Licensing Program Analyst (LPA) Quoc Huynh conducted a pre-licensing visit to the above noted facility. At 11:58AM, the LPA met with Applicants Anthony Gezalyan and Milena Tovmasyan. Entrance interview conducted. An application to operate a Residential Care Facility for the Elderly was submitted on 10/02/2025. A dementia program was included in the plan of operation. A Hospice Waiver for six (6) was requested. The facility's fire clearance was approved on 11/06/2025 for a total of six (6) residents, all non-ambulatory. Component II orientation was completed on 03/16/2026. Component III was completed with the Applicants during today's visit. Beginning at 12:02PM, a physical plant tour was conducted. The following was observed: KITCHEN: Knives were stored in locked drawers and cleaning supplies secured under the sink. The supply of dishes, utensils, pots, pans and drinkware is adequate. The freezer was maintained at 0*F and the refrigerator was maintained at 40*F. The supply of perishable and nonperishable foods was adequate in addition to emergency food and water. There were no pesticides or toxins stored in any food storage area or preparation area. Appliances in the kitchen were clean and functional. The kitchen area also contained designated secure storage for files and medications. Report Continued on LIC 809-C BEDROOMS/BATHROOMS: The facility had four (4) bedrooms: three (3) resident rooms and one (1) staff room/office. Bedroom #1 and the staff room had direct exits to the outside. Bedrooms #1, #2, and #3 were approved for non-ambulatory residents. Resident rooms were set up with beds, nightstands, lamps, chairs, and clothing. The beds were furnished with box springs, comfortable mattress and clean linen, which includes, a mattress pad, top and bottom linens, pillowcases, and blanket (if needed). Lighting in the rooms appeared adequate. The bedrooms were large enough to allow for easy passage between the beds and furniture with a wheelchair or walker. In addition, no bedroom was used as a passageway to another room, bath or toilet. All rooms were free of odors. All window screens were clean and maintained in good repair, however Bedroom #2 had a missing window screen. There were two (2) bathrooms: one (1) staff and one (1) shared resident bathroom. The bathrooms have non-slip mats but in need of secured grab bars. The hot water temperature was tested and measured within the required range of 105*F to 120*F. COMMON AREAS: The living room and dining room was appropriately furnished, and the lighting was adequate. There was a television, and/or activity supplies in the common areas. First aid kit was observed to be complete, including a thermometer and a current version of a first aid manual. There was sufficient space to accommodate both indoor and outdoor activities. The physical plant is consistent with the submitted facility sketch/floor plan. The facility had emergency lighting, which included but not limited to flashlights. Hallway ceiling lights will be utilized in lieu of nightlights as they are dimmable. The facility has central heating and air conditioning to maintain rooms to a comfortable temperature. At 12:21PM, the smoke and carbon monoxide detectors were tested in addition to the fire door and functioned properly. One (1) fire extinguisher was fully charged and purchased on 10/25/2025. A supply of extra bed linens were observed in the hallway cabinets. There is a functioning telephone on the premises. All required postings are posted in the dining area. There was a secured garage that contained general storage and laundry machines in good condition. Report Continued on LIC 809-C OUTDOOR AREA: The exterior passageways were clean and clear of any obstructions. Two (2) emergency side exits led to the front yard/driveway. There were no bodies of water on the premises at the present time. There was a shaded patio area in the rear yard with furniture in good condition. The following corrections are needed prior to licensure: - Install grab bars - Replace Bedroom #2’s window screen The Applicant is to provide the LPA with proof of the corrections, but no later than 04/29/2026. This report and corrections will be provided to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect approval of your license. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of following areas: 1.
