On 1/15/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit. LPA Flores was greeted by Administrator Luz Price and explained the purpose of the visit. LPA was granted entry and conducted a tour alongside Luz. During the tour, LPA observed the following: The facility is a single-story structure which consist of (5) five resident bedrooms, (1) one staff bedroom, (2) two bathrooms, living room, kitchen, dining area, laundry room, garage, and backyard. LPA observed a shaded outside patio with seating. Indoor and outdoor passageways were free from obstruction. LPA observed a television stand which stored crafts and games to encourage resident socialization. Kitchen area was observed to be clean and free of litter. The facility maintained a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable food. Cleaning supplies and other disinfectants were observed to be stored in a locked cabinet under the kitchen sink. LPA observed an additional locked cabinet which stored knives and other sharp items. Medication is centrally stored in a locked closet. Resident bedrooms were observed to have the required bedding furniture and functional lighting. Bathrooms were observed to be in operating condition. Shower areas were equipped with the required grab bars and slip-resistant material. LPA observed a fully charged fire extinguisher near the entrance. There are no pools and/or bodies of water. Per Administrator, there are no firearms and/or ammunition on the premises. Resident records reviewed included but not limited to signed admission agreements, pre-placement appraisals, needs and service plans, medical assessments, identification and emergency information, and personal rights. Staff records reviewed included but not limited to valid first-aid/CPR certification, criminal record clearance or an exemption, health screenings, TB test results, employee rights, and training verification. Type of visit: Case Management - Health Checks

On 09/10/2025, Licensing Program Analyst (LPA) Janette Romero arrived unannounced to the facility to conduct a case management visit to deliver an Immediate Exclusion order for Staff, Ismael Loyola. LPA was greeted and granted entry by Caregiver, Jocelyn Baker who was informed of the purpose of the visit. During the visit, Administrator Luz Price arrived to the facility to meet with LPA and was also informed of the purpose of the visit. LPA handed Administrator Price the Immediate Exclusion order for Loyola dated 09/09/2025. Per Administrator Price, Loyola has not worked in the facility since June 2024. LPA toured the facility with Caregiver Baker and did not observe Loyola to be present or any imminent health or safety concerns. No deficiencies were issued during today’s visit and Administrator Price has provided a copy of the updated staff roster. Licensing Program Analyst (LPA) Kathleen Banrasavong arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry. LPA began inspection with introduction and visit purpose. Upon arrival LPA learned that five (5) residents live at this facility. There was one (1) staff member present. The Administrator, Luz Price was advised of the annual and came to conduct and completed the facility tour. Client Records/Incident Reports/Clients Rights Information: LPA reviewed client records. Five (5) records were reviewed. LPA reviewed for identification and emergency information, admission agreement, medical assessment, and TB test results, needs and service plans, placement, functional assessment, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. Personnel Records/Training/ Staffing/ Administration: LPA reviewed employee records. Two (2) records were reviewed. LPA reviewed employee records for first aid certification, criminal record clearance or an exemption, health screening and TB test results, employee rights, training verification, and current administrative organization. Luz Price’s, Administrator’s certificate expiration date was 04/21/2024. Price submitted her renewal on March 2024 to the Department. Food Service: Food prep areas are clean and organized. Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables. Emergency food and water supply is present. There is a location for sharps in the kitchen. Physical Plant and Safety of Environment/Operational Requirements: LPA toured the facility inside and outside. LPA observed the facility to be clean and in good repair. The facility is maintained at 75 degrees for the client’s comfort. Lighting is sufficient for safety. Water temperature measured 108.0 degrees F. Laundry is done in the laundry room. There is a locked location for storing laundry soap, cleaning supplies and chemicals in the closet in the laundry room. All outdoor and indoor passageways are free of obstruction. Emergency lighting is available. There is a telephone working at this location. LPA dialed the facility’s landline number, which rang and was operable. The LIC 610, emergency disaster plan is maintained. There are no firearms at this facility. There is one (1) secured fireplace at this facility. There are zero (0) pools at the facility. There is one (1) secured gate that has a self-latching lock located on the northwest side of the house. LPA observed emergency supplies and two (2) first aid kits. The last emergency fire drill was conducted on 10/29/2024. Infection Control: The LPA observed the hand washing stations in the facility restrooms and kitchen had hand hygiene supplies and hand washing signs. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. LPA reviewed the facility's infection control plan which met department requirements. LPA reviewed staff records and found that staff had infection control training. Medications/Health Related Services/Incidental Medical Services: The medications are centrally stored. There is a locked cabinet allocated for medication storage. Centrally stored medication and destruction logs are maintained separately. LPA reviewed medication logs and observed that they were dispensed accurately. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. LPA observed seven dual (7) smoke detectors and carbon monoxide detectors throughout the facility. There were two (2) fire extinguishers on site, date charged was 06/28/2024. Pursuant to Title 22 of The California Code of Regulations Division 6, there are zero (0) deficiencies observed. On 1/24/2024, Licensing Program Analyst (LPA) Chinwe Nwogene arrived unannounced at the facility to conduct an annual inspection. LPA Nwogene met with Administrator, Luz Price who was informed of the purpose of visit. At the time of visit there was three #3 staff and five #5 residents present. LPA toured the facility inside and out with Luz Price. Tour included: Kitchen: LPA toured the kitchen and observed kitchen to be clean. Food is stored in a safe and healthful manner. Utensils and dishware are sufficient for the capacity. The refrigerator and stove are in working order. Sharps are stored in a locked kitchen cabinet, available only to authorized individuals. Trash cans has tight-fitting lid. Fridge, Freezer and all need appliances were present and shown to be in working condition and clean. Dining and Livingroom; LPA toured the dinning and Livingroom area. LPA observed area to be clean and furnitures in good condition. Temperature was 73 degrees Fahrenheit. Hallway: LPA toured the hallway and observed hallway to be clean with no pathway obstruction. LPA inspected the fire extinguisher and found it to be in compliance. Carbon monoxide & smoke detector were tested and functioning properly. LPA observed additional linens and hygiene items. Medication: Medications were labeled and stored in separate bins inside of a locked medication cabinet and are distributed according to physician orders. The first aid kit was complete. Bathroom: LPA toured hallway bathroom and observed bathroom to be clean and equipped with grab bar. There is also a good number of personal toiletries available for the residents in care. The hot water measured at 105 degrees Fahrenheit. Bedroom; LPA toured five #5 out of #5 resident bedrooms and observed bedrooms to be clean and furnished according to regulation, which includes proper furniture, dressers, chairs and lighting. Night lights were maintained throughout the facility. Resident bedroom #5 has a private bathroom. LPA observed bathroom to be clean and hot water was measured at 105 degrees Fahrenheit. Garage; LPA tour the garage and observed garage to be clean. Laundry; Washing machine and dryer are all in good repair and sufficient for the census. Cleaning supplies are stored away in the garage, inaccessible to clients. Backyard: LPA toured the backyard and observed backyard to be clean and furnitures in good condition. The backyard was free from obstruction and the side gate remain unlocked. No bodies of water were observed. Food Services: There are seven days non-perishable and two days of perishable food supply present, and all food was properly stored and available to residents. Fridge and Freezer are large enough to accommodate perishable foods. Records: All staff present have a criminal record clearance in file and are confirmed as being associated with the facility. Random staff and residents' records were reviewed. All required postings were posted near the entryway and throughout the facility. The administrator certificate expires on 4/21/2024. Interview; Three staff and three residents were interviewed. No deficiencies noted at the time of visit.
