On 4/24/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced for an annual inspection visit. LPA was met by Administrator (A1), Virginia Penner. LPA introduced self, explained reason for visit and was permitted entry into the facility. LPA completed a health and safety check on residents in care. LPA toured the facility completed. Residents observed in common areas and in rooms. There was 1 resident on hospice at the time of the inspection. Pathways and doors were clear and free from obstruction. Facility was clean and without odor. Common areas were clean, adequately furnished, and adequately lit. Carbon monoxide detectors were present and operate on a system. Smoke Detectors were present and operational per report given to LPA by California Code of Regulations. Fire extinguisher last serviced 10/16/25. Last fire drill on 4/21/26. Resident rooms observed to have the required furnishings and with adequate lighting. Linen supplies are kept in linen closets. Sharps, chemicals and medications were located in locked closets/rooms. Residents hot water temperature in the bathrooms were measured at a range of 114.2 to 118 in 5 bathrooms. The following issues were observed during todays visit: 2 medications were successfully administered but not initialed by staff. Medication details located on LIC-809D for medication details. 5 out of 5 resident showers did not contain non-skid mats or strips. 3 out of 5 residents did not contain the Safeguard for Property Values form in resident files. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. LPA requested the following documents to be submitted to CCL by 3/6/25: current copy of Administrator’s Certificate, Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-D), Personnel Report (LIC 500), Receipt for Liability Insurance coverage, Register of Facility Clients/Residents (LIC 9020) in order to update the facility file. On 3/6/25 Licensing Program Analyst (LPA) M. Garza completed an unannounced case management visit. LPA met with Administrator, Jenny Penner, explained reason for visit and was permitted entry into the facility. A health and safety was completed on residents in care. Residents observed in common areas and in rooms. This case management is being conducted as a follow up visit to a case management completed 2/27/25 for R1. This visit is for additional information on R1, documents was requested (physicians report, incident report, hospital discharge records, therapy notes, R1 care plan, medication list, face sheet for R1) and interviews were conducted (Administrator, S1 and S2) and follow up will be needed. Type of visit: Case Management - Annual Continuation

On 3/6/2025 Licensing Program Analyst (LPA) M. Garza completed an unannounced annual continuation visit. LPA met with Administrator, Jenny Penner, explained reason for visit and was permitted entry into the facility. LPA observed residents in common areas. Annual continuation visit is being conducted to complete items from previous visit on 2/27/2025. Visit was a continuation for resident records review, hospice care plans, staff file reviews, incidental medical/dental, medications and a completion of the care tool. The following issues were observed during todays visit: Residents with health conditions do not have the required exceptions/care plans in place. 5 of 5 resident files were not observed with the required pre-admission appraisal/reappraisals. Type of visit: Case Management - Other

On 2/27/25 Licensing Program Analyst (LPA) M. Garza completed an unannounced case management visit. LPA met with Cheif Executive Officer (CEO), Roberta "Ro" Linscheid and Licensed Vocational Nurse (LVN), Cecilia Barraza was informed Administrator, Virginia Penner was unavailable. LPA introduced self, explained reason for visit and was permitted entry into the facility. This Case Management is being conducted as follow up for Incident Reports (IR) received by the Department. 1) IR was received by CCL for an incident occurring on 8/31/2024. R1 fell and was sent to Emergency Room. R1 was given a diagnosis of a pelvic fracture. Facility did not report this incident to CCL until 9/14/24. Deficiency cited for late reporting per Title 22. 2) IR received by CCL for incident on 10/26/24. R2 had physical altercation with R3 and R4. PD called to the facility. SOC 341 was completed but not submitted to CCL. Deficiency cited for reporting requirements per Title 22. 3) IR received by CCL for incident occurring on 12/30/24. R5 fell in dining room, hitting their shoulder and falling on wrist. R5 was sent to the hospital and was diagnosed with a fracture to their left wrist. Facility did not provide discharge paperwork showing follow up for R5. During record review of discharge paperwork, R5 returned to the physician on 1/1/25 and 1/10/25 for follow up on this injury. R5 wore brace with no further follow up. No deficiencies cited for this incident. 4) IR received by CCL for incident occurring on 2/11/25. R6 was found outside at 5:15 am. Per S1 there was a freeze warning the night of 2/10/25. Per incident report R6 was found outside memory care unit in the patio area with bleeding to their head. R6 was moving but not responsive to conversation. R6 sent to ER via EMS and treated for low blood pressure, hypothermia and head injury. Additional information need for this case management. LPA will return once gathered. Deficiencies will be issued at that time, if necessary. CONT... CONT... 5) During annual visit conducted on 2/27/25 R7 was observed with bruising to the left side of the face. R7 stated they fell. Record review of R7's file disclosed incident occurred on 2/22/25. Residents observed in common areas and in rooms. There was 2 resident on hospice at the time of the inspection. Pathways and doors were clear and free from obstruction. Facility was clean and without odor. Common areas were clean, adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors were present and operate on a system. Fire extinguisher last serviced 10/10/24. Last fire drill on 2/24/25. Resident rooms observed to have the required furnishings and with adequate lighting. Linen supplies are kept in linen closets. Sharps, chemicals and medications were located in locked closets/rooms. The following issues were observed during todays visit: Small tear in carpet at doorway of room C-13. Facility sketch missing meeting point. Refrigerator in snack bar in need of cleaning. Bathroom shower in need of cleaning in rooms M-4 and M-7. 2 of 2 Hospice Care Plans observed incomplete. Deficiencies will be cited during annual continuation visit. Type of visit: Case Management - Incident

On 3/19/2024 Licensing Program Analyst (LPA) M. Garza arrived for an unannounced case management visit. LPA met with Administrator, Virginia Penner explained reason for visit and was permitted entry into the facility. A health and safety check was completed on residents in care. LPA observed residents in common areas and in rooms. This case management is being completed for 2 special incidents received by the facility. Special incident #1 was received on R1. R1 had received some unexplained bruising. LPA reviewed medical chart which notated R1 had a fall. R1 was seen by the physician the following day and assessed with no additional orders. LPA reviewed records and completed interviews. At this time, no further follow up needed. Special incident #2 was received on R2 stating that R2 had a fall resulting in R2 being taken to the ER. R2 has a history of falls. It was reported R2 sustained a suspected GI bleed. Hospital noted a GI lesion that explained reason for concern. LPA reviewed records and completed interviews. Residents observed in common areas and in rooms. There was 1 resident on hospice at the time of the inspection. Pathways and doors were clear and free from obstruction. Facility was clean and without odor. Common areas were clean, adequately furnished, and adequately lit. Smoke detectors were present and operate on a fire system. Carbon monoxide detectors present and operational at time of visit. Fire extinguisher last serviced 10/30/23. Last fire drill on 03/19/24. Water temperature measured 118 and 120 degrees F. Resident rooms observed to have the required furnishings and with adequate lighting. Linen supplies are kept in linen closets. Sharps, chemicals and medications were located in locked closets/rooms. LPA observed sufficient seating under covered patio areas. LPA requested the following documents to be submitted to CCL by 3/26/24: current copy of Administrator’s Certificate, Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-E), Affidavit regarding Resident Cash Resources (LIC 400), Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020) in order to update the facility file. Allegation investigated: Staff are not following COVID -19 protocols. Untrained staff are caring and supervising residents in care.

On 10/19/2023 Licensing Program Analyst (LPA) M. Garza completed an unannounced complaint visit to deliver findings. LPA met with Administrator, Jenny Penner, explained reason for visit. During the investigation, LPA reviewed documentation (schedules, staff roster and contact information, training records) and completed interviews with staff. Interview with Administrator indicated that there have been new staff that was hired. Upon review of the training records, it was observed that 4 of 4 staff did not have the required number of hours of training and 3 of 4 did not have the required training covering hospice, dementia, resident rights and change in condition). It was also indicated that the facility was not following COVID guidance by the most strenuous restrictions set forth and were using staff to cross cover for positive and negative residents.
