Allegation investigated: Staff denied client access to facility.

At approximately 10:15 a.m. on 08/19/26 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced complaint visit. LPA met with the administrator and disclosed the reason for the visit. To investigate the allegation above, LPA conducted a file review at 4:00 p.m. on 08/18/26 of eviction notices and a telephone conversation with the administrator. Today, LPA toured the facility inside and out at 10:25 a.m., interviewed the administrator and four (04) residents between 10:30 a.m. and 12:00 p.m., and conducted a record review of pertinent records, including but not limited to an admission agreement, medical assessment, care plan, sign-in sheets, eviction notice, and staff and client rosters at 12:30 p.m. Regarding the allegation "Staff denied client access to facility" it was alleged Resident #1 (R1) was not allowed back in this facility after being denied admission to their new facility. File review of R1’s eviction notices revealed the administrator issued multiple 30-day evictions to R1 on 06/01/26 and 07/26/26. Unsubstantiated Interview with the administrator at 10:30 a.m. today revealed R1 violated multiple house rules, so they worked with R1’s Care Coordination Agency (CCA) to find a new home. On 08/11/26, R1 was discharged from the facility and took all belongings with them. R1 returned about two (02) hours later after they were denied admission to the potential new facility. The Administrator had R1 wait with their CCA in front of the facility for about one (01) hour while the Administrator requested guidance from the Department. File review verified the phone conversation on 08/11/26 between the administrator and the Woodland Hills-South Regional Office. The Administrator readmitted R1 to the facility after that phone conversation. Interviews with four (04) out of four (04) residents revealed they were all comfortable and well cared for in the home. Zero (00) out of four (04) stated they had been denied access to the facility. R1 was unavailable for interview. Record review of R1’s files revealed no pertinent information to the investigation. Based on interviews and record review, there is insufficient evidence to verify the above allegation. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. No immediate health or safety concerns observed during today’s visit. Exit interview conducted. Licensing Program Analyst (LPA) Michael Cava conducted a Pre-Licensing Inspection with the applicant/administrator, Gayane Manukyan, and her consultant, Ripa Tavitian. An Application to operate a Residential Care Facility for the Elderly (RCFE) was received by Community Care Licensing (CCL) on June 18, 2025. The facility is a one story building. A fire clearance was approved on July 7, 2025 for five (5) non-ambulatory residents and one (1) bedridden resident, for a total capacity of six. The applicant is also requesting a hospice waiver to retain six (6) residents. The smoke alarms are hard wired and inter-connected. The carbon monoxide detector is functional and installed in the hallway. The fire extinguishers is located by the entrance. It was purchased on June 21, 2025. A tour of the physical plant was initiated at approximately 10:00am and the following was observed: KITCHEN: The kitchen area is equipped with a refrigerator, stove/oven, and sink. At this time, perishable food items not required as there are zero (0) residents. Applicant advised to purchase perishable food items once facility is licensed and operational. There is an adequate supply of nonperishable/emergency food supply. BEDROOMS: There are four (4) bedrooms designated for resident use. Bedroom #1 and #2 are private. Bedrooms #3 and #4 are shared. Bedroom #1 is approved for bedridden (per STD 850). The applicant furnished the resident bedrooms with beds, night stand, chairs, dresser, bedding and linen. The bedrooms have sufficient lighting and closet space. BATHROOMS: The facility has three (3) bathrooms. Two (2) bathrooms are designated for resident use, and one for staff. Bedroom #2 has it's own bathroom. The bathrooms designated for resident use were observed to have the proper fixtures, grab bars, and non-skid mats. The hot water in resident bathrooms were measured between 108 to 111 degrees. COMMON AREAS: These included the living room and dining room. The living room is furnished with adequate seating, couches, table and television. The dining area has a table large enough to seat up to six (6) residents. There is no fireplace. Floors were clean and in good repair. Passageways and hallways were clear of obstruction. LAUNDRY: The laundry area is located at the side of the home. MEDICATIONS: The medication cabinet is located by the dining room. LPA observed storage bins for resident use. A complete first aid kit and manual is also maintained in the medication cabinet. GARAGE: There is no garage. SURROUNDING GROUNDS: The driveway, passageways and entrance to the home were clear of obstruction. All entry and exit doors have a functional auditory alert when the doors open. The backyard of the facility has a patio and backyard furniture to accommodate the six (6) residents. The facility backyard has sufficient yard space. There is no swimming pool or any other bodies of water. In addition to the Pre-Licensing inspection, a Component III power point presentation was held at the start of the visit. Pursuant to Title 22, Division 6 of the CA Code of Regulations, the facility's physical environment is compliant and ready for licensure.
