Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with staff who contacted Administrator who arrived to the facility shortly afterwards. Today's census is five residents in care with three residents on hospice services. Facility is currently licensed for six non-ambulatory residents with a hospice waiver for four. LPA observed Administrator Certificate #7007681740 to be active with expiration date of February 23, 2026. Today's inspection, LPA and Administrator conducted a tour of the facility to ensure the health and safety of residents in care. LPA observed facility to have Community Care Licensing and Long Term Care Ombudsman compliance poster to be posted in a conspicuous space. LPA observed residents to be in their private rooms. LPA observed facility to be at a comfortable temperature for residents in care at 73* F. LPA observed carbon monoxide alarms to be in working condition. Fire extinguisher was recently serviced on July 14, 2025. LPA observed residents rooms to be free of medications, toxins, and sharps. LPA observed bathrooms to be clean with non-skid mats and paper towel for resident usage. LPA observed the kitchen to be free of pest and/or rodents, with the presence of at least two days of perishable and seven days of non-perishable food for residents in care. LPA observed food to be kept in a safe manner away from cleaning supplies. File review was conducted. LPA observed all required documents to be complete. Emergency Disaster Plan was recently reviewed on May 15, 2025. LPA observed fire drills documented to be conducted quarterly. Liability insurance was observed to be current and active, compliance to Health and Safety Code. CARE tool was completed and found facility to be in compliance. No deficiencies cited. Type of visit: Required - 1 Year
On 11/26/2024, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual visit utilizing the full care tool. LPA met with Administrator and explained the purpose of the visit. Facility census is six residents in care with one on hospice services. Facility is licensed for six non-ambulatory and hospice waiver for four. LPA observed the presence of R1's hospice binder with communication and care notes, compliance to PIN 22-24 ASC. During today's visit, LPA and Administrator conducted a tour of the interior of the facility to ensure the health and safety of residents in care. Areas toured included but not limited to: six residents bedrooms, two bathrooms, laundry room, kitchen and the common areas. LPA observed facility to have ample supply of clean linens. LPA observed facility to have 2+ days of perishable and 7+ days on nonperishable foods. Sharps, toxins and medications observed to be locked. LPA observed fire extinguisher to be recently serviced on June 19, 2024. Emergency drills were conducted quarterly in January 2024, April 2024, July 2024, and October 2024. Emergency Disaster Plan observed to be reviewed and approved on May 1, 2024. Liability insurance observed to be active and in compliance to Health and Safety Code. LPA observed Administrator Certificate to be active on Community Care Licensing Division website, expiration date of 02/24/2024 to 02/23/2026. File review conducted for six resident records and three personnel records. As a result of today's inspection, no deficiencies cited. Nov 30, 2023Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On 11/30/2023, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced to conduct an annual inspection utilizing the full CARE inspection tool. LPA met with Administrator, Mariana Oltean, and explained the purpose of the visit. Facility census is six (6) residents in care, with three (3) residents on hospice services. During time of visit, LPA observed R1 to be leaving the facility with friends for lunch. LPA and Administrator discussed the current concerns at the facility. Administrator stated no concerns with staffing. Administrator informed LPA there is concerns regarding family disputes with R2's family members. LPA informed Administrator LPA will seek advisory from management to better assist Administrator. During today's inspection, LPA and Administrator conducted a tour of the facility to ensure the health and safety of residents in care. Areas toured including but not limited to: kitchen, living room, laundry room, two (2) caregivers room, six (6) residents private rooms, two (2) shared restrooms and the common areas. LPA observed sharps, toxins and medications to be locked and secured. LPA observed facility to have ample supply of nonperishable foods. During LPA's tour of the facility, no immediate health, safety and personal rights violations was observed. No deficiencies observed. LPA conducted a file review of four (4) residents records and four (4) personnel records. LPA observed residents and personnel files to be complete with the required documents. Full CARE inspection tool was completed and facility is found to be in compliance. At this time, LPA is requesting a copy of facility liability insurance to be emailed to LPA by Friday 12/08/2023.
