Licensing Program Analyst (LPA) Quoc Huynh arrived at the facility unannounced to conduct a required annual visit at 10:38AM. The LPA met with the Coordinating Manager Marine Bekyan and informed them of the reason for the visit. Entrance interview conducted. Beginning at 11:50AM, the LPA and Coordinating Manager toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. The facility is a single-story residential home on shared property with another related facility. The following was observed: COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. There was an office space located in the dining area, with required postings and centrally stored medications in a locked cabinet. The facility maintained a comfortable temperature throughout the visit. There was a laundry closet located in the hallway containing laundry supplies. The laundry door remains locked when not actively in use and the machines were observed to be operational. KITCHEN: The LPA observed knives stored inaccessible in a locked drawer near the sink. Kitchen appliances were clean and in operable condition. The facility had a supply of perishable and non-perishable food, as well as emergency food and water stored in the office. Food in the refrigerator and freezer were observed to be properly stored with labels and dates. Report Continued on LIC 809-C BEDROOMS/RESTROOMS: There are four (4) total bedrooms. Bedroom #4 is cleared for one (1) bedridden resident and had a direct exit to the outside. Bedrooms were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Extra linens and supplies are stored in each bedroom located in cabinets. There are four (4) total restrooms in the facility: three (3) attached private resident restrooms and one (1) common restroom located in the hallway. Restrooms were clean and sanitary and in operating condition with grab bars and non-slip surfaces. All restrooms were sufficiently stocked with soap, paper products, and displayed hand washing signs. Hot water was tested and measured between 117.3 degrees F and 118.4 degrees F which is within the required range per regulation. OUTDOOR AREA: The surrounding grounds had multiple shaded patio areas equipped with furniture in good condition for resident and visitor use. The front yard had a driveway with a remote operated gate as well as a door for everyday use. There was one (1) emergency exit door located on one side of the facility. The other side of the facility had a locked side gate which contained a shed and general storage/equipment. All exits and passageways were free of obstructions. The LPA observed one (1) shed in the backyard that contained general storage and was locked. The facility had additional gates with access to the neighboring three (3) facilities that the Licensee also operates. RECORDS: Record review began at 11:34AM. Resident records were reviewed for, but not limited to care plans, physician's report, admissions agreement, and consent forms. All records were in order. Personnel records were reviewed for, but not limited to health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All records were in order. Report Continued on LIC 809-C INFECTION CONTROL/EMERGENCY DISASTER PLAN: During today's visit, LPA reviewed the facility's infection control plan and emergency disaster plan. Both documents were observed to be complete and reviewed annually as required. Emergency disaster drills are conducted quarterly, with the last documented drill on 06/20/2026. Smoke and carbon monoxide detectors as well as fire door were tested at 12:37PM and were operational. A fire extinguisher was observed and was last serviced on 05/13/2026. MEDICATIONS: Medication review began at 12:42PM. Medications are centrally stored and kept inaccessible. Medications were observed for two (2) residents. Medications were labeled and checked for expiration dates and were properly documented on the centrally stored medications and destruction record. No errors observed during the medication review. Licensing Program Analyst (LPA) Quoc Huynh conducted an initial complaint visit for the above allegations. LPA arrived at 10:04AM and met with the Licensee Lusine Srmikyan and Coordinating Manager (CM) Marine Bekyan and explained the reason for the visit. Entrance interview conducted. The LPA and Licensee conducted a safety check tour beginning at 10:09AM. No immediate concerns were observed. Between 10:09AM and 10:32AM, the LPA interviewed two (2) residents, the CM, and the Licensee. Record review at 11:05AM revealed that the complaint was created under the wrong facility address as Resident #1 (R1) resided in the Licensee's neighboring facility. No deficiency cited. There was an office/staff nook located in the dining area, with required postings and centrally stored medications in a locked cabinet. The facility maintained a comfortable temperature throughout the visit. A fire extinguisher was observed in the dining area and was last serviced on 04/16/2025. There was a laundry closet located in the hallway and contained laundry supplies. The laundry door remains locked when not actively in use and the machines were observed to be operational. Ramps were observed throughout the facility for wheelchairs. Licensing Program Analyst (LPA) Sandra Urena arrived at the facility unannounced to conduct a required annual visit. The LPA met with staff and explained the reason for the visit. The Coordinating Manager, and the licensee arrived shortly thereafter, and the LPA explained the reason for the visit. The LPA and the staff toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that the facility is in compliance with Title 22 Regulations. KITCHEN: The LPA observed the kitchen/dining area. Knives are stored in a locked kitchen drawer. Kitchen appliances are in operable condition. The facility has a sufficient supply of perishable and non-perishable food. The hot water measured at 113.9-degree Fahrenheit. Medications and first aid kits are located in a locked cabinet near the kitchen area. BEDROOMS: The LPA observed resident bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Inside temperature was maintained at a comfortable level, 72 degrees. RESTROOMS: Restrooms are relatively clean and sanitary and in operating condition with grab bars and non-skid mats. Hot water measured at 117.0 degrees Fahrenheit. The sinks had sufficient liquid soap, and paper towels. Signs are posted throughout bathrooms and facility to promote hand washing. COMMON AREAS: The LPA observed the common area to be relatively clean and properly furnished. The LPA observed the fire extinguisher to be fully charged and last serviced on 05/15/2024. At 1:35 p.m. fire alarms/ carbon monoxide detectors were tested and functioned properly. Laundry units are located in one of the hallways. Night lights were present in the hallways and passages. Continues on LIC 809-C… OUTDOOR AREA: The backyard has a covered outdoor area equipped with furniture for client use. No bodies of water noted. Cleaning supplies and disinfectants are kept in locked in a storage area. RECORDS: Records review began at 12:15 p.m., Residents’ records were reviewed for, but not limited to care plans, medical records, admissions agreement, consent forms. All records were in order. Personnel records were reviewed for, but not limited to health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All files were in order. MEDICATIONS: Medications review began at 1:45 p.m.; medications are centrally stored and locked in a cabinet near the kitchen area; medications are labeled and checked for expiration dates. Medications are properly documented on the centrally stored medications and destruction record. No errors observed during the medication review. INFECTION CONTROL: The facility has an adequate supply of Personal Protection Equipment (PPE) and the facility is able to obtain additional supplies as needed. The facility’s cleaning protocol is sufficient. If needed, the facility has the capacity to designate a single isolation room if the facility has a confirmed case of COVID-19.
