Allegation investigated: Staff did not seek timely medical attention for resident. Resident sustained unexplained bruises while in care. Staff did not ensure resident’s care needs were being met.

Licensing Program Analyst (LPA) Abeye Duguma conducted a subsequent complaint visit to the facility to investigate the above allegations. LPA met with administrator, Chery Monje-du, and explained the reason for the visit. --- Staff did not seek timely medical attention for resident. It was alleged that resident was covered in a rash and had open wounds that were bleeding. To investigate the allegation, on 05/08/2024, LPA interviewed three (03) staff from around 12:30 PM to 1:30 PM and interviewed two (02) out of four (04) residents from around 1:30 PM to 2:15 PM. On 08/22/2024 at around 10:30 AM, LPA requested additional documents. During interviews with staff, all staff stated Resident #1 (R1) did not have any bleeding from open wounds but that they are aware of the rash. All staff stated the rash comes and goes. It was alleged that staff are not feeding residents resulting in malnourishment and dehydration. To investigate the allegation, on 05/08/2024, LPA conducted a physical plant tour at around 11:30 AM, interviewed three (03) staff from around 12:30 PM to 1:30 PM and interviewed two (02) out of four (04) residents from around 1:30 PM to 2:15 PM. During the physical plant tour, LPA observed facility staff preparing and serving meals. LPA also observed more than seven (07) days perishable and two (02) days non-perishable foods. Licensing Program Analyst (LPA) Abeye Duguma conducted an unannounced initial complaint visit to the facility. LPA met with Licensee, Chery Monje-Du, and explained the reason for the visit. --- Staff did not keep toxins inaccessible to persons other than employees. It was alleged that staff left magnet key that stores toxins accessible to residents in the main restroom on a paper towel. To investigate this allegation, on 12/06/2023, LPA made observations at around 12:30 PM and interviewed three (03) staff from around 1:00 PM – 2:30 PM. LPA observed cleaning supply storage cabinet to be locked and keys in possession of staff. During interviews, Staff #1 stated that they only heard about the incident, Staff #2 stated that they did not know anything about the incident and Staff #3 stated they are aware of the incident and took steps to immediatley train and remind staff not leave the magnetic key in a places accessible to residents.
