Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced visit to the facility to conduct a Plan of Correction (POC) visit to confirm that a citation which was issued on 08/27/2026, has been corrected. LPA was granted entry by caregiver Khaira Holgado, when she identified herself. LPA discussed the purpose of the visit with caregivers Holgado, Edita Palado, and Shabrina Febre. Administrator Stanley Alegre and Licensee Gemma Alegre later arrived and joined the visit. The following citations were reviewed during today's visit: California Code of Regulations, Title 22, Section 87202 (a)(2) Fire Clearance. On 08/24/2026 the Department conducted the facility’s required annual inspection and cited for a Fire Clearance deficiency. The plan of correction was due on 08/24/2026. The Department received the facility's initial POC on 08/24/2026. During today's visit, the facility's POC was cleared during the visit and clearance letter was issued. No new deficiencies were observed or cited during today's visit. Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced required Annual Inspection. LPA identified herself and was granted entry by caregivers Khaira Holgado and Edita Palado, and discussed the purpose of the visit with caregivers Holgado and Palado. Caregivers Mary Jhun and Shabrina Febre, and licensee Gemma Alegre later arrived and joined the visit. According to the facility’s license, there may be a maximum of 6 non-ambulatory residents at any given time at the facility site. During today’s inspection, the facility’s current census is 5 residents living at the facility. There was 1 resident present at the facility site during the inspection, and 4 residents arrived later from their respective programs. LPA inspected the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. One bedroom contained a full bed railing that was not approved, but the facility removed the bottom half of the railing prior to LPA's departure. Doors, windows, toilets, and showers were in working order. Extra linens and hygiene supplies were present. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and activities. The facility’s ambient internal temperature was compliant, at 84ºF. Hot water temperature at taps accessible to residents were also compliant throughout the facility, reaching between the allotted temperature ranges, 105ºF - 120ºF. [CONTINUED ON LIC 809-C] [CONTINUED FROM LIC 809] There were at least 2 days of perishable food, and at least 7 days of non-perishable food present. Cooking, dining equipment, and utensils were present, and all were safely stored. Toxic chemicals and/or poisons were inaccessible. Medications will be stored in a locked cabinet. No pools or bodies of water on the premises. The facility fireplace was made inaccessible to residents. Per Licensee Alegre, no firearms or ammunition are kept at the facility. Carbon monoxide detectors, emergency lighting, and the facility telephone were all working. The fire extinguisher was present (02) and serviced within the last 12 months. The first aid kit was complete and readily accessible. LPA spoke with staff and reviewed staff and resident records. LPA's visit did not raise licensing concerns. The files that LPA reviewed contained the required documents. Confidential records were stored in a locked area. Required licensing postings were observed in a visible area of the facility. There were a deficiencies observed and cited during today's annual inspection and may be found on the LIC809-D page of this report. An exit interview was conducted with licensee Gemma Alegre, to whom a copy of this report, along with the Licensee/Appeal Rights (LIC9058 03/22), were provided at the conclusion of the visit. Licensing Program Analyst (LPA) Jose De La Cruz made an unannounced visit to conduct a Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified himselve to, and discussed the purpose of the visit with Licensee/Administrator Gemma Alegre and Caretaker Warlita Jimenez. According to the facility’s license, the facility has a maximum capacity for six (6) clients, of whom all can be non-ambulatory. Per LPA observation, LIC602 Physician’s Reports, and interviews: During today’s inspection, there were a total of five (5) clients in care, three ambulatory and two non-ambulatory. LPA interviewed staff and the client who was currently in the facility. LPA also reviewed the care records for all clients, and the personnel and training files for active staff. LPA, accompanied by Licensee, toured the interior and exterior of the facility and inspected all common areas and bedrooms. In the facility’s backyard, there was a shaded outdoor activity space/patio, as required. Area was clean and free of obstacles and well maintained. Hallway to the outside was also free of obstacles. Four trash cans in good condition were present on the outside of the facility. The facility was clean, sanitary, and in good repair, no foul smells or odors were present. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings and were specifically decorated for each resident with mementos, toys and other memorabilia, as well as religious items. Doors, windows and screens, toilet, and shower were all working. Extra linens and hygiene supplies were present. CONTINUED ON LIC 809-C, 1 of 2] [CONTINUED FROM LIC 809] The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility’s ambient internal temperature was complaint at 76 F. Where tested, hot water temperature at taps accessible to clients were compliant in temperature: Kitchen Sink was 118.05 F and Bathroom Sink was 120 F. Appliances to preserve perishable food were also compliant in temperature. Cooking/dining equipment and utensils were present and in good condition. There were no fireplaces, swimming pools, or similar bodies of water on the premises. There were no medications, toxic chemicals/poisons, or open-faced heaters accessible to clients. Confidential records were stored in locked areas. Per the Licensee, no firearms or ammunition are kept at the facility. The facility’s license does not include endorsements for delayed-egress doors or secured perimeter doors, and neither of these were present. Carbon monoxide detector, smoke alarms, night lights, and emergency lighting were all working. The facility’s fire extinguisher was serviced within the last twelve (12) months. Required licensing postings were observed in visible areas of the facility. Licensee presented proof of current business liability insurance and surety bond. Client care files contained the required records. Staff personnel files contained the required records. Licensee performed disaster drills on all three (3) shifts at the required intervals. Staff were also trained on Personal Protective Equipment (PPE) and the facility’s written Emergency Disaster Plan within the last twelve (12) months, as required. There were reserve supplies PPE present. Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced required Annual Inspection. The facility file was reviewed prior to the visit. LPA Lopez identified herself, was granted entry by caregiver Evelia Guerta. LPA discussed the purpose of the visit with caregiver Guerta. Licensee Gemma Alegre and Administrator Stanley Alegre later arrived and joined the visit. According to the facility’s license, there may be a maximum of six (6) residents all of whom may be non-ambulatory in at any given time at the facility site. During today’s inspection, the facility’s current census is 6 residents living at the facility. There were no residents present at the facility site during the inspection. LPA, accompanied by Licensee Alegre and caregiver Guerta, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings, but one residents bed railing did not have an exception or waiver on file for approval by the Department but the facility remedied during the inspection. Doors, windows, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and activities. The facility’s ambient internal temperature was comfortable and compliant, at 78 degrees Fahrenheit (F). Hot water temperature at taps accessible to clients were also compliant: kitchen sink measured hot water at 115.2 degrees F; sink in restroom #1 delivered hot water at 115.2 degrees F; sink in restroom #2 delivered hot water at 115.2 degrees F; hot water in restroom #3 delivered hot water at 106.6 degrees F; and hot water in restroom #4 delivered hot water at 115.2 degrees F. There was at least 2 days of perishable food, and at least 7 days non-perishable food present. Cooking and dining equipment and utensils were present, and all safely stored. There were no toxic chemicals or poisons accessible to residents. Medications were properly labeled, as required, and stored in locked cabinet. During today’s visit there were no residents on the facility premise as they were attending their respective day program. LPA visit did not raise any licensing concerns. The files which LPA reviewed contained required documents. Confidential records were stored in a locked area. Required licensing postings were observed in a visible area of the facility. There were deficiencies observed and cited during today's annual inspection and may be found on the LIC809-D page of this report. An exit interview was conducted with Licensee Alegre and Administrator Alregre to whom a copy of this report along with the Licensee/Appeal Rights (LIC9058 03/22) were provided at the conclusion of the visit.
