Licensing Program Analyst (LPA) Ruth Martinez visited the facility to deliver findings for the investigation into the above identified complaint allegations. LPA arrived at facility and was greeted at the door by caregiver and granted entry. LPA spoke with Ameer Attrah, Administrator and explained the purpose of the visit. The complaint was investigated by the Department. Findings are based upon this investigation which included records review (St. Josephs Hospital records from 8/1/25-10/01/25, County of Orange Death Certificate, resident facility file), interviews with the following: 3 staff, 3 residents and 1 witness. It is alleged staff neglect resulting in sepsis. Record review from St. Josephs Hospital revealed that resident (R1) had been sent to the hospital on August 26, 2025, due to an altered state of mind and was found to have a urinary tract infection. An unannounced Complaint Investigation was conducted on this day by Licensing Program Analyst (LPA) Claudia Gutierrez. LPA met with Licensee Ahmed Attrah and explained the purpose of the inspection. Regarding the allegation, Staff leaves a resident soiled while in care, the following was revealed: It is alleged Staff 1 (S1) leaves Resident 1 (R1) soiled for an extended period of time, up to three hours. During the course of the investigation, interviews were conducted with R1, three additional facility residents, and two staff. During their interview, R1 denied being left soiled for an extended period of time and stated facility staff change them regularly. During their interview, three of three additional facility residents denied being left soiled or wet for extended periods and denied having any knowledge of any other resident being left soiled or wet for extended periods of time. Licensing Program Analyst (LPA) Fred Arias conducted an unannounced complaint visit to finalize an investigation into the above allegation. LPA was greeted and granted entry into the facility and explained the reason for the visit. It was alleged staff member yells at residents in care. During the investigation, LPA conducted interviews with residents in care and staff. LPA reviewed records obtained. The investigation determined as follows: Regarding the allegation staff member yells at residents in care, it was reported a staff member yells at everyone. LPA interviews with three out of four residents stated they have not been yelled at directly. One out of those three residents added they have heard loud voices through the door but does not know who is speaking. The remaining resident stated they have been yelled at by staff sometimes but believes that is the staff's personality. The resident added she likes the staff. Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. conducted an unannounced initial 10-Day complaint visit to initiate the investigation into the above allegation and to deliver the findings of the investigation. LPA was greeted and granted entry into the facility and met with Caregiver Stefania Paunescu. LPA explained the reason for the visit. Administrator (AD) Ameer Attrah arrived during the visit. This agency has investigated the complaint alleging that facility staff emotionally abuses resident. Regarding the allegation, the following was revealed: During the course of the interviews one of seven individuals interviewed confirmed the allegation. During the course of the interviews with residents, Resident 1 (R1) reported that staff have never emotionally abuse her. Per R2, staff are respectful and stated that she feels safe.
