On 04/10/2025 at 9:09AM Licensing Program Analyst (LPA) Renese Howell-Small conducted an unannounced visit to the facility in order to deliver findings for the above allegations. LPA discussed the purpose of the visit with Administrator, Melvin Torres. The investigation consisted of interviews and record review. In regards to the allegation of staff did not inform the resident's authorized person of resident's death: LPA interviewed the Administrator, a representative from a placement agency, reviewed R1's Admission Agreement and reviewed the Sheriff's Department report dated 01/15/2025. LPA confirmed that R1 was capable of making independent decisions, was self-responsible and did not list any relatives or friends on the facility's documents. Therefore, this allegation is UNSUBSTANTIATED. Allegation investigated: Staff financially abused resident in care Staff withheld deceased residents personal property from residents family

On 01/06/2025 at 1:16PM Licensing Program Analyst (LPA) Renese Howell-Small conducted an unannounced visit to the facility to deliver findings for the above allegations. LPA was greeted by Administrator Melvin Torres, stated the purpose of the visit and gained access to the facility. The investigation consisted of interviews and record review. In regards to the allegation of staff financially abused resident in care: LPA interviewed three (3) residents and (3) staff. LPA also reviewed resident records. LPA confirmed that Resident #1's (R1) rent checks matched the signed Admission's Agreement. Residents in care deny staff financial abuse and stated that they trust staff. Allegation investigated: Facility staff is a joint tenant on resident's bank account.

On 9/16/24 at 9:30AM, Licensing Prorgam Analysts (LPA's) Renese Howell-Small and Melody Brown conducted an unannounced visit to the facility to commense a compLlaint investigation. LPA's Small and Brown were greeted and granted entrance by a staff member and icensee/Administrator Melivin Torres was contacted by a staff member and informed of the reason for our visit. LPA's Small and Brown identified themselves and discussed the purpose of the visit to staff#3 (S3), during the visit Licensee/Administrator Torres arrived and met with LPA's Small and Brown and discussed the elements of the allegation with Licensee/Administrator, Torres. The investigation was conducted by LPA's Small and Brown. The investigation consisted of file review and interviews with relevant parties. The allegation indicated that Facility staff is a joint tenant on resident's bank account.LPA's Small and Brown were able to obtain evidence to corroborate the allegation.
