensee/Administrator Ann Phumirat Voodtikon.. Type of visit: Required - 1 Year

On 10/16/2025, Licensing Program Analyst (LPA) Beena Singh made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Facility Licensee/Administrator Anchalee Phumirat and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE). License capacity of (6) with a current census of (6) residents. LPA Singh conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant: Indoor and outdoor passageways were free of obstruction. The facility has no swimming pool or outdoor bodies of water. The facility has a covered outdoor patio area and self-latching backyard gate. The facility has sufficient lighting and is maintained at a comfortable temperature. Fireplace is adequately screened. The facility has operating fire/carbon monoxide alarms and telephone service. Posters such as personal rights, Ombudsman telephone number, Community Care Licensing complaint telephone number, emergency disaster plan, and emergency phone numbers were posted in a common area. Sharps and cleaning solutions were kept locked, inaccessible to residents in care. Food Service: Kitchen and dining areas were maintained clean. The facility’s refrigerators had sufficient space for food storage and were maintained in healthful manner. The facility has sufficient non-perishable food supply for the number of residents in care. Facility has sufficient cups, plates, and utensils for resident use. Facility has no Liability insurance coverage. Deficiency will be issued. Records review indicated facility's emergency Fire/disaster/Earthquake drill was conducted on 08/20/2025. Bedrooms: Resident’s bedrooms were equipped with beds, bed linen, night stands, chairs, storage space, and sufficient lighting. Resident’s bathrooms were equipped with grab rails, bath chairs and operating in safe and sanitary condition. The hot water temperatures measured 118 degrees F. Care & Supervision: Facility has sufficient care staff for coverage 5 days a week (Mon-Fri). All staff members working in the facility have criminal record clearance through the department. Records Review: LPA Singh reviewed three (3) client files for admission agreements, updated physician reports, and needs and services plans. LPA also reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings. Based on the observations made during today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report (LIC809) was discussed and provided to one deficiency was cited during today's visit per the California Code of Regulations and Health and Safety Codes. LPA met with Anchalee Phumirat, Licensee and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE). License capacity of (6) with a current census of (6) residents. LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Indoor and outdoor passageways were free of obstruction. The facility as no swimming pool or outdoor bodies of water. The facility has a covered outdoor patio area and self-latching backyard gate. The facility has sufficient lighting and is maintained at a comfortable temperature. Fireplace is adequately screened. The facility has operating fire/carbon monoxide alarms and telephone service. Posters such as personal rights, Ombudsman telephone number, Community Care Licensing complaint telephone number, emergency disaster plan, and emergency phone numbers were posted in a common area. Sharps and cleaning solutions were kept locked, inaccessible to residents in care. Review of facility's emergency drill training reveals, last staff training was conducted in July 2023. Kitchen and dining areas were maintained clean. The facility’s refrigerators had sufficient space for food storage and maintained in healthful manner. The facility has sufficient non-perishable and perishable food supply for the number of residents in care. Facility has sufficient cups, plates, and utensils for resident use. Resident’s bedrooms were equipped with beds, bed linen, nightstands, chairs, storage space, and sufficient lighting. Resident’s bathrooms were equipped with grab rails and operating in safe and sanitary condition. The hot water temperatures measured at 117 degrees F. Resident's medications were centrally stored in a locked cabinet; However, review of residents medication records reveals records a not current. The administrators stated that some of Resident #1(R1s) prescribed medications are temporarily not being administered to R1 due to their effects on R1; However, there is no documentation to of the effects or doctors/nurses orders to stop administering the medication. Facility staff records reviewed had First Aid Certifications, fingerprint clearances/exemptions, health screenings, and training. LPA review of resident's records reveals Resident #2 (R2) did not have have tuberculosis results on file. Deficiencies were cited during today's visit per the California Code of Regulations and Health and Safety Codes.
