Type of visit: Required - 1 Year
At approximately 8:40 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year visit. Administrator Phoebe Mungai was not at the facility during the inspection due to a prior engagement. Gracious Living at Porter Creek LLC is Licensed as a Residential Care Facility for the Elderly (RCFE). The facility is a single story ranch house with a large deck and yard area. The facility has an approved fire clearance for six (6) non-ambulatory residents, two (2) of which may be bedridden. The facility has a Hospice Waiver for three (3) residents. Upon arrival, LPA was informed that there were five (5) residents in care. LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 9:15 AM, LPA toured the facility. All exits were clear and unobstructed. The facility's three (3) fire extinguishers were last serviced and tagged on 3/1/2026. The facility was sufficiently lighted. LPA inspected four (4) resident bedrooms and found all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Toxins were observed to be stored inaccessible to residents. Facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility is conducting fire and emergency drills quarterly. The last disaster drill was conducted on 4/12/2026. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors were tested and observed to be operational. Continued on 809-C... ...Continued from 809 At approximately 10:15 AM, LPA reviewed five (5) resident files. Five (5) of five (5) resident files were observed to be well organized and thorough with all required documentation. LPA reviewed five (5) staff files. Five (5) of five (5) staff files were observed to be well organized with all required documentation including First Aid and CPR certification and proper training documentation. LPA audited Medication for three (3) residents. LPA observed all medications to be centrally stored, secure and with proper documentation. The facility does not handle resident’s monies for personal and incidental items. Phoebe Mungai's Administrator Certification 7030487740 is current with an expiration date of 2/9/2027. LPA requested the following documents be submitted to Community Care Licensing by 6/22/2026: LIC 500 Personnel Report LIC 610E Emergency Disaster Plan Updated Proof of Liability Insurance-In August when policy renews No deficiencies cited during today's visit. Type of visit: Required - 1 Year
At approximately 10:15 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year visit and was greeted by Caregiver (CG) Jane Muthoga. Administrator Phoebe Mungai arrived at 10:45 AM. Gracious Living at Porter Creek LLC is Licensed as a Residential Care Facility for the Elderly (RCFE). The facility is a single story ranch house. The facility has an approved fire clearance for six (6) non-ambulatory residents, two (2) of which may be bedridden. The facility has a Hospice Waiver for three (3) residents. Upon arrival, LPA was informed that there were five (5) residents in care and two (2) staff members, excluding the administrator, on-site. At approximately 10:50 AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 10:55 AM, LPA toured the facility with Administrator Mungai. All exits were clear and unobstructed. All three (3) fire extinguishers were last serviced and tagged on 3/25/2025. Sonoma County Fire Department inspected and cleared the facility on 5/12/2025. The facility was sufficiently lighted. LPA inspected four (4) resident bedrooms and found all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Toxins were observed to be stored inaccessible to residents. Facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility is conducting fire and emergency drills quarterly. The last disaster drill was conducted on 6/7/2025. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Continued on 809-C ...Continued from 809 Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors were tested and observed to be operational. At approximately 11:40 AM, LPA reviewed five (5) resident files. Five (5) of five (5) resident files were observed to be well organized and thorough with all required documentation. LPA reviewed five (5) staff files. Five (5) of five (5) staff files were observed to be well organized with all required documentation including First Aid and CPR certification and proper training documentation. LPA spot checked Medication for two (2) residents. LPA observed all medications to be centrally stored, secure and with proper documentation. The facility does not handle resident’s monies for personal and incidental items. During the inspection, residents were observed actively engaging with staff in common spaces and participating in various activities and mingling amongst each other. Residents and staff were also observed enjoying the afternoon in the shaded deck area. Phoebe Mungai's Administrator Certification 7030487740 is current with an expiration date of 2/9/2027. LPA requested the following documents be submitted to Community Care Licensing by 8/18/2025: LIC 500 Personnel Report LIC 610E Emergency Disaster Plan Updated Proof of Liability Insurance The facility is in compliance. No deficiencies cited during today's visit. Exit interview conducted. Copy of report and discussed and provided to Administrator Mungai. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Christi Coppo arrived unannounced to conduct a required Annual inspection and was greeted by Administrator Phoebe Mungai. Facility contact information was reviewed. At approximately 9:30am LPA and Admin toured the building and grounds. The facility was found to be clean and at a comfortable temperature. LPA observed at least a 2 day supply of perishable and 7 day supply of non-perishable food. Food items in refrigerator found open and not covered such as can of tuna and open onion. One zucchini found with white and blue spotting and 3 others with wilted withered skins, soft and mushy to the touch. One avocado with white spotting with concave skin. Kitchen cabinet containing cleaning supplies was locked. Kitchen drawer with sharp knives locked. All bedrooms were equipped with lighting, night stand, and chest of drawers. All bedrooms were clean and in good repair. Extra hygiene products and linens were available. Resident bathroom had required bath mat and grab bar. Bath mat in resident bathroom showed minimal spotting of a black and brown substance, LPA advised that all bath mats must be clean and free from substances that could be toxic to residents. Water temperature in sink accessible to residents in care measured at 112.7, 111.4, and 110.9 degrees F respectively which is within the allowable range of 105 to 120 degrees F. Facility has three [3] fire extinguishers, both of which were last inspected 3/22/2024. Smoke/Carbon Monoxide detectors located throughout the facility were operational. Facility’s last quarterly disaster drills were conducted on 6/19/2024. Facility has a backup generator for use during a power outage. At approximately 10:30am LPA conducted review of 5 staff records. All required documentation present. LPA discussed with Admin their materials used for training. Admin advised the materials are CDs from Community Care Options from 2011. Continued on 809C... Continued from 809... LPA and Admin discussed that materials were passed down from previous owner and staff watch the same CDs each year; therefore, no current or new information is being presented or learned. LPA and Admin discussed choosing new materials through an approved vendor, of which Community Care Options is one. LPA will send Admin a list of other approved vendors from which they may choose to select current and new materials for training. At approximately 11:30am LPA conducted a review of 4 resident records. R2 and R3 have half rails presents on bed but doctor's orders not on file. LPA and Admin discussed when 1/2 rails are present, a doctor's order must be kept on file. At approximately 12:00pm LPA and Admin conducted a spot check of medication and medication records. Medication is centrally stored in a locked cabinet. R1 did not have a Centrally Stored Medication Log (CSML) completed for their medications. Admin advised LPA that hospice provider keeps the CSML for R1, but did not know where hospice provider keeps it and Admin does not have a copy of it. LPA advised a CSML must be maintained at the facility for each resident even when they are on hospice (deficiency cited, see 809D). Phoebe Mungai Administrator Certificate 7030487740 expires 2/9/2025. All fees are current as of this time. LPA and Administrator discussed facility's Infection Control Plan and Emergency Disaster plan. No new updates. Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: LIC500- Personnel Report, LIC308- Designation of Responsibility, and a copy of current Liability Insurance Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation and the Health and Safety Code. Appeal rights given and discussed with Licensee. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Type of visit: Post Licensing
At approximately 9:00AM, Licensing Program Analysts (LPAs) Victoria Bertozzi and Christi Coppo made an unannounced Post Licensing inspection visit. LPAs met with Licensee Phoebe Mungai. At approximately 9:30AM, LPAs toured the building and grounds which was found to be clean and in good repair. LPAs observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerator, and freezer were clean, and food was stored properly. Toxins are stored in locked laundry room and locked under kitchen sink. Water temperature at faucets accessible to residents measured at 117.3F and 104.7F, respectively, which is within regulation. Fire extinguishers inspected 11/08/2022 were charged. Smoke detectors/Carbon Monoxide detectors were tested and functional. There was enough lighting in all common areas, resident rooms, and hallways. NIghtlights present in hallways. Medication is centrally stored in the locked kitchen pantry. At approximately 9:30AM LPAs observed facility to have enough supplies to operate for more than 72 hours in an emergency. At approximately 10:45AM, LPAs reviewed 4 resident records and found residents to have physician's reports, signed admission agreements, care plans and physician's orders on file. LPAs reviewed 4 of 7 staff records. CPR and first aid certificates were current.
