Licensing Program Analyst (LPA) Michael Cava conducted a subsequent complaint visit to the facility to conclude the investigation regarding the above allegations. The ten-day visit to this investigation was made on 02/03/24. LPA met with the administrator, Rima Agaroynyan, and advised her of the complaint. LPA Cava’s investigation consisted of interviews with facility administrator, hospital staff, facility staff, residents and record review of hospital and facility records. In regards to the allegations, it was reported that Resident 1 (R1) was discharged from a Skilled Nursing Facility (SNF) to the Residential Care Facility for the Elderly (RCFE) on or around 11/10/23. At the time of discharge from the SNF, R1’s skin was intact. On or around 12/6/23, R1 was sent to ER with a pressured sore on the lower back. The licensee indicated that they had noted a wound over the past weeks. Licensing Program Analyst (LPA) Michael Cava conducted a complaint visit to the facility to investigate the above allegations. LPA met with the administrator, Rima Agaronyan, and advised her of the complaint. Today's investigation consisted of a physical plant inspection, interviews and record review. At approximately 09:11am to 10:11am, LPA made a physical plant inspection to insure the health and safety of the residents in care. Between 10:11am to 11:30am, interviews administrator, staff (S1) and five (5) of five residents were made. Between 11:30am to 12:00pm, a record review conducted. Staff did not allow resident to leave their bed/Staff inappropriately pushed resident while in care: In regards to the allegations, it was reported that Resident 1 (R1) was just left in bed. While attempting to get up to walk out of the facility and try and stretch to ease their back pain, facility staff demanded that R1 stay in bed, pushing them back into bed. Licensing Program Analyst (LPA), Patrick Shanahan, arrived at the facility in response to the above mentioned allegation. The LPA was greeted by facility staff and called the administrator. The administrator arrived a short while later. The LPA was able to speak with all residents in care. 2 out of the 3 residents were verbal and were able to be interviewed. Residents interviewed denied ever being hit by a facility staff member or ever seeing another resident being hit by staff. The administrator was also interviewed and denied ever having a complaint from any resdident or family member regarding abuse. Staff interviewed also denied ever hitting any resident or ever seeing any other staff hit a resident. Based on interviews with staff, residents and the administrator, this allegation is deemed unsubstantiated. Exit interview conducted and report issued. Licensing Program Analyst (LPA), Patrick Shanahan, arrived at the facility in response to the above mentioned allegation. The LPA was greeted by the facility staff and the facility administrator arrived about 20 minuets later. The LPA explained the reason for the visit. The LPA was able to interview the facility staff and the administrator regarding this allegation. At 9:30 am, the administrator provided the LPA with a copy of the Residence and Services Agreement Short Term Respite/Guest Stay document. The document was signed and agreed to by the resident in question's (R1) responsible party. The document confirmed that R1 would stay at the facility between 7/20/22 and 7/30/22 for a reduced rate of $400 in total. After 7/30/22, the administrator informed the responsible party that the regularly daily rate of $150 would begin to accrue and eviction was discussed.
