Unannounced Annual visit made out to this facility on 02/25/2026 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility live-in caregivers, Minerva Ramit and Necilei Ramit, who were briefly interviewed at this time. This LPA requested that they go ahead and contact the facility designated Administrator, Lolita Ramit, to inform her that CCL was present at this time. The facility designated Administrator, Lolita Ramit, was present at this facility and briefly interviewed at this time. Current census was 5 residents. It was learned that there were (4) residents under the care of hospice at this time. This facility does have an approved waiver to be able to accept and retain up to (4) residents under the care of hospice at any given time. It was learned that this facility has a program to be able to accept and retain dementia residents at any given time. It was learned that there were (5) residents diagnosed with dementia at this time. It was learned that there were (4) residents receiving services through home health at this time. It was learned that this facility does have a bedridden fire clearance to be able to accept or retain up to (2) residents deemed to be bedridden at any given time. There were (2) residents deemed to be bedridden at this time. Tour of this facility was conducted. Dining area, living area, and all other areas intended for resident use were toured. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Linen closet, located in facility hallway closet, was reviewed and observed to contain a sufficient supply of towels, sheets, and bedding able to meet the needs of the residents at this time. Kitchen area was toured. Kitchen drawers and cabinets were opened and reviewed. Food supply for 2-day perishable and 7-day nonperishable quantities was reviewed to make sure that they were in compliance at all times. Pantry area was toured. Garage area was toured Laundry area, located next to the garage area, was toured and observed to be locked at this time. A review of the bleach, detergent, and all other cleaning supplies was conducted to make sure that they were locked and made inaccessible to the residents at this time. Administrator certificate # 7015195740 for Lolita Ramit was observed to have an expiration date of 02/14/2028 and in compliance at this time. Medication cabinet, located in the facility kitchen cabinet, was observed to be locked and made inaccessible to the residents at this time. First aid kit, located in the medication cabinet, was reviewed. This LPA observed that it did contain all of the required components at this time. Fire extinguisher located hanging on the wall adjacent to the kitchen was observed to have been annually inspected by the local fire extinguisher company, Assured Fire Extinguisher, with the inspection date of 01/13/2026 and in compliance at this time. Facility resident bedrooms were toured. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Facility resident restrooms were toured. Grab bars and non skid mats were observed to be present and in good repair at this time. Hot water temperatures were taken to make sure that they were within the allowed range of 105-120 degrees at all times. A tour of the facility exterior grounds was conducted. A review of the facility perimeter fence, side gates, and all other exits was conducted. A review of (5) facility personnel records was conducted and noted on the following LIC 859. A review of (5) facility resident records was conducted and noted on the following LIC 858. Licensing Program Analyst (LPA) Renee Campbell arrived at the facility to conduct an unannounced annual inspection on 02/20/2025. LPA met with Lolita Ramit, Administrator (#7015195740) and explained the purpose of the visit. Upon entry LPA Campbell observed one resident in a wheelchair watching TV. A See Something Say Something and Ombudsman posters were displayed in the seating area near the kitchen. LPA Campbell inspected the physical plant including but not limited to the common area, kitchen, dining area, client bedrooms, client bathrooms, laundry room and outside courtyards of the facility to ensure compliance with Title 22 regulations. This facility is a single story building licensed to serve four (4) non-ambulatory residents, two (2) bedridden with a hospice waiver for three (3). Referrals are usually received from Transitions for Life. LPA Campbell observed the facility to be free of odor, clean and in good repair. LPA Campbell observed bedrooms to be properly furnished with appropriate bedding and lighting. There is a pool that LPA Campbell observed surrounded by a gate that is locked. LPA Campbell observed sufficient seven-day non-perishable and two-day perishable food supplies. Hot water temperature was measured in the kitchen at 107 degrees Fahrenheit (F). The thermostat was set at 75 degrees F. LPA Campbell observed that the facility floor plan had not changed. The smoke alarm and carbon monoxide alarm were tested and found to be functioning. The fire extinguisher was last tested on January 10, 2025 and was fully charged. Medication was observed to be inaccessible to clients. The first aid kit was complete and contained scissors, tweezers and a first aid manual along with bandages. The thermometer was at the entrance to the facility for use by guests. LPA Campbell requested client and staff files for review. LPA Campbell reviewed 4 of 5 resident files and 4 of 7 staff files. All files were observed to be complete. Toxins were made inaccessible to clients in care and were stored in the laundry room. On 2/1/24 Licensing Program Analyst (LPA) Maja Jensen arrived at facility unannounced visit to conduct a required 1 year annual inspection. LPA Jensen met with Licensee Leilani Ramiro and explained the purpose of today's visit. LPA Jensen toured the facility grounds. All paths were observed to be free of obstruction. The grounds were well maintained. There are shaded areas and patio furniture for outdoor activities. All screens were observed to be in good repair. There is a swimming pool on site that is surrounded by a fence that is regulation height. At the time of this inspection the fence surrounding the swimming pool was observed to be locked. LPA Jensen toured the physical plant and observed it to be sanitary and free of odor. All required postings were observed. There was adequate lighting and furnishings throughout. The facility maintains an adequate linen supply. The bathrooms are equipped with grab bars and non-slip mats in the showers. The bathroom water temperature was measured at 109 degrees Fahrenheit which falls within the required range of 105-120 degrees Fahrenheit. The thermostat was set 79 degrees which falls within the required range of 68-85 degrees Fahrenheit. LPA Jensen inspected the kitchen and observed a 2 day supply of perishable food and a 7 day supply of non-perishable food. Knives were observed to be locked and inaccessible to residents in care. All cleaning supplies and medications were locked and inaccessible to residents in care. The fire extinguisher was last serviced in January of 2024 and is in compliance. The carbon monoxide detector and smoke detector were observed to be in good working order. The facility conducts and logs regularly scheduled fire drills. There is emergency lighting and food available. The first aid kit was observed to be complete and in compliance. The Emergency Disaster Plan was reviewed and is current and in compliance. Continued on LIC 809C... LPA Jensen interviewed 3 of 6 clients and all were satisfied with every aspect of their care. LPA Jensen reviewed 4 staff files and observed staff 4 (S4) to lack finger print clearance. A deficiency is being cited and a civil penalty assessed for lack of criminal background clearance. LPA Jensen reviewed 3 of 6 client files and observed 3 of 3 files to lack Needs and Service Plans and contain Physician Reports that were in excess of a year old. Technical assistance is being provided in multiple areas on documentation required for resident files. LPA Jensen reviewed the Liability insurance and determined it to be current and in compliance. LPA Jensen reviewed the LIC 308 and it is in compliance. The inspection tool was used during the course of this visit. Deficiencies are being cited pursuant to the California Code of Regulations (CCR) Title 22, Division 6. Civil Penalties are also being assessed.
