Licensing Program Analyst (LPA) Erica Mosley conducted a Pre-licensing visit and met with the Applicant Viktorita Mardoian and Administrator Meri Zulalyan. This is a change of ownership (CHOW) facility with two (2) residents currently residing in the home. The application is for a Residential Care Facility for the Elderly (RCFE), for six (6) non-ambulatory residents; one (1) of which may be a bedridden resident(s). Advertising Dementia Special Care Program was submitted with the application. Fire Clearance was approved on 03/10/2026. The bedridden resident was approved to be in bedroom #1. Hospice waiver granted for five (5) residents. At 09:35 a.m., the LPA, and the applicant toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that the facility is in compliance with Title 22 Regulations. COMMON AREAS: The living room area is equipped with a television and is furnished appropriately. Required documents were posted in the common area.The emergency telephone numbers are posted in the common hallway. At 10:13 a.m. smoke, fire doors and carbon monoxide alarms were tested and functioned properly at the time of the visit. Medications were stored and locked in a cabinet adjacent to the refrigerator in the kitchen area. The residents’ and staff files are stored and locked in a cabinet located in the dining area. Activities were observed in the common areas. LAUNDRY: There is a laundry area equipped with a washer and dryer located inside a small, locked shed located outside the facility. Detergents and cleaning supplies are stored inside and next to the washer and dryer. KITCHEN: Kitchen knives are stored locked and inaccessible in a locked kitchen drawer. The facility has a sufficient supply of two (2) day perishable and seven (7) day non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. The supply of dishes is adequate. Appliances in the kitchen were clean and all appeared functional. Report Continued on LIC 809-C PAGE 2... (PAGE 2) Report Continued from LIC 809-C... Kitchen cleaning supplies will be stored and locked under the kitchen sink. Hot water temperature was recorded at 114.7 degrees Fahrenheit. Trash cans have a tight-fitting lids. There were no pesticides or toxins stored near food, or preparation area. The first aid supplies were complete, including a thermometer and a current version of a first aid manual. A fire extinguisher is located in the kitchen area, which was purchased on 05/06/2026. Auditory alarms at the entrances and exits were observed and functional at the time of the visit. BEDROOMS: There are three (3) total bedrooms in the facility; All bedrooms are designated as shared, double occupancy, resident rooms. All bedrooms were supplied with the required bedding and linens. There was sufficient lighting as well as closet and drawer space available. All window screens were clean and maintained in good repair. There is no staff room and Administrator stated that staff remain awake at night. Bedroom #1 is approved for bedridden and observed to have an exit and fire door. The emergency exiting plans/sketch are posted in every room. BATHROOMS: There is one (1) full bathroom, with shower area, no bathtub. Bathroom is equipped with toilet and shower grab bars, and slip resistant surface / mat. There are sufficient supplies of towels, paper goods and personal hygiene supplies. Hot water delivered was recorded at 114.1 degrees Fahrenheit. SURROUNDING GROUNDS/OUTDOOR AREA: The exterior passageways were relatively clean. The patio is furnished with outdoor furniture for residents’ use, and shade is available. The building has a central entrance for residents and visitors. The left passageway is cleared and has a self-latching gate. The self-latching mechanism is located inside the gate. An auditory device is in place to monitor the gate exit. There is a swimming pool which is gated, locked and inaccessible to residents in care. There is a shed located at the end of the patio, which was observed to be locked at the time of the visit. Residents will be using the front/main door and side gate as way to getting to the back yard. At the time of the visit the LPA observed one (1) Additional Dwelling Unit (ADU). The unit is identified by the address #8828, and is located on the right-hand side of the facility. The ADU unit is separated from the main address. The ADU was not part of Fire Clearance and is not affiliated with the facility. Report Continued on LIC 809-C PAGE 3... (PAGE 3) Report Continued from LIC 809-C PAGE 2... INFECTION CONTROL/ EMERGENCY DISASTER PLANNING: During today’s visit the LPA reviewed the facility’s infection control practices and the facilities emergency disaster plan. Both documents were observed to be complete and updated annually as required. The facilities policies and procedures, as they pertain to infection control and emergency planning meet the regulatory standard. RECORDS: During the visit LPA reviewed the facilities Administrator's file and Licensee representatives file. Both files were reviewed for, but not limited to: personnel records, health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All records were in order. DOCUMENTS: Documents obtained during the visit include: LIC 500 facility roster and LIC 9020A Resident roster and copy of the Limited Liability insurance. At the time if the visit the LPA reviewed the facilities contact information on file including phone numbers, and email address. Licensee Representative confirmed that all information is accurate. During todays visit the applicant completed Component III orientation. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating under the new license until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect the approval of your license. Pre-Licensing is complete. No citations issued. Exit interview was conducted and reviewed with the applicant. Identification of the Applicant and Administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, Applicant and Administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB analyst confirmed Applicant and Administrator’s understanding of following areas: 1.
