Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Administrator Lorena Palafox and explained reason for visit. The facility is licensed to serve six (6) non-ambulatory residents in the age range of 60 and over. Approved for Dementia residents. The facility currently has no residents or staff. The facility is a single-story home, located in a residential area of Lakewood. A tour of the single-story facility included the 3 resident bedrooms (2 residents per room), 1 staff office, 2 bathrooms, living room, kitchen, dining area, attached garage, front yard and backyard. LPA Gutierrez conducted the tour with Administrator and observed the following: sufficient food supplies for at least 2 days of perishables and 7 days of non-perishables were observed in the kitchen. Kitchen appliances are clean and were operating at the time of the visit. Sharps were observed locked in a kitchen cabinet. Chemical and cleaning solutions are kept locked in the garage. The First Aid kit is kept in the kitchen, and it is fully stocked with all required items including a current manual. Clean towels and extra clean linen were observed in the hallway closet. Dining and living room have sufficient lighting and sitting area. Medications will be kept locked in a kitchen cabinet. All bedrooms have all required furniture, lighting, and bedding. The bathrooms are clean and have the required grab bars in the shower and near the toilet for non-ambulatory residents. The showers have non-skid materials. Auditory devices were seen on all exit doors which are required for dementia residents and were operating at the time of the visit. The water temperature was tested in both bathrooms and measured at 125.9 F and 126.8 F not within the required 105-120 degrees. Administrator was advised to lower water temperature. A fire extinguisher was observed in the kitchen and are fully charged. Smoke detectors were observed throughout the facility and were operable during the visit. Carbon monoxide was observed in the hallway and was operable during the visit. The front yard and backyard are clean. There is a shaded area with seating in the backyard. No pools or bodies of water were observed at the facility. Passageways and exits are free of obstruction. Resident and staff files will be kept locked in the staff office. Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during the visit. Licensing Program Analyst (LPA), Mayra Cota, conducted an announced annual visit. LPA contacted, DeAngela Santos, Licensee via phone call and explained the reason for the visit. Lorena Palafox, Administrator, arrived to the facility thereafter and assisted with today's visit. Facility is licensed to serve (6) non-ambulatory residents in the age range of 60 and over. Facility currently has no residents or staff. The facility is in a residential area of Lakewood, CA. During the visit, LPA toured the facility's indoor and outdoor environment and reviewed one (1) staff file. The facility is a single-story house which consists of: living room, dining area, kitchen, (3) resident bedrooms, (2) bathrooms, office/medication room, laundry area in the garage, front and backyard/patio, and attached garage. Facility was observed clean and all walkways and passageways are free from debris and clear of obstructions. Front and backyard are well maintained and the backyard patio has a shaded area readily available for residents. Patio furniture is in good repair. Living room and dining area have enough seating for residents. Kitchen appliances are clean and were operating at the time of the visit. Sharps were observed locked in a kitchen cabinet. Chemical and cleaning supplies are kept locked in the garage. Sufficient food supplies for at least 2 days of perishables and 7 days of non-perishables were observed in the kitchen. First Aid kit is kept in the kitchen, was observed stocked with all required items including a current manual. Clean towels and extra clean linen were observed in the hallway closet. Resident bedrooms have the required furnishings and bed linen, ready for residents to use. Bathrooms were inspected and were observed clean and sanitary. Water temperature was measured and observed to be within compliance range of 105 - 120 degrees F. Personal hygiene products are readily available and stored in bathroom cabinet. ****Continues on LIC 809-C The required grab bars and non-skid mats were observed in the shower. Auditory devices were seen on all exit doors which are required for dementia residents and were operating at the time of the visit. A fire extinguisher was observed in the dining and was observed charged. Additional emergency preparedness supplies are kept in a closet by the main entry way of the home. Smoke detectors were observed throughout the facility and were operable during the visit. Administrator file was reviewed and contained relevant documentation. Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during the visit.
