Licensing Program Analysts (LPAs), Ivashia Wright and Abdoulaye Zerbo conducted an unannounced 1 year visit. LPAs were granted entrance by Licensee Leon Arizobal. The home is licensed to provide care to six 2 ambulatory and 4 bedridden adult residents. Administrator Ivan Arizobal arrived shortly after. The home consists of (5) bedrooms, (3) bathrooms. The facility has a formal dining room, kitchen, living room, and a garage. Currently, bedroom 4 is used as an office space. The facility is operating in the capacity and conditions approved by community care licensing. The Facility was observed clean and free of odors. Dishes and utensils were in sufficient supply and in good repair. Knives and other sharp objects are stored in a locked cabinet in the kitchen area. Disinfectant and other cleaning solutions were observed in a locked cabinet in the laundry room. According to Administrator, there are no firearms on the premises. Resident bedrooms had the required bedding, furniture, and lighting. Smoke and carbon monoxide detectors were observed to be operable. Facility's temperature was comfortable. Hot water measured within the department's requirements. Showers and toilets are operable. Linens and hygiene items are sufficient for residents. Each room is equipped with lamps and appropriate lighting to ensure residents’ comfort and safety. Non-perishable and perishable food is sufficient. Food is being prepared and stored properly. Facility has a variety of food available for residents. The facility does not have any residents and staff, therefore, there were no staff/resident files for LPAs to review. Administrator Ivan provided a copy of his Administrator certificate which expires 12/13/26 and CPR certificate which expires 1/27/28. No immediate health and safety concerns were observed during the visit. Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an announced visit for the purpose of confirming the recent changes to the facility. LPA received an updated approved fire clearance and updated facility sketch on 11-24-2025. Riverside County Fire Department approved fire clearance on 11-24-2025. LPA was greeted and granted entry by Administrator Ivan Arizobal. The facility is licensed for a capacity of 2 ambulatory and 4 bedridden residents. The facility has an approved hospice waiver for 6. LPA conducted a tour of the interior and exterior of the single story home, and observed that the home to now consists of 5 bedrooms, 3 bathrooms, each bedroom was fully furnished. The home was clean, clutter and odor free. Inside the home bedroom # 5 and #1 were approved for bedridden residents. Bedroom #2, #3 and #4 were approved for ambulatory residents. LPA observed for the facility food supply to meet the minimum requirements of having a 2 day supply of perishable and 7 day supply of non-perishable food items. No health and safety concerns were observed during today's visit. On 02/5/25 Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an unannounced one (1) year required visit. LPA was granted entry by the Administrator, Ivan Arizobal, who was informed of the purpose of visit. At the time of the visit there were no residents residing at the facility. The Administrator was observed to have obtained proper fingerprint clearance and was associated to the facility. LPA conducted a tour of the facility with Administrator Ivan Arizobal. The physical plant is a single-story structure that consisted of four (4) resident bedrooms, and three (3) bathrooms. The facility has a formal dining room, kitchen, living room, and a garage. Indoor and outdoor passageways were free of obstruction. Water temperature was measured at 105.8-degree Fahrenheit meeting the requirements. Dishes and utensils were in sufficient supply and in good repair. Knives and other sharp objects are stored in a locked cabinet in the kitchen area . Disinfectant and other cleaning solutions were observed in a locked cabinet in the laundry room. According to Administrator, there are no firearms on the premises. Resident bedrooms had the required bedding, furniture, and lighting. Smoke and carbon monoxide detectors were observed to be operable. The facility does not have any residents and staff, therefore, there were no staff/resident files for LPA to review. The Administrator told LPA that they are planning to accept residents this year. During today's visit, LPA did not observe any immediate violations or concerns. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Janira Arreola conducted an announced visit to the home in order to conduct an initial pre-licensing inspection. LPA met with Applicant, Ivan Arizobal. LPA conducted a walk through and reviewed the pre-licensing documents. The facility will serve elderly ages 60 and above in a residential care facility for the elderly. The fire clearance was approved for (6) ambulatory residents. The home is a one story home with attached garage, with (4) bedrooms, (1) staff office, and (3) bathrooms. No pools or firearms are present at the facility. LPA observed PPE equipment and hand washing stations in the facility. LPA observed the emergency supplies and emergency exits. The physical plant was observed to be in good repair and free of hazards. Provisions of safety such as grab bars were observed, The bedrooms had the required furniture along with linens. The facility kitchen can prepare food in a clean and safe environment. The facility possessed requirement amount of food supplies. The smoke and carbon monoxide alarms were tested and observed to be operational. There are designated locked areas for resident medication and sharp hazardous objects. The hot water temperature was recorded in resident restroom at 106.3F, and the land line was operational at (951) 599-4790. There are no objections for the applicant to proceed in the pre-licensing process.
