Allegation investigated: Staff handles resident roughly Staff comes into resident's room in underwear

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct a complaint investigation and deliver complaint findings. LPA Valerio met with Administrator Sherry and explained the purpose of the visit. The investigation consisted of an interview with the Reporting Party (RP), Staff 1 (S1), and Resident 1 (R1). On 04/09/2026, it was alleged that R1 was handled roughly by S1 and S1 came into R1's room with only boxers. On 04/17/2026, R1 informed LPA that I was overwhelmed last week and misunderstood. I was wrong. R1 stated that when that information was reported, R1 was upset. R1 made up everything and regrets ever saying any of that stuff. R1 did not think it would be that serious. R1 has no issues or concerns with the home. LPA Valerio interviewed S1. S1 stated denied both allegations. S1 stated that R1 wanted to leave the facility and S1 was pushing R1's wheelchair. When R1 wanted to leave, S1 could not prevent R1. R1 was able to leave unassisted at the time of the complaint. Based on this information, the allegation is unfounded. A finding of unfounded means the allegation is false, could not have happened, or is without a reasonable basis. Per California Code of Regulations (CCR) - Title 22, no deficiencies are being cited. Licensing Program Analyst (LPA) Christina Valerio arrived to the facility unannounced to conduct an annual required inspection. LPA Valerio met with facility staff, and explained the purpose of the visit. LPA Valerio contacted Administrator Sherry via cell phone. LPA Valerio was later met by Administrator Sherry Ahuja. The facility has an approved hospice waiver for six (6) residents. One (1) resident may be bedridden and reside in bedroom #5 only. LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed resident bedrooms. Bedrooms were fully furnished and free from odors. LPA Valerio spoke to three (3) residents. Residents were observed sleeping or laying down watching television. LPA Valerio observed groceries being delivered. LPA Valerio observed resident bathrooms. The hot water temperature was measured in two (2) bathrooms and were observed to be 105.6 degrees F and 110.1 degrees F. LPA Valerio observed common areas to be fully furnished. The facility had fans in the common areas and in resident bedrooms. The air conditioning thermostat was set to 75 and the temperature was 81 degrees. According to administrator, they have someone coming out to fix the a/c. LPA Valerio observed the kitchen area. The facility was observed to have a minimum of two day supply of perishable food items and a seven day supply of non perishable food item. All sharps, medications, and toxins were observed to locked and inaccessible to residents in care. LPA Valerio observed the exterior area. The front exterior is surrounded by a gate, which was observed to be unlocked. The backyard exterior area was observed to have no emergency exit ways obstructed or blocked. LPA Valerio reviewed staff and resident files. Files were current with up to date documentation. LPA Valerio requested the following annual documentation: LIC 500, LIC 308, LIC 610, copy of liability insurance. Per California Code of Regulations (CCR) - Title 22 - no deficiencies were cited. On 07/17/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA was greeted by staff member, Lydia Wilkie and explained the the purpose of the visit. LPA also called the Facility Designated Administrator (FDA), Sherry Ahuja and informed her that CCL was present at this time. It was stated by FDA Ahuja that she would be at the facility within 30 minutes. There was one other staff member present at the time of LPA Pascua's arrival, Ryan Mapp. Current census was 5. This facility is licensed to serve 6 residents who are 60 and older. Of the 6 residents, all may be on hospice, 3 may be non-ambulatory and 1 may be bedridden. The bedridden room is cleared by the local fire department for bedroom #5 only. In addition, non-ambulatory residents may reside only in bedrooms #3 and bedroom #4. Ambulatory residents are to reside in bedroom #1 and #2. Bedroom #2 is the only shared bedroom at the time of the fire clearance. LPA reviewed 5 resident files. Based on the file review conducted, it was learned that 5 out 5 resident files did not have a current needs and services plan. 3 out 5 residents did not have an inventory sheet or a pre placement appraisal. LPA reviewed 3 staff files. Based on filed review conducted, it was learned that 3 out 3 staff members do not have current annual training and staff files appears to have documentation from prior facility. The current FDA has an active administrator certificate #7027497740 and expires on 01/30/2026. A tour of the facility was conducted. A tour of the kitchen was conducted. LPA observed a sufficient amount of 2-day perishable and 7 day non-perishable food supply. LPA observed knives and toxins locked and made inaccessible to residents in care. A fire extinguisher was also located in the kitchen and last serviced on 11/22/2023 by Jorgenson Co and is not in compliance at this time. LPA Pascua observed the local Fire Extinguisher company arrived at the time of the visit to service the extinguishers. A tour of the living area, dining area, and other areas intended for resident use was conducted. Furniture was observed to be in good repair and meet resident needs. A tour of 3 bathrooms were conducted. Hot water was taken to ensure compliance within regulation. Grab bars were present and in good repair. A tour a 4 resident bedrooms were conducted. Resident furniture was observed to be in good repair and meet residents needs. 2 additional staff bedrooms was toured. A linen closet was identified and presented a sufficient amount of linens to meet the needs of the residents. Carbon monoxide and smoke alarms were observed and tested to ensure it was in proper working condition. A tour of the garage was conducted. Toxins and other cleaning supplies were identified and made inaccessible to residents in care. A tour of the back yard was conducted with no hazards present. Perimeter fence and exit gates were observed to be in good repair. The following documentation was requested to be submitted to the department: -LIC 308 -LIC 500 -LIC610e -Liability insurance A technical violation was provided for Sections 87506(a) and 87412(a). Per California Code of Regulations, Title 22 Division 6, Chapter 8, deficiencies are being cited today in violation of California Code of Regulations. The Licensee was informed at the time of the visit that failure to complete Plan of Corrections (POC) by the POC date may result in additional civil penalties. Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct a prelicensing visit. LPA Moleski met with applicant Sherry Ahuja and explained the purpose of the visit. During an tour of the facility with staff, LPA Moleski opened the medication cabinet and observed that it was unlocked. Staff searched for the key but were not able to immediately produce it. S1 said S1 was busy cleaning. S2 was preparing food in the kitchen. Neither were passing medications upon arrival. LPA Moleski observed two staff rooms left unlocked. LPA Moleski and Ahuja observed medications present in both rooms. S1 moved all resident medications into a separate locking cabinet during this visit. A citation was issued to the licensee at this address in a separate report. Deficiencies as described above were observed during this visit. LPA Moleski will return for a follow up visit at a later date to reassess compliance. Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct a prelicensing visit. LPA Moleski met with applicant Sherry Ahuja and explained the purpose of the visit. LPA Moleski conducted Component III orientation with Ahuja. LPA Moleski toured the facility with Ahuja and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas. Ahuja is still working on correcting deficiencies recorded on 3/5/24, including replacing window screens, and associating staff to the new facility roster. LPA Moleski observed holes in a backyard deck. LPA Moleski observed first aid supplies, a fully-charged and up-to-date fire extinguisher, and carbon monoxide/smoke detectors. LPA Moleski observed a minimum 2-day supply of perishable food and a minimum 7-day supply of nonperishable food. LPA Moleski observed a locking cabinet for the storage of medication. LPA Moleski observed locking cabinets for the storage of cleaning solutions and knives. Deficiencies were observed based on citations issued on 3/5/24.
