Licensing Program Analyst (LPA) Ivashia Wright conducted an unannounced annual required visit. Upon entry, LPA was greeted by Aron Oda, Administrator, and informed them of the purpose of the visit. At the time of the visit, there were two (2) staff members and four (4) residents present. Facility Overview: The facility is a two story home, three (3) bedrooms and one (1) bathroom, a dinning room, a living room, a kitchen, a laundry room (Upstairs), an outdoor area, and a garage. There is no gated pool and there are no firearms on the premises. Downstairs is designated for residents. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was measured at 105F, meeting the department requirements. LPA observed fire extinguishers to be fully charged with an expiration 9/8/2027. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate and CPR certification which expires 7/26/2028. Record Review and Resident/Staff Files: LPA reviewed files for three (3) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Two (2) resident files were reviewed and contained all required documentation. LPA observed first aid kit to be stored and inaccessible to residents. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPA reviewed medications for 2 (two) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last fire/earthquake drill conducted on 7/13/26, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an unannounced visit for a required annual inspection. The LPA was greeted by Caregiver Andrea Oda, notified them of the purpose for the visit and was allowed to enter the facility to conduct the inspection. Licensee/Administrator Aron Oda joined the visit at a later time. Facility Overview: The facility is a single- story building with 3 residents' bedrooms, 1 bathroom, a dinning room, a living room, a kitchen, a laundry room (Upstairs), an outdoor area, and a garage. There is no gated pool and there are no firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department's requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked in the kitchen and inaccessible to residents. The smoke detectors and carbon monoxide detectors were operable. LPA observed fire extinguisher to be in compliance with the department's requirements and with an expiration date of 09-08-2026. The water temperature was tested within regulations measuring 111.7 F Continued 809-C...... Care & Supervision/Administration: Adequate staff were present to supervise residents during the visit. The Administrator holds a current Administrator’s certificate with the expiration date of 01-04-2027 and CPR certification with the expiration date of 09-01-26 Record Review and Resident/Staff Files: LPA reviewed files for 2 staff members, confirming criminal clearance, updated training, and health screening. 3 residents' files were reviewed and contained all required documentation. LPA observed first kit to be available for the residents in care. The residents and staff files were kept locked and inaccessible to unauthorized individuals. Health-Related Services/Incidental Medical Services: All residents' medications were securely locked in a cabinet and located in the kitchen. LPA reviewed medications for 3 residents confirming that all medication were listed and accounted for. Information obtained after records review and interviews revealed the facility is not approved for bedridden but admitted R3 who is bedridden. Citation will be issued Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last emergency drill conducted on 07-06-2025, which met the department's requirements. All facility exits were clear of obstructions. Due to a zero tolerance for unapproved fire clearance, the facility is being assessed an immediate civil penalty in the amount of $500, as they are operating without the proper fire clearance. Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced annual required visit. LPA was granted entry and met with Administrator, Aron Oda, who was informed of the purpose of the visit. At the time of the visit there was two () 3staff and five (5) residents present. The facility is a two story home, three (3) bedrooms and one (1) bathroom downstairs are designated for residents on the first floor. No pool or firearms are present. Infection Control: LPA observed hygiene supplies, PPE equipment, and cleaning supplies to do regular cleaning of the facility. LPA reviewed the facility's infection control plan which met department requirements. LPA reviewed staff records and found that all staff had infection control training. Physical Plant: Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair were present. The outdoor area was observed to be free of hazards with outdoor furniture and shaded area for residents. Laundry equipment was observed to be in good working condition. The sharp and dangerous objects were observed to be locked and inaccessible to the residents. The smoke detector and carbon monoxide was operational, and the hot water temperature 105.2F. Care & Supervision/Administration: Adequate staff are present for the supervision of residents during the visit. LPA also reviewed the staff scheduled showing adequate staff coverage. The listed administrator, possesses a current administrator's certificate. The facility liability insurance policy was out of date, the licensee agreed to have this renewed and submitted to LPA. Deficiency was cited. Technical note was documented for the licensee to send the grant deed to the LPA by September 27, 2024. Record Review and Resident/Staff Files: LPA reviewed three (3) staff files and training. All staff have criminal clearance and updated training along with CPR/First Aid Certification. Three (3) resident files were reviewed. One (1) resident who did not have an updated LIC602, the licensee coordinated with the resident's responsible party during the visit and established a date when the resident would see their primary care physician. (1) resident did not have a signed Needs and services plan. Deficiency was cited for the resident to have an updated file. One (1) resident is currently receiving home health services, who did not have a plan of care from the home health agency on file, but does have a plan of care with the facility. Technical note was documented and the licensee agreed to send this plan of care to the LPA by September 27, 2024. Health Related Services/ Incidental Medical Services: All resident medication was locked in a kitchen cabinet. LPA reviewed medications for three (3) residents and found all medication listed on MARS and all required labeling was found to be in place. Disaster preparedness: LPA reviewed the facility's emergency and disaster plan. LPA reviewed documentation showing the facility's last fire and earthquake drill was on 07/20/2024 which met the department requirements. LPA observed all facility exits were clear from obstructions. LPA observed emergency supplies in the garage and first aid kit with all required items.
