Type of visit: Required - 1 Year

On 03/11/26, at 10:05am, Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, annual visit. Junaly Cobrador, Caregiver was advised of the visit. Sean Abalajon, Administrator was called and arrived about fifteen (15) minutes later. LPA asked for the census, resident, and staff files. The facility is a one (1) story home that has a total of six (06) bedrooms and four (04) bathrooms. Fire Clearance is approved for six (06) non-ambulatory of which one (01) may be bedridden resident and the facility has a hospice waiver for six (06). Last fire drill/Fire Alarms were conducted on 03/01/2026. The living and dining room are neat and clean. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located near the kitchen, observed to be full and last purchased on 12/2025. There are cameras in the common areas. There is a signal system in the house. Kitchen is sufficiently stocked with at least two (02) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Kitchen cabinets contain extra food. Knives and sharps are observed to be locked and inaccessible to residents in one (1) of the top cabinets. The medication is kept next to the kitchen area locked and inaccessible to the residents in a top cabinet. LIC 809C-continued There is one (1) washer and dryer with toxins that are kept in area enclosed locked and inaccessible to residents. Living Room and Dining Room: LPA observed the living room and furniture to be clean and in good repair. LPA observed the dining area to be clean and in good repair. The dining room area has six (6) chairs for residents and a fire place covered. The facility temperature is between 71 and 72 degrees Fahrenheit. There is another fire extinguisher in the hallway fully charged with an expiration date of 12/2025. Bedrooms: Facility has six (6) bedrooms. One (1) bedroom is currently empty, One (1) bedroom is shared, One (1) bedroom is for staff and three (3) bedrooms are single, occupied. Bathrooms: There are four (4) bathrooms that have proper grab bars, non-skid mats. The bathrooms contained a trash can with tight-fitting lid. Three (3) bathrooms are private and the other is in the hallway. Hot water was tested and measured between 110F and 112.8F within regulations. Outside/Backyard: There is no garage. The outside/backyard has furniture for residents to have proper seating and a television. There is a pool that is inaccessible to the residents it is locked. There is a pool house with a separate bathroom. Administrative: The administrative Certification is current and expires 05/15/2026. There is a yes sign, Ombudsman, Personal Rights and Emergency Disaster Plan against the wall of the facility on your left-hand side of the entrance of the facility. There is proof of insurance. Staff/Resident Files: LPA reviewed five (5) resident files. LPA reviewed three (3) staff files. Jan 31, 2025Facility evaluation reportReport on file

Type of visit: Prelicensing

Licensing Program Analyst Abeye Duguma (LPA) conducted an announced Pre-licensing visit at 01:30 PM and met administrator, Sean Abalajon. LPA conducted an entrance interview with the Administrator. With the assistance of the Administrator, LPA conducted a facility tour of both the inside and outside. The facility was inspected for Fire Safety, Personal Accommodations and Services, Medication Procedures and Food Service. This is a single-story property. Fire Clearance is approved for six (06) non-ambulatory of which one (01) may be bedridden resident. Facility has six (06) bedrooms, of which five (05) are for residents and four (04) bathrooms. Resident bathroom has properly installed grab bars and shower has non-skid mats. Hot water temperature measured at 108.3ºF during the visit. All residents’ bedrooms were adequately furnished. The linens were stored in the storage space located in the hallway cabinets. The common areas were appropriately furnished and had adequate furniture. The LPA observed entertainment equipment and games for activities. The records were stored in a file cabinet located in kitchen nook. The first-aid kit is complete. At the time of the visit, facility had water, perishable and nonperishable food supplies should they admit a resident(s). LPA observed a fireplace that has appropriate screen. LPA inspected the kitchen and observed the stove and refrigerator to be clean and working. Sharps are stored in a locked kitchen cabinet. Smoke detectors and Carbon Monoxide detectors were checked and function properly. There are two (02) fully charged fire extinguishers located in the kitchen area and hallway. Fire extinguishers were certified on 12/06/2024. (CONT. on LIC809-C) LPA Duguma observed a washer and dryer in the laundry room at the end of the hallway to the right. All toxins were stored in a cabinet under the sink. There is sufficient outdoor space with seating and a shaded area with proper furniture for outdoor use. Facility has two (02) empty water fountains and a swimming pool with appropriate locked fencing. At the time of this visit the physical plant is meeting Title 22 requirements. The Component III completed with LPA. No health and safety hazard were noted during this visit.
