At approximately 8:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct an investigation into the above allegations. LPA was greeted by caregiver Lino Caramat as Administrator Norbert Sacro was not present at the time of this visit. LPA reviewed documents and interviewed staff. At approximately 8:45AM, LPA reviewed Resident, R1, file and observed the file did not contain evidence of a medical examination prior to admission, no Pre-admission appraisal and no care plan. R1 was admitted to the facility 05/23/2023. LPA observed the file contained a letter sent to the responsible party on 09/15/2024,stating R1 needed a higher level of care. A similar letter was also in the file dated 09/26/2024. The letter dated 09/26/2024 appears to be a formal 60 day eviction notice. Neither of these documents were sent to Community Care Licensing at the time. Licensing Program Analyst (LPA) Erik Gonzalez Campos conducted an investigation regarding the allegation above. LPA arrived unannounced on 05/26/2022 to deliver complaint findings. On 02/15/2022 Community Care Licensing recieved a report that resident was admitted to the hospital from the facility with a pressure injury. LPA conducted interviews with hospital staff who indicated injury was documented at admission and photographs were taken. LPA requested Investigations Branch (IB) request documentation from hospital. Review of hospital records revealed that resident was admitted with an unstageable wound, a prohibited health condition. Based on the investigation, the preponderance of evidence standard has been met, therefore the above allegation has been SUBSTANTIATED. Licensing Program Analyst (LPA) Erik Gonzalez Campos arrived unannounced on 02/17/2022 to initiate complaint investigation regarding the allegation above. LPA met with Norbert Sacro, Administrator. There is an allegation of an unlawful eviction of resident. Administrator indicated R1 was transported to the hospital on 02/06/2022 for refusing to eat or drink. At the hospital R1 tested positive for COVID. Administrator stated that his last contact with the hospital regarding R1 was on either 02/08/2022 or 02/09/2022. Reporting Party indicated that they had not been able to reach facility to discuss R1's return. LPA followed up with the hospital who informed LPA that R1 was cleared to return to the facility. Administrator indicated that he was hesitant to readmit R1 because of his condition.
