Mar 13, 2026Facility evaluation reportReport on file

Type of visit: Prelicensing

Licensing Program Analysts (LPAs), Ivashia Wright and Mia Lankford conducted an announced pre-licensing inspection at the facility and met with Applicants, Aysan Nazeri and Nahid Asgharloo. Application: The inspection is for an initial Residential Care Facility for Elderly (RCFE) application. The Riverside County Fire Department approved a fire clearance for three (3) clients (1) ambulatory, (1) non-ambulatory and (1) bedridden on 9/17/2025. Buildings and Grounds: The home is composed of two (2) client bedrooms, (2) staff bedrooms a living room area,(3) bathrooms, a laundry room, kitchen and dining area, a garage, and front and back yard. The interior and exterior walkways of the home were observed to be clutter free with no obstructions. Smoke and Carbon Monoxide detectors are in operable conidtion. There is no gated pool and no weapons stored in the home. Client bedrooms are fully furnished, and privacy is available. Outdoor areas had sufficient room for activities and leisure. A washing machine and dryer were available and in working order. Water temperature measured at 118.3F. Storage and Supplies: Medications will be stored in a locked cabinet next to the kitchen inaccessible to any unauthorized individuals. Secured locked file cabinet located in the kitchen is available for facility files, staff files and client files. A complete first aid kit was observed to be available. Cleaning supplies will be stored away in a secured location in the laundry room. Linens, and equipment appeared to be in good repair and sufficient for the approved census. Fire extinguisher is available and fully charged with an expiration date of 08/27/2026. (Continued LIC 809C) Food Service: Utensils and dishware are sufficient for the requested capacity. The refrigerator and freezer are in working order. Sharps knives will be stored in a locked drawer available only to authorized individuals. Forms: The following signs were observed to be posted at the home: Personal Rights, Complaint information, Emergency disaster plan, the facility sketch, ombudsman’s poster. LPAs verified the Administrator's Certification, with an expiration date of 2/23/2028 and CPR certification with the expiration date of 8/28/2027 LPAs observed that the physical plant is clean, in good repair, and to be hazard-free during today’s visit. The applicant has completed COMP III orientation on 3/10/2026. LPAs determined the facility meets the operational requirements for licensure. The Pre-licensing inspection is complete and has satisfied all requirements in accordance with Title 22, California Code of Regulations. COMP II by CAB successfully completed Facility Type: RCFE Application Type: INTL Capacity: 3 Census : 0 Method: Telephone call with CAB COMP II Participants: Nazeri Aysan (Licensee/ Administrator), & Dianna Esparza (Analyst). Licensee & administrator participated in COMP II via Telephone call with CAB Analyst. Identification of licensee/administrator was verified by confirming driver’s license numbers. During COMP II, licensee/administrator confirmed the understanding of Title 22. Component II was successfully completed. Licensee/administrator were advised to email signed LIC 809 with copy of photo ID to CAB. During COMP II, CAB analyst confirmed licensee's/administrator’s understanding of following areas: 1. Facility operation: License type, client/resident populations, and program 2. Admission Policies 3. Staffing requirements & Training 4. Restrictive/Prohibited Health Conditions 5. General provisions 6. Emergency Preparedness 7. Complaints & Reporting 8.
