On today's visit, LPA met with Administrator, Brian Buenviaje and Assistant Administrator, Gloria Gibson who assisted with today's visit. The investigation consisted of interviews with Administrator, Staff #1, Resident #1- Resident #3, review of resident medications, and tour of facility. Regarding the allegation that staff yell at residents. Administrator and Staff #1 denied the allegation. They stated that staff do not yell at residents. Residents interviewed were unable to corroborate the allegation. 3 out of 3 residents stated that staff do not yell at residents. Regarding the allegation that the facility is dirty and staff does not clean resident room(s), Staff interviewed stated that the facility is cleaned on a daily basis, and residents' room(s) are also cleaned daily. Licensing Program Analyst (LPA) Cynthia Chan conducted an unannounced complaint investigation for the allegation listed above. LPA met with Administrator Brian Buenviaje and explained the purpose of the visit. The investigation consisted of the following: LPA obtained copies of the staff roster, resident roster, and pest control reports from July 2022 through October 2022. Interviews were conducted with the Administrator, 3 Staff, 5 Residents, Program Social Worker, and the C & K Pest Control owner. In regards to allegation - Facility has pests. It was alleged that a resident's leg swelled because of bed bugs. Per the Administrator, a resident reported seeing bed bugs in the room last week. However, they checked resident's room and none were observed. The Administrator provided a copy of their protocol when there are or suspect insects in a resident room. They are taking precautions to prevent any infestations of pests since residents like to bring in items from the community. Regarding the allegation that facility failed to seek timely medical treatment for resident #1 and facility is not assisting resident #1 with ADLs. The investigation was conducted by the department and consisted of interview(s) with Administrator, facility staff, hospital medical personnel, and Resident #1. Review of Resident #1's file including medical records was also conducted. The investigation revealed the following: Administrator and staff interviewed denied that resident #1 fell at the facility. Facility staff indicated that they send resident(s) to the hospital to be assessed if they fall. Facility staff sent Resident #1 to be assessed at the hospital on 10/12/20, when it was observed that resident #1 had swelling on her left ankle.
