Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced required 1-year visit on 03/16/2026. LPA was met by staff Celina Vasquez and explained the purpose of the visit. Facility is licensed to serve six (6) ambulatory and non-ambulatory residents aged 60 and above of which one (1) may be bedridden. Facility is approved to care for dementia residents. The property lot is made up of three (3) separate buildings (13971, 13973, & 13975 Telegraph Road). LPA OBSERVATIONS: The facility is a single-story dwelling located on a residential street and consists of three (3) resident bedrooms, one (1) bathroom, office, kitchen, dining room, living room, detached garage, and backyard. LPA used the Compliance & Regulatory Enforcement Tool (CARE) during today’s inspection. LPA observed this facility to be under construction, and zero (0) residents and zero (0) staff were observed inside the facility. According to staff Vasquez, this facility has been under renovation since 2022 and has not had any residents or staff in the facility. During file review, LPA Ramirez observed documentation sent to Monterey Park Regional Office that indicates no residents will be in the facility until all renovations are completed and licensing is notified. LPA observed construction materials on the inside and outside of the facility. Per staff Vasquez, no staff are assigned to this facility, therefore LPA did not review staff files or resident files. All annual fees are current for this facility. Staff Vasquez agreed to submit facility Emergency Disaster Plan for Residential Care Facilities for the Elderly LIC (610E) and liability insurance upon admission of the first resident. Type of visit: Required - 1 Year
License Program Analysts (LPA) Luis De Leon conducted an unannounced annual required visit. LPAs met with Assistant Celina Vasquez. The purpose of today’s visit was explained to facility personnel met during this visit. Facility is licensed to serve six (6) ambulatory and non-ambulatory residents aged 60 and above of which 1 may be bedridden. Facility is approved to care for dementia resident. Manager Cynthia Tadeo arrived later and joined the visit. The LPA use the Compliance & Regulatory Enforcement Tool (CARE) during today’s inspection. The visit consisted as follows: Ms. Tadeo informed LPA that there are currently no residents residing in Walnut Villa - Telegraph I and informed LPA that she is currently helping with the transition to a new ownership. The property lot is made up of three (3) separate buildings (13971, 13973, & 13975 Telegraph Road). LPA toured the facility at 13971 and observed that all interior rooms are under construction. The facility has three (3) bedrooms, one bathroom, office, a nook, kitchen, living room, and dining room. LPA toured the house and did not observe any residents living in the facility. Ms. Tadeo informed LPA that a letter was sent to Monterey Park CCLD office informing of the remodeling project to bring facility to compliance and ready to accept residents. New ownership has started the process to obtain a new license for one of the buildings. LPA explained to Ms. Tadeo that, if the new ownership plans to keep each building with its own individual license, the new ownership needs to apply for a new license for each building. LPA observed that new license process has been started for building 13975 only. No deficiencies cited. Apr 30, 2024Facility evaluation reportReport on file
Type of visit: Required - 1 Year
Licensing Program Analyst/ (LPA) Angelica Rea, conducted an unannounced annual inspection visit at the Walnut Villa - Telegraph I facility. LPA was allowed entry into the facility by Celina Vasquez, Administrator. LPA Rea explained to Ms. Vasquez the purpose of the required 1 year annual inspection visit. Ms. Vasquez informed LPA that there are currently no residents residing in Walnut Villa - Telegraph I. LPA toured the facility with Ms. Vasquez.. The residential house is one of three residential homes (on the property) has three bedrooms, one bathroom, kitchen, dining room, living room, and storage room. LPA toured the house and did not observe any residents living in the facility. Ms. Vasquez informed LPA that she faxed and mailed, the Monterey Park CCL Office, a letter stating that there are no residents living in the facility, and that the current owners of the facility are planning to refurbish the house for future residents to be admitted. LPA informed Ms. Vasquez to contact the Community Care Licensing office prior to beginning renovations on the property.. The Administrator stated that they still want to maintain the license and remain operational. No deficiencies cited.
