Type of visit: Required - 1 Year

On 02/24/2026, Licensing Program Analyst (LPA) Marisa Chiarelli, arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection. LPA met with Facility Administrator, Aaron Elliot and explained the purpose of the visit. LPA Chiarelli and Administrator toured facility together to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, backyard, shed, and common restrooms. LPA observed the facility to be clean, in good repair and odor-free and each bathroom to have the necessary grab bars, non-skid flooring or shower chair, paper towels, and trash can with lids. Facility has a 2-day perishable and a 7-day non-perishable amount of food and sharps to be locked. Hot water temperature was measured at 116.8 F. LPA observed several fire extinguishers all of which were serviced in April 2025. Facility has sprinkler system with fire detectors and carbon monoxide detectors system in one. Facility has additional carbon monoxide detectors in common areas and hallways. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA reviewed a total of five (5) residents' files and four (4) staff files which contained all the required documentation. LPA Chiarelli reviewed four (4) resident's medications. Facility has own medication room locked which only staff have access too. Facility uses an electronic E-MAR. Several topics were discussed. Type of visit: Prelicensing

On 01/15/2025 Licensing Program Analyst (LPA) Ivan Avila arrived announced at the facility for a Pre-licensing inspection. LPA met with Administrator, Aaron Elliot and discussed the purpose of the visit. During today's visit, LPA Avila and Administrator toured the interior and exterior of the facility. Areas toured included but not limited to common areas, resident bedrooms, bathroom, kitchen, and outside of the facility. LPA observed cleaning products and other toxins to be locked away. LPA observed knives and sharps found to be locked and secured in the kitchen area inaccessible to residents in care. There are seven (7) fire extinguishers that are serviced and up to date. LPA observed the facility to have the required postage on the wall. LPA and Administrator complete the pre-licensing tool together and found the facility to be in compliance. Component III was waived during today’s inspection as Aaron Elliot is currently Administrator certified and has existing facilities. Applicant has satisfied all requirements in accordance to Title 22, California Code of Regulations. Application is pending and LPA will forward findings to the Centralized Application Bureau (CAB) for final review and approval. CAB will further contact applicant on final status of application. Identification of the applicant(s) and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant(s) and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of following areas: 1.
