Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual evaluation using the CARE Inspection Tool. LPA was greeted and granted entry by Caregiver Angel Morales Alvarado after introducing self and stating the reason for the visit. Administrator Ameer Attrah arrived on premise to assist with the inspection. Administrator Attrah has a valid certificate expiring April 19, 2027 per the Active Administrator Certificate List. The following was observed during the inspection: The facility is located in a residential neighborhood comprised of two private resident bedrooms, two shared resident bedrooms, and two resident full bathrooms. LPA observed six residents in care, none of which are currently on hospice and bedridden. LPA observed two staff on duty. Facility is operating within the conditions and limitations specified on the license. LPA toured the physical plant. LPA inspected all common areas including the attached two car garage, sun room, and all resident bedrooms. The resident bedrooms' were appropriately furnished, beds and bedding supplies were in good condition, adequate lighting was provided, and sufficient storage space for each residents' personal belongings were observed. Facility appeared clean, sanitary, and in good repair as well as all bathrooms. However, LPA observed Room #3 occupied by Resident #2 (R2) is kept untidy. Admin expressed working with R2 to keep the room in order and tidy. Slip resistant mats were observed to be available. The hot water temperature in the resident bathrooms measured within range measuring at 116.7 and 117.3 degrees Fahrenheit. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food. LPA toured the exterior portion of the facility. The outdoor passageway is free of obstruction, and there are sufficient seating and shading. LPA observed two secured gate exits as well as an enclosed gate surrounding the perimeter of the swimming pool. The first aid kit contains all necessary elements. The one fire extinguisher was mounted, charged, and purchased on May 19, 2026. The auditory devices and dual functioning smoke/carbon monoxide detectors were tested and operational in all areas. Facility maintains emergency food, water. but disaster supplies noted on the Emergency Disaster Plan (LIC610E) were not available. A working telephone number (657) 201-3719 was tested and remains available. Complaint Poster, 'See Something, Say Something,' (PUB 475) was available and posted in the entry way in the correct size. LPA reviewed six residents and two personnel files. No discrepancies noted. Present staff are background cleared and associated to the facility. Staff met their annual 40 hour training. The Emergency Disaster Plan (LIC610E) was reviewed. Fire drills are conducted quarterly with the last date on May 1, 2026, however drills of various scenarios have not been conducted. Medications were audited for two of six residents. No discrepancies noted. Based on the observations made during today's visit, no deficiency is being cited. Advisories are being issued. On May 16, 2025, at 8:30 AM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim was greeted and granted entry by Caregiver (CG) Kevin Verduzco. Administrator (AD) Ameer Attrah arrived at the facility around 9:05 AM and LPA Kim explained the purpose of the visit. The facility is licensed to operate for six (6) nonambulatory residents, of which one (1) may be bedridden in bedroom #3, and has a hospice waiver for six (6) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: four (4) resident bedrooms, two (2) bathrooms, two living area, kitchen, an enclosed swimming pool, outdoor covered patio room, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with ADMIN Attrah. There were no obstructions in the facility. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, and Resident Room 4. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 107.0 degrees F to 107.6 degrees F. A comfortable temperature of 75 degrees F was maintained in the facility. Evaluation Report Continues on LIC 809-C LPA Kim observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food both were available and maintained properly. Emergency food, emergency water, and emergency supplies are stored in the garage. The facility has one (1) fire extinguisher that is charged and mounted on the wall in the kitchen. During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. The smoke detectors and carbon monoxide detectors were operable. A working telephone (657-201-3719) and video teleconferencing tablet dedicated to the residents both remain available. An emergency drill was conducted on March 5, 2025, and is conducted quarterly. First aid kit is maintained and contains all the necessary elements. Evidence of Liability Insurance is effective September 14, 2024, and expires September 14, 2025. LPA Kim conducted an audit of resident files (R1-R5), staff files (S1-S4), and medication and medication administration record were all in order and complete. LPA Kim conducted one (1) staff interviews. No deficiencies were cited during this visit. Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Lydia Martinez conducted an unannounced Case Management visit in conjunction with a complaint visit 22-AS-20240416125427 on previously owned Licensee. LPA met with Administrator Ameer A. Attrah. LPA explained the reason for the visit. During the visit LPA observed the following: Resident #2 and Resident #3 were observed with two 1/2 bed rails to make one full bed rail. Residents are not receiving hospice services and there is no doctor's order on file for the 1/2 bed rails. In reviewing resident records, facility has not updated their records to show the new License number and New facility name. LPA noted pool gate door was not secured. Based on the observations made during today's visit, no deficiency is being cited today. Advisory Notes will be issued. Licensing Program Analyst (LPA) Ruth Martinez conducted an announced visit to the facility for purpose of a pre-licensing evaluation. LPA arrived at facility was greeted and granted entry. LPA met with Ameer Attrah, Administrator and explained the visit. A change of ownership application to operate an Adult Residential Facility for the Elderly, age 60 years and over, for (6) capacity, (0) ambulatory, (5) non-ambulatory, and (1) bedridden resident was submitted to CCL on 09/13/23. Structure: The facility is a one story house with an attached garage with 4 resident bedrooms, 2 full bathrooms, a living rooms, a dining room/kitchen. The resident’s bedrooms are spacious and will easily accommodate the resident’s furnishings. There is a large back yard with a closed patio, seating areas for resident and an exit walkway on each side of the house. Air/Heating: Central air/heating system installed with a central panel to control entire house. Bedrooms Residents: Bedrooms will accommodate 6 residents with 2 private rooms and 2 shared room accommodating two residents. Bedrooms Staff: No bedroom designated for awake-staff. Bathrooms: Facility has two full bathrooms. All bathrooms have a working toilet, wash basin, walk in shower. Linens & Hygiene Supplies: Adequate supply of linen in storage. Emergency Phone Numbers, Exit Plan & Menu: Posted & readily available for review an emergency disaster plan with means of exiting and emergency phone numbers listed. Menus posted and available. Menus prepared one week prior and listed for food serve for one week. Food Service: Adequate supply of 7-day non-perishable and 2-day perishables are stored in the kitchen. Additional food storage in garage in spare refrigerator and freezer. Smoke Detectors: Smoke detectors and carbon monoxide alert systems are hardwired, were tested, and Continued on LIC809-C found operational. 1 fire extinguishers mounted in kitchen, charged, and dated January 10, 2024.Appliances: Gas five burner stove, two ovens, two refrigerators (kitchen/garage), freezer, microwave, dishwasher, washer, and dryer are clean and noted to be operational. Toxins: All and any toxic chemicals, cleaning solutions and disinfectants are inaccessible to residents are stored and locked in a cabinet in laundry unit. Water Temperature: Tested and recorded maintained at a comfortable temperature and the water temperature measures 105.7 Fahrenheit degrees in facility bathrooms. Medications, First-Aid Kit & Book: Medication and First Aid kit stored in locked storage cabinet in the kitchen. First aid book is mounted on the wall. Resident & Staff Files: Records will be kept locked in storage cabinet located in the garage. Reading Material, Games, Equipment & Materials: The facility has board games, books, and other recreational materials for the resident's use, commensurate with the plan of operation. Pool: Facility has a swimming pool adjacent to the house in the backyard with a fence around it. The pool gate has a self-latching entry door which opens away from the pool. The fence has a key lock at the gate door for inaccessibility. The pool gate measures 5.25ft from base of the floor to the top of the fence and it was observed to enclose the entire pool area. Fire clearance: Was approved on October 25, 2023. Component III: Conducted at the Pre-Licensing visit, information provided about how to operate the facility within substantial compliance. The applicant has met all pre-licensing requirements. LPA will submit notification to CAB in Sacramento for final review prior to license being issued. Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. conducted an unannounced initial 10-Day complaint visit to initiate the investigation into the above allegation and to deliver the findings of the investigation. LPA was greeted and granted entry into the facility and met with Administrator Assistant Ameer Attrah. LPA explained the reason for the visit. This agency has investigated the complaint alleging that staff did not seek medical attention in a timely manner and staff did not communicate with resident's responsible party in a timely manner of incident. Regarding the allegations, the following was revealed: Four of four individuals interviewed denied the allegations. During the initial visit on 03/06/24 LPA observed that the facility Golden Years Group1 has not been licensed. Therefore, the allegations are deemed UNFOUNDED, meaning the allegations are false, could not have happened and/or are without a reasonable basis.
