Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Christi Coppo arrived unannounced to conduct a required Annual inspection and was greeted by caregiver. Licensee Mercedes Vegvary arrived shortly after. Administrators/licensees are Julius Vegvary Administrator Certificate # 7007660740 expires 2/18/27 and Mercedes Vegvary Administrator Certificate 7000812740 expires 6/30/27. At approximately 11:00am LPA and licensee toured the building and grounds. The facility was found to be clean and at a comfortable temperature. LPA observed at least a 2 day supply of perishable and 7 day supply of non-perishable food. Food was found to be stored in a safe manner with open items covered. Kitchen now has a permanently affixed physical barrier ensuring residents do not have access to the stove. However, drawer with sharp knives still remains locked. LPA observed laundry room located off kitchen to be locked and all cleaning supplies located in locked laundry room. All bedrooms were equipped with lighting, night stand, and chest of drawers. All bedrooms were clean and in good repair. Extra hygiene products and linens were available. Resident bathrooms had required bath mat and grab bar, except for the bathroom in the cottage. Per licensee, they will install a grab bar in the cottage bathroom, by no later than 3/15/26. Water temperature in sinks accessible to residents in care measured at 108.7 degrees F in the cottage, 109.4 degrees F in room #2, and 102.9 degrees F in room #5 which are within the allowable range of 105 to 120 degrees F. Fire extinguishers were last inspected 2/19/26. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Sprinklers in facility and cottage are hardwired and are serviced by a vendor, last serviced March 2025. Per licensee, vendor will be providing annual service on Continued on 809C... Continued from 809... 3/26/26. Facility’s last quarterly disaster drills were conducted 2/22/26. Facility has a backup generator for use during a power outage. LPA observed oxygen in use sign for resident using oxygen. At approximately 12:00pm LPA conducted review of five [5] staff records. All documentation present. No deficiencies cited. At approximately 12:45pm LPA conducted review of seven [7] out of seven [7} resident records. All documentation present. No deficiencies cited. At approximately 2:00pm LPA and licensee conducted a spot check of medication and medication records. Medication is centrally stored in a locked closet. LPA and licensee discussed ensuring PRN MARs are completed and all PRN medications are logged on the Centrally Stored Medication Log (CSML). LPA observed medication closet to be locked. No deficiencies cited. Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: LIC500- Personnel Report LIC308- Designation of Responsibility Liability Insurance No deficiencies cited. Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Christi Coppo arrived unannounced to conduct a Case Management and was greeted by Administrator Mercedea Vegvary. Administrator Mercedes Vegvary recently purchased new training materials to conduct dementia training for facility staff. LPA reviewed training materials. LPA reviewed book “The Practical Dementia Caregiver Guide,” by Sam Toroghi, MD published 2024. LPA observed book to cover subjects that meet 87705(b)(1)(B) and (C). LPA reviewed book “The Dementia Caregiver’s Survival Guide,” by Janet G. Cruz published 2023. LPA observed book to cover subjects that meet 87705(b)(1)(B) and (C) as well as HSC 1569.625(c)(8). Additionally, Admin advised LPA that she will be getting a supplemental book “A Caregiver’s Guide to diet and nutrition for dementia, by Sam Toroghi MD, published 2024 that addresses 87705(b)(1)(A). LPA reviewed supplemental book table of contents and it appears to address 87705(b)(1)A). Per regulation 87705(b): (b) Licensees shall be responsible for the following:(1)Ensuring staff receive the following training as part of the training requirements specified in Section 87208 Plan of Operation: (A) Dementia care, including, but not limited to, knowledge about hydration, nutrition, skin care, communication, therapeutic activities, behavioral challenges, the environment, and assisting with activities of daily living; (B) Recognizing symptoms that may create or aggravate behavioral expression, as defined in Section 87101, Definitions, including, but not limited to, dehydration, urinary tract infections, and problems with swallowing; and (C) Recognizing the effects of medications commonly used to reduce behavioral expression. Additionally, per Health and Safety Code (HSC) 1569.625: (c) The training shall include, but not be limited to, all of the following: (7) Dementia care, including the use and misuse of antipsychotics, the interaction of Continued on 809.... Continued from 809... drugs commonly used by the elderly, and the adverse effects of psychotropic drugs for use in controlling the behavior of persons with dementia. (8) The special needs of persons with Alzheimer’s disease and dementia, including nonpharmacologic, person-centered approaches to dementia care. Materials reviewed today with LPA appear to meet 87705(b) and HSC1569.625(c)(8). Materials appear to be comprehensive in covering topics listed in regulations 87705(b) and HSC1569.625(c)(8). Once supplement nutrition book and materials addressing HSC1569.625(c)(7) are obtained Admin will forward to LPA for review. Additionally, all books are available in Spanish and as audio books, both in English and Spanish. No deficiencies cited. Type of visit: Required - 1 Year
Licensing Program Analyst (LPAs) Christi Coppo and Ethel Contreras arrived unannounced to conduct a required Annual inspection and was greeted by Administrator Julius Vegvary and Mercedes Vegvary. At approximately 9:30am LPAs and Admin toured the building and grounds. The facility was found to be clean and at a comfortable temperature. LPAs observed at least a 2 day supply of perishable and 7 day supply of non-perishable food. Food was found to be stored in a safe manner with open items covered. Kitchen cabinet containing cleaning supplies was locked. Kitchen drawer with sharp knives locked. Laundry room located off kitchen was locked and all cleaning supplies were locked in cabinets. All bedrooms were equipped with lighting, night stand, and chest of drawers. All bedrooms were clean. Room #3 has flooring that is worn and lifting up such that it could present as a tripping hazard. Admin agrees to immediately replace and send pictures of replacement to CCL no later than April 17, 2025. Extra hygiene products and linens were available. Resident bathroom had required bath mat and grab bar. Water temperature in sinks accessible to residents in care did not measure within regulation upon first measurements. Admin immediately adjusted water heater temperature and LPAs tested the water again after a period of time. Water then measured at 112.2 degrees F in the cottage, which is within the allowable range of 105 to 120 degrees F. Admin and licensee agree to keep an eye on the water temperature. Fire extinguishers were last inspected 2/27/25. Smoke/Carbon Monoxide detectors located throughout the facility and cottage are hardwired, along with sprinklers, and are serviced by a vendor. Detectors are serviced annually and were last serviced March 2025. Facility’s last quarterly disaster drills were conducted 12/31/24. LPA reminded Admin to conduct drills quarterly. Facility has a backup generator for use during a power outage. Continued on 809C... Continued from 809... At approximately 11:00am LPA conducted review of five [5] staff records. CPR card present for S1 but not First Aid, card stated AED/CPR for Infant and Child but no First Aid completed. Admin was unaware that training did not include First Aid. All other staff have First Aid completed. Admin will have S1 complete First Aid by no later than 4/2/25. LPA reviewed staff training documentation and materials. Admin is using CCL PINs and printed handouts from Advanced Healthcare Studies, no publication date present, but appears to be outdated. LPA reviewed documentation of training which appeared to be fabricated: all training dates for staff conducted on the same day for all 5 staff members, some hours listed showed as many as 8 hours of training conducted on the same day. LPA asked Admin if all five staff are receiving training and she is the one conducting the training. who is attending to the residents. LPA also observed training documentation to be photocopied, no variation in dates or duration of time. Admin confirmed that training documentation was photocopied, but stated that it was not fabricated. LPA discussed with Admin the option of using an approved vendor to conduct training and print out the certificates issued as documentation, this could potentially mitigate the appearance of any fabrication. LPA also discussed with Admin the other option of purchasing training materials that are current, Admin will then submit the current training materials to CCL for approval. At the time of submission, Admin agrees to also present to LPA an example of how they will document the training. LPA and Admin will discuss together if the process by which the training will be documented is within regulation. Admin will submit training materials for approval no later than April 25, 2025. At approximately 1:00pm LPA conducted a review of six [6] resident records. All required documentation present. 1/2 rails present and on file for all residents for which they are required. At approximately 2:30pm LPA and Admin conducted a spot check of medication and medication records. Medication is centrally stored in a locked closet. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Christi Coppo arrived unannounced to conduct a required Annual inspection and was greeted by Administrator Mercedes Vegvary. Facility contact information was reviewed. At approximately 9:45am LPA and Admin toured the building and grounds. The facility was found to be clean and at a comfortable temperature. LPA observed at least a 2 day supply of perishable and 7 day supply of non-perishable food. Food was found to be stored in a safe manner with open items covered. LPA discussed with Admin that all open food items must be covered and labeled with date opened. Kitchen cabinet containing cleaning supplies was locked. All bedrooms were equipped with lighting, night stand, and chest of drawers. All bedrooms were clean and in good repair. Extra hygiene products and linens were available. Resident bathroom had required bath mat and grab bar. Water temperature in sink accessible to residents in care measured at 106.1 degrees F which is within the allowable range of 105 to 120 degrees F. Facility has 3 fire extinguishers all of which were last inspected during LPA visit today 3/26/2024. Carbon Monoxide detectors were tested and operational as indicated by inspection sticker by fire alarm on March 2023, LPA verified with Santa Rosa Fire Equipment Service Inc technician that facility will have the next service on 3/29/2024, appointment confirmed. Per LPA observation and Admin interview, facility’s quarterly disaster drills are performed with staff individually; however, the facility has not conducted a disaster drill for every employee within the past quarter. Per Health and Safety Code (HSC) 1569.695(c) A facility shall conduct a drill at least quarterly for each shift...documentation of the drills shall include the date, the type of emergency covered by the drill, and the names of staff participating in the drill (deficiency cited, see 809D). Facility has a backup generator for use during a power outage. Continued on 809C... Continued from 809... At approximately 11:30 am LPA conducted a review of seven [7] resident records. Four [4] out of seven [7] residents (R3, R4, R6, and R7) did not have current Appraisals. Per Title 22 regulation 87463 Reappraisals (c) The licensee shall arrange a meeting with the resident, the resident’s representative, if any, appropriate facility staff, and a representative of the resident’s home health agency, if any, when there is significant change in the resident’s condition, or once every 12 months, whichever occurs first...(deficiency cited, see 809D). At approximately 2:00pm LPA conducted a review of five [5] staff records. Per LPA and Admin review of Guardian roster, staff (S1) does not yet have fingerprint clearance, it is "In Process" per the Guardian roster. S1 has been employed at at facility beginning 8/23/2022. Per Title 22 regulation 87355(d) Criminal Record Clearance (d) All individuals subject to criminal record review shall be fingerprinted (deficiency cited, see 809D. Civil Penalty assessed). LPA advised Admin all employees must have fingerprint clearance in order to work at the facility. Admin agrees to have S1 cease working at the facility until fingerprint clearance is obtained and Guardian roster shows S1 as "Eligible-Clearance." S1 immediately left facility and ceased working. At approximately 12:00pm LPA and Admin conducted a spot check of medication and medication records. Medication is centrally stored in a locked cabinet in the kitchen. LPA and Admin observed pre-poured medications filled in respective residents' pill boxes for the next day and evening. Per Title 22 regulation 87465 Incidental Medical and Dental Care (h) The following requirements shall apply to medications which are centrally stored: (5) Each resident's medication shall be stored in its originally received container. No medications shall be transferred between containers (deficiency cited, see 809D). Mercedes Vegvary Administrator certificate 6003396740 expires 6/30/2025. Julius Vegvary Administrator certificate expired 2/18/2023; per LPA review Admin cert for Julius is currently in pending status. All fees are current as of this time. Appeal rights given and discussed with Licensee. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties.
