Licensing Program Analyst (LPA) Mary Rico conducted an unannounced visit to investigate and deliver findings on the allegations listed above. LPA met with Administrator Edgar Gallardo and explained the purpose of the visit. The investigation consisted of staff interviews, resident interviews, record review and facility tour. For the allegation, Staff left resident in soiled diapers causing resident to sustain a rash. During residents’ interviews 9 out of the 9 residents stated they do not have a diaper rash. During staff interviews 6 out of the 6 staff stated they are responsible to change their resident’s brief every two hours. In addition, 6 out of the 6 staff stated the facility does not have a resident with a diaper rash. During record review, LPA obsevred no resident sustain a diaper rash. Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to deliver findings on the allegation listed above. LPA met with Administrator Edgar Gallardo and explained the purpose of the visit. The investigation consisted of staff interviews, resident interviews and document review. For the allegation, Resident sustained unexplained injuries. Investigation was conducted by Department staff which consisted of seven (7) staff interviews five (5) resident interviews and document review. Evidence revealed that resident had multiple falls while living at the facility and on two (2) occasion R1 was handled roughly by staff resulting in multiple injuries. Staff interviews indicate that R1’s health rapidly declined and had multiple falls. Staff indicated that R1 would be found on the ground or hanging off the bed almost every day. This unannounced visit by Amy Goldenberg, Licensing Program Analyst (LPA), is being conducted to initiate the 10 day visit to investigate the above-mentioned complaint allegation. During the course of this investigation visit LPA reviewed one resident record (R1), interviewed R1, and interviewed one employee (E1). LPA collected a copy of a personal property inventory for R1 and the physician's report of R1. After review of the information LPA learned the following: R1 alleges that staff are entering their bedroom and taking their personal belongings. R1 is unable to provide any name of staff that they suspect stole their property. The personal property inventory reviewed does not list any items. It is blank and signed by R1 on 10-23-2020 and it is checked that they declined to have personal property inventoried. Licensing Program Analyst (LPA) Jennifer Semin conducted an unannounced visit to deliver the findings for the above complaint allegation. LPA met with Resident Relations Director, Edgar Gallardo. The investigation consisted of interviews and review of records. The allegation, Facility staff mismanaged resident's medications while in care. Staff and Resident 1 (R1) stated staff give R1 their medication on time and in correct amount. The Medication Administration Record (MAR) indicates that the medication was not given according to R1's physician’s order and that R1 had refused or refused to take the full dose prescribed on multiple occassions. Based on interviews and documentation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
