Type of visit: Required - 1 Year
On 04/07/26, at 8:55am, Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, annual visit. Bernadette Babelonia, Caregiver was advised of the visit. Sean Abalajon, Administrator was called and arrived about five (05) minutes later. LPA asked for the census, resident, and staff files. The facility is a one (1) story home that has a total of five (05) bedrooms and three (03) bathrooms. Fire Clearance is approved for six (06) non-ambulatory and the facility has a hospice waiver for one (01) resident. Last fire drill/Fire Alarms were conducted on 04/01/2026. The living and dining room are neat and clean. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located near the kitchen, observed to be full and last purchased on 03/2026. There are cameras in the common areas. There is a signal system in the house. Kitchen is sufficiently stocked with at least seven (07) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Kitchen cabinets contain extra food. Knives and sharps are observed to be locked and inaccessible to residents in one (1) of the top cabinets. The medication is kept in the kitchen area locked and inaccessible to the residents in a top cabinet. The toxins/chemicals are kept under the sink. There are two (2) sinks in the kitchen. LIC 809C-continued There is one (1) washer and dryer outside in the backyard in a secured area. Living Room and Dining Room: LPA observed the living room and furniture to be clean and in good repair. LPA observed the dining area to be clean and in good repair. The dining room area has several chairs for residents and a fire place covered. The facility temperature is between 68 and 69 degrees Fahrenheit. There is another fire extinguisher in the hallway fully charged with an expiration date of 03/2026. There is also another fire place that is covered. Bedrooms: Facility has five (5) bedrooms. All bedrooms are single occupied. Two (2) bedrooms have a private bathroom and the other bathroom is in the hallway. Bathrooms: There are three (3) bathrooms that have proper grab bars, non-skid mats. The bathrooms contained a trash can with tight-fitting lid. Hot water was tested and measured between 110.8 F and 110.9 F within regulations. Outside/Backyard: There is a detached garage. The outside/backyard has furniture for residents to have proper seating and a television. There is a no bodies of water. There is also a shed in the backyard and an extra refrigerator. Administrative: The administrative Certification is current and expires 05/15/2026. There is a yes sign, Ombudsman, Personal Rights and Emergency Disaster Plan, Rights of Resident Council, Emergency Numbers against the wall of the facility on your right-hand side of the entrance of the facility. There is proof of insurance. Staff/Resident Files: LPA reviewed five (5) resident files and four (4) staff files. Type of visit: Case Management - Annual Continuation
At 11:10a.m. Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted an unannounced Required One (1) year inspection continuation to this facility. LPA met with Caregiver and granted entry to the facility. Caregiver called Assistant Administrator via-phone. At 11:35a.m. Assistant Administrator joined the visit and explained the reason for the visit. Administrator, Yule indicated not being able to joined today’s visit and authorized Assistant Administrator to sign the report. Deficiencies observed on 02/19/2025 visit as follow: -The licensee failed to follow their own plan of operation. The facility retains two (02) hospice residents, but hospice waiver is approved for one (01) resident. - Licensee did not ensure that the facility is in good repair. Facility retains dementia residents and one of the gates leading outside was broken. -Licensee did not ensure that facility temperature is comfortable for all residents. Thermostat was broken for a while and the rooms were heated by portable heaters. - Licensee did not ensure that outdoor passageways are free of obstruction. There are several pieces of lumber with sharp edges leaning against back wall of the facility. -Upon review of residents records, LPA noted that the documents on the records identify other facilities names. Physician reports were not signed by the doctors. -Licensee did not ensure that the facility is free of vermin. -Licensee did not ensure that the facility has qualified Administrator that could confirm applicable law, rules and regulations. - Facility retains two (02) hospice residents but is approved for 1 hospice waiver. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted an unannounced Required One (1) year inspection to the facility. LPA met with Assistant Administrator and Administrator and explained the reason for the visit. Administrator, Yule indicated not being able to stay for today’s visit and authorized Assistant Administrator to sign the report. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: At 10:05p.m. a tour of the physical plant was conducted. There is only one entrance being utilized at the facility. Living and dining room furniture were checked. The living room is neat and clean. The dual carbon/smoke detectors are interconnected and observed to be operational. Fire extinguishers with service date of 01/03/2025 are located thorough out the facility. The facility has five (05) bedrooms and three (03) bathrooms currently occupying three (03) residents. Three (03) rooms are private room and two (02) rooms are empty. The backyard of the facility has outdoor furniture with a covered shaded area for residents. There is no body of water at the facility. There is also a shed at the backyard. Laundry area is located next to detached garage, laundry detergents, cleaning agents and other toxins were observed to be locked. Food Service/Kitchen area was sufficiently stocked with two (02) days of perishable and seven (07) days of non-perishable food. Knives and sharp objects were observed to be locked and inaccessible to residents. Cont. on LIC 809-C Residents rooms are adequately furnished with appropriate furniture and lighting system. The bathroom was checked for cleanliness and proper operation. LPA observed the appropriate grab bars in the shower and toilet. The hot water temperature was measured at 111.8 degrees. There was enough clean linen available in stock at the cabinet. LPA observed medication in the kitchen cabinet to be locked and inaccessible to residents. There are two (02) complete first aid kit. Resident Files: A file review of resident records to insure compliance of licensing forms. Staff Files: A file review of staff records to insure forms and training are up to date and compliance with licensing forms. Licensing Program Analyst(LPA) Antonia Alvizar-Ettima met with the Administrator and Assistant Administrator and an unannounced Required One (1) year inspection visit to this facility. During inspection, LPA Alvizar-Ettima discovered the following Part-Time Staff (S1) and (S2) have been present without a Background Clearance or Transfer and Association to this facility. S1 job title is Caregiver, first day of work was 07/11/2022. S2 job title is Caregiver, first day of work was 10/18/2023. LPA request/received S1 Republika NG Pilipinas. LPA request/received S2 United States of America Permanent Resident Card. LPA verified using Guardian Background System Check, staff S1 and S2 names did not appeared on facility roster. Type of visit: Case Management - Other
In conjunction with a One (1) year inspection to the facility, Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted a case management visit to address the deficiencies unrelated to the inspection. During inspection, at about 1:52p.m., LPA noted resident #1 (R1) was restrained to the wheelchair with a gait belt as postural support. The gait belt easy release buckle was on the back of the wheelchair. At about 1:53p.m. LPA interviewed staff #1-#2 (S1-S2) and indicated that the gait belt is used to prevent R1 from standing and sliding off wheelchair. Interview with R1 revealed that did not know why they placed the gait belt on them. R1 illustrated that was not able to release the gait belt on their own during emergency. LPA interviewed Administrator and requested prescription for the postural supports. Administrator was unable to provide the document. Based on inspection, observation and interviews it was concluded that licensee did not ensure that postural support provided to resident not depriving residents’ movement. Resident was restrained to the wheelchair and had no ability to release it. Deficiencies will be cited and recorded on LIC809D. Exit interview conducted. Licensing Program Analyst (LPA) Rosaura Valenzuela conducted an unannounced Required One (1) year inspection to the facility. LPA met with Administrator Mark Yule and explained the reason for the visit. A tour of the physical plant was conducted at 12:00pm and the following was noted: There is only one entrance being utilized at the facility. Living and dining room furniture were checked. The living room is neat and clean. The facility maintains a comfortable temperature at 73 degrees. The smoke detectors are hardwired and interconnected and observed to be operational. There is a carbon monoxide detector in the facility. Fire extinguishers are located thorough out the facility. The facility has five (05) bedrooms and three (03) bathrooms currently occupying five (5) residents. One (01) room is a shared room and four (04) rooms are private room. The backyard of the facility has outdoor furniture with a covered shaded area for residents. There is no body of water at the facility. There is also a shed at the backyard being used for storage. Laundry area is located in the back of the facility, laundry detergents, cleaning agents and other toxins were observed to be locked. Food Service/Kitchen area was sufficiently stocked with two (2) days of perishable and seven (7) days of non-perishable food. Knives and sharp objects were observed to be locked and inaccessible to residents. Continue on 809-C The residents rooms are adequately furnished with appropriate furniture and lighting system. The bathroom was checked for cleanliness and proper operation. LPA observed the appropriate grab bars in the shower and toilet. The hot water temperature was measured at 120 degrees. There was enough clean linen available in stock at the cabinet. Medications-LPA observed medication in the kitchen cabinet to be locked and inaccessible to residents.
