Allegation investigated: Staff are not providing adequate food service to residents resulting in sickness

Licensing Program Analyst (LPA) Paola Guerrero arrived at the facility to deliver investigative findings. LPA met with Facility Administrator Janeth Medrano and explained the purpose of the visit regarding the allegations stated above. First allegation: Staff are not providing adequate food service to residents resulting in sickness. Regarding the allegation stated above, LPA conducted interviews with Residents #1-5, regarding the alleged allegation and Residents #1-5 informed LPA that they have not been served food that is raw, cold, or not cooked thoroughly. Residents #1-5 also denied getting sick or food poisoning due to the food. LPA conducted interviews with Staff #1-5 regarding the alleged allegation and Staff #1-5 informed LPA that it was reported that residents did fall sick however, it was not due to food poising but rather the stomach flu which was reported to Community Care Licensing and Department of Public Health. Staff #1 informed LPA that last symptoms were reported on 4/30/2026. Unsubstantiated Based on corroborating evidence LPA has determined that the above allegation is Unsubstantiated, meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Type of visit: Case Management - Other

Licensing Program Analyst Paola Guerrero, (LPA) arrived at Oakmont of Chino Hills, unannounced to conduct a Case Management Visit for a follow-up visit concerning bedbugs. This case management visit is in response to a Special Incident Reports, (SIR's) submitted to the Community Care Licensing Office on 04/16/2026 & 4/20/2026. LPA was greeted by Facility Administrator Janeth Medrano. LPA explained to Janeth the purpose of the visit and informed Janeth that a health and safety check will be conducted. LPA explained to Janeth that documents of bedbug treatment and treatment plan will be reviewed and collected by the LPA. ***Health and Safety Check*** Licensing Program Analysts (LPA) Paola Guerrero conducted an unannounced visit to the facility on 04/29/2026 at 09:40 AM for the purpose of a Health & Safety check. LPA Guerrero identified herself with Janeth Medrano and discussed the purpose of the visit. Residents in care were present during visit. No imminent health and/or safety concerns observed at the time of visit. During the visit LPA inspected Room #308A, 214, 335, LPA observed that as of 4/29/2026 no new cases of bedbugs have been reported. LPA collected invoices and continued the inspection. During the inspection LPA Guerrero observed no health and/or safety hazards inside the facility. The needs of the residents in care appear to be met during this inspection. No deficiencies were observed during this visit. Type of visit: Case Management - Deficiencies

Licensing Program Analyst (LPA) Paola Guerrero arrived at the facility to deliver findings on complaint number 56-AS-20260305140133. This case management visit is in response to complaint number 56-AS-20260305140133. LPA met with Facility Administrator and explained the purpose of the visit. LPA explained to Janeth that a deficiency for failing to report will be issued to the facility in relation to complaint control number 56-AS-20260305140133. During interview with Facility Administrator it was reported to LPA that bedbug issue began February 14,2026 however, the matter was not properly reported by staff. Facility Administrator informed LPA that the facility learned about the matter concerning bedbugs on February 21, 2026. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Paola Guerrero made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Facility Administrator Janeth Medrano and was granted entry to the facility. The facility is a Residential Care Facility for Elderly (RCFE) Licensed capacity is (170) current census (139). LPA was accompanied by Facility Administrator, to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected resident’s bedrooms; they are equipped with required furniture such as: mattresses, nightstands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for resident/staff files. Medications are kept inside Med-Room's inaccessible to residents in care. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. All staff members working in the facility have criminal record clearance through the department. Record Review: LPA reviewed four (4) resident files for admission agreements, updated physician reports, and needs and services plans. LPA also reviewed four (4) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings. Medications were audited at random and appeared to be dispensed appropriately by staff members. Based on the observations made during today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analysts (LPAs) Paola Guerrero and Beena Singh made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPAs met with Facility Administrator Janeth Medrano and was granted entry to the facility. The facility is a Residential Care Facility for Elderly (RCFE) Licensed capacity is (170) current census (142). LPAs were accompanied by Facility Administrator, to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPAs inspected resident’s bedrooms; they are equipped with required furniture such as: mattresses, nightstands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPAs observed sufficient furniture and lighting throughout the facility. LPAs measured and observed the water temperatures in the bathrooms to be 111.6 degrees F The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for resident/staff files. Medications are kept inside Med-Room's inaccessible to residents in care. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Allegation investigated: Licensee did not refund preadmission fees.

Licensing Program Analyst (LPA) Paola Guerrero conducted an unannounced visit to deliver findings on the allegation listed above. LPA met with Facility Director Janeth Medrano and explained the purpose of the visit. The investigation consisted of interviews and record review. First allegation, Licensee did not refund preadmission fees. LPA conducted an interview with Facility Director Janeth Medrano who explained that a refund check to Resident #1 was given on 2/7/2024. LPA Guerrero obtained a copy of Resident #1 Service Agreement document and observed the description and breakdown of the fees that were owed to the facility along with the total amount that will refunded back to Resident #1. Based on documentation LPA observed that Resident #1 personally picked-up a refund check at Oakmont of Chino Hills on 2/7/2024. Based on the evidence obtained during the course of the investigation, LPA has determined that the above allegation is Unsubstantiated. Unsubstantiated Unsubstantiated; meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
