Type of visit: Required - 1 Year
In conjunction with complaint control #31-AS-20260407091355, Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Myline Olivas, and explained the reason for the visit. At approximately 10:30am, with the assistance of the administrator, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are hardwired and interconnected. There are carbon monoxide detectors that functions properly. The fire extinguisher is located in the kitchen. The charge date is 08/11/25. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives were stored in a locked drawer in the kitchen. No cleaning supplies observed accessible and out in the open. A complete first aid kit is maintained in the kitchen. Bedrooms: There are six (6) bedrooms. Five (5) bedrooms are designated for the residents' use. One (1) bedroom is designated for staff. Of the five bedrooms designated for resident use, four (4) are private and one (1) is shared. Per STD 850, bedroom #5 has the fire clearance to retain one bedridden client. LPA observed that all resident rooms were properly furnished with appropriate beddings and linens with sufficient closet space and lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures, grab bars and non-skid mats. Hot water temperature was measured between 107 to 108 degrees Fahrenheit. No toxins or cleaning supplies present in each bathroom. Common Areas: These included the living room and dining area. The common areas were properly furnished with sufficient seating of coaches, chairs, table and television. The dining room table can fit up to six (6) residents. Furniture is in good repair. Floors were mopped and clean. Hallways/passageways are clear of obstruction. The auditory alarms at all exit doors were on and functional at the time of the visit. OFFICE/STAFF WORKSTATION: Staff office is located by the living room. Resident and personnel files are maintained in a locked filing cabinet there. Surrounding Grounds: Entry/exits to the front and backyard were free of obstruction. There is sufficient yard space in the back yard for outdoor activities. There is also a shed in the backyard that is used to store PPE and incontinent supplies. Shed is kept locked at all times. Back gate leads to an alley, but it is not used as a direct exit. There is no swimming pool or any other bodies of water. Laundry: The laundry area and detergents are located at the backyard. Detergents and cleaning supplies were observed stored and locked in a cabinet there during the day's visit. Resident Files: LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files: LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications: Medications are stored and locked in a drawer in the living room. Medication and Medication Records were reviewed for proper storage and documentation. Garage: No garage, only car port in the back yard. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Albert Salunga, and explained the reason for the visit. At approximately 9:00am, with the assistance of staff, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are hardwired and interconnected. There are carbon monoxide detectors that functions properly. The fire extinguisher is located in the kitchen. The charge date is 08/09/24. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer in the kitchen. Properly labeled medications were locked in one of the kitchen cabinets. A complete first aid kit is maintained in the kitchen. Bedrooms: There are six (6) bedrooms. Five (5) bedrooms are designated for the residents' use. One (1) bedroom is designated for staff. Of the five bedrooms designated for resident use, four (4) are private and one (1) is shared. LPA observed that all resident rooms were properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 108 to 110 degrees Fahrenheit. No cleaning supplies observed in the bathrooms during the day's visit. Common Areas: These included the living room and dining area. The common areas were properly furnished. Furniture is in good repair. Floors were mopped and clean. Hallways/passageways clean of obstruction. The auditory alarms on main exit doors were on and functional at the time of the visit. OFFICE/STAFF WORKSTATION: Staff office is located by the living room. Resident and personnel files are maintained in a locked filing cabinet there. Staff Files: LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications: Medications are stored and locked in a drawer in the living room. Medication and Medication Records were reviewed for proper storage and documentation. The licensee/administrator, Myline Olivas was called and came to the facility shortly after. At approximately 10:00am, with the assistance of staff, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are hardwired and interconnected. There are carbon monoxide detectors that functions properly. The fire extinguisher is located in the kitchen. The charge date is 08/09/24. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer in the kitchen. Properly labeled medications were locked in one of the kitchen cabinets. A complete 1st aid kit is maintained in the kitchen. Bedrooms: There are six (6) bedrooms. Five (5) bedrooms are designated for the residents' use. One (1) bedroom is designated for staff. Of the five bedrooms designated for resident use, four (4) are private and one (1) is shared. LPA observed that all resident rooms were properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 109.8 to 112.7 degrees Fahrenheit. No cleaning supplies observed in the bathrooms during the day's visit. Common Areas: These included the living room and dining area. The common areas were properly furnished. The auditory alarms on main exit doors were on and functional at the time of the visit. OFFICE/STAFF WORKSTATION: Staff office is located by the living room. Resident and personnel files are maintained in a locked filing cabinet there. Surrounding Grounds: Entry/exits were free of obstruction. There was sufficient yard space in the back yard for outdoor activities. The laundry area and detergents are located at the backyard. The fire extinguisher is located in the kitchen. The charge date is 5/1/2022. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer in the kitchen. Properly labeled medications were locked in one of the kitchen cabinets. Bedrooms: There are six (6) bedrooms. Five (5) bedrooms are designated for the residents' use. One (1) bedroom is designated for staff. Of the four resident bedrooms, four (4) are private and one (1) is shared. LPA observed that the three rooms in use by residents were were properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured at 112.7 degrees Fahrenheit. No cleaning supplies stored in the bathrooms. Common Areas: These included the living room and dining area. The common areas were properly furnished. The auditory alarms on all exit doors were on and functional at the time of the visit. OFFICE/STAFF WORKSTATION: Staff office is located by the living room. Resident and personnel files are maintained in a locked filing cabinet there. Surrounding Grounds: Entry/exits were free of obstruction.
