Licensing Program Analyst (LPA), Tihesha Smith conducted an unannounced Required 1-year inspection at this facility. LPA was greeted by the administrator and disclosed the purpose of the visit. LPA conducted a tour of the physical plant to ensure there are no health and safety hazards, and the facility is following Title 22 Regulations. Common areas, including the kitchen, and the combined living and dining room, were evaluated for their ability to safely meet residents' needs. These spaces were inspected for cleanliness, and furniture was assessed for functionality and condition. All common areas were found to be clean, sanitary, and furnished with seating sufficient to accommodate residents comfortably. LPA reviewed the food service areas, food storage and supply (perishable and nonperishable foods). The kitchen food supply was observed and sufficient for the five (5) residents currently residing there. Two (2) days of perishable food observed. The freezer is stocked with frozen foods. Resident medications locked in cabinets near laundry area. Medications and sharps are stored and locked under kitchen counter. Both observed to be inaccessible to residents. There is one (1) fire extinguisher in facility attached to the wall in the living room and observed to be charged. Laundry room is located near kitchen. The appliances observed to be functional. The locked cabinets near washer and dryer contain toxins. The facility has a total of four (4) bedrooms and two (2) bathrooms for residents in care. The residents’ bedrooms were properly furnished with at least one chair, nightstand, and sufficient lighting for each resident. The bedrooms had appropriate and adequate bedding and linens such as sheets, pillowcases, and blankets. Bedrooms with residents using oxygen had designated signs posted on their doors. Each bathroom has the following items available: hand soap, paper towels, and trash cans. Main bathroom has linen cabinet. The hot water temperature was measured for the two (2) bathrooms to ensure it is within the required range for residents’ comfort and safety. The water temperature was 113-degrees Fahrenheit for both bathrooms. Backyard has the following: Covered patio and furniture was observed to be in good repair. Garage: used for storage. Carbon monoxide and smoke detectors were tested and operable at time of visit. Facility grounds were free of hazards. At approximately 11:20 am and 1:30 pm, LPA reviewed two (2) random staff files and three (3) resident files. Staff files had the appropriate training to include first aid and CPR. Resident files included current physician’s report and assessments. Pursuant to Title 22 Division 6 of the CA Code of Regulations, no deficiencies observed during the visit. At approximately 10:45 a.m. on 01/31/2025, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced annual inspection. LPA met with the administrator and disclosed the reason for the visit. The facility was last visited on 12/28/2023 for a prelicensing inspection. It is a single story building with four (04) bedrooms, two (02) bathrooms, kitchen, laundry room, common areas, and outdoor areas. It has an approved fire clearance for six (06) nonambulatory residents, of which one (01) may be bedridden in Bedroom #4. Approved hospice waivers for six (06). LPA observed a maintained front yard upon arrival. The entry gate was unlocked. The main pathway was free of debris and the ramp was secure. A screening station at the front contained surgical masks, N95 masks, sanitizer, a digital thermometer, and a visitor log. Postings at the front included facility license, confidential complaints contacts, ombudsman contacts, emergency disaster plan, rights of resident councils, personal rights, theft and loss policy, and the facility sketch. Walls, floors, windows, screens, and blinds were clean and in good repair. The living room contained furniture in good repair, a television, board games, puzzles, and exercise equipment. At 11:10 a.m. LPA observed a fully charged fire extinguisher near the kitchen. It was last inspected on 01/14/2025 with a tag attached. At 11:15 a.m. LPA measured the room temperature to be 70 degrees Fahrenheit. Night lights were observed throughout the hallways. The facility has four (04) bedrooms. All bedrooms contained a chair, lamp, nightstand, storage, and a bed with adequate bedding. All furnishings were clean and in good condition. All rooms had hospital beds with wheels in the locked position. Bedrooms with residents using oxygen had designated signs posted on their doors. The facility has two (02) shared bathrooms. Both bathrooms contained liquid soap, paper towels, trash can with a tight fitting lid, grab bars near the toilet and shower, and a non-skid mat in the shower. At approximately 11:30 a.m. LPA measured the water temperature in the bathroom near Bedroom #4 to be 109.2 degrees Fahrenheit. LPA observed an adequate supply of perishable and non-perishable foods in the kitchen pantry, refrigerator, freezer, and the extra refrigerator in the garage. Appliances were in good condition. Sharps and medications were locked below the counter top. A locked refrigerator on the counter contained medications. The stove surface and hood were clean. A washing machine and dryer were located in the laundry area along with an adequate supply of fresh linens. Both were in working order. Detergents and cleaning supplies were locked near the appliances. A complete first aid kit was also locked and hung on the laundry room wall. The garage was locked and contained extra supplies. LPA observed a patio area in the rear of the facility. The patio contained a shaded area and furniture in good condition. Pathways were free from obstructions. All emergency exit paths were free of hazards. Four (04) out of four (04) auditory alarms were turned on and functioning. Fire sprinklers were located throughout the facility. At approximately 12:10 p.m., smoke and carbon monoxide detectors were tested and operational. Detectors were hard-wired and functioned simultaneously. At approximately 12:15 p.m. LPA reviewed staff and resident files. All files were complete and available for audit. LPA called the house telephone at 1:00 p.m. The phone was deemed operational. During today's inspection, the facility was in compliance with Title 22 regulations. No immediate health or safety hazards were observed. Exit interview conducted. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Tihesha Smith arrived at the facility on 12/28/2023 at 8:00am to conduct an announced Change of Location visit. LPA Smith was greeted by the administrator Sandra Bello and disclosed purpose of the visit. Administrator identity was verified by valid California Driver’s License. Fire Clearance is approved for 5 non-ambulatory and 1 bedridden resident. LPA inspected facility for Fire Safety, Personal Accommodations and Services, and Food Service. First-aid kit is complete; facility has adequate linen, nonperishable food supplies. There is one (1) fire extinguisher in the property located near front door. There is no swimming pool or other body of water. Facility has working alarms on all exits. Front and backyard are fenced. The kitchen, living and dining room combination area observed to have some furnishings, sufficient lighting and observed to be clean. The kitchen glass table and the living room coffee table to be moved by current tenant by 12/30/2023 per administrator. The laundry room is next to the kitchen: appliances observed to be in good repair. The garage used for storage and is part of the facility however the property owner has items stored in the garage. Administrator revealed owner will have items moved by 12/30/2023. There are four (4) bedrooms and two (2) bathrooms in the home: with one of the bathrooms in Room #4. The two (2) bathrooms observed to be clean with grab bars, mats, and trash cans with lids. Three (3) of the four (4) rooms were unoccupied and have beds delivered by hospice. Bedroom # 2 has owners personal items stacked by closet door and Room #4 is currently occupied with two (2) tenants. Administrator revealed tenants will be completely out of facility on 12/30/2023. LPA observed tenants in the process of moving items out. (Cont. to 809C) (Cont from 809) LPA Smith contacted Licensing Program Manager (LPM) Naira Margaryan regarding issues found during the physical plant tour and move out information provided by Administrator. LPM Margaryan spoke with the Administrator and informed her that the facility is not currently ready to be licensed due to the issued discovered. The Administrator also was advised to contact the trustee of the old location and request an extension to last eviction day, by reason of the new location may not be licensed by 12/30/2023. This immediate change of location was a result of the owner defaulting on the old location property located at 9540 Donna Ave Northridge, CA 91324. LPA Smith conducted an exit interview with the Administrator and informed that the following corrections must be made before completion of pre-licensing visit: Two (2) tenants currently residing in bedroom #4 must move out immediately. All furniture and personal items must be removed from room and bathroom. All personal items belonging to owner and/or tenants shall be removed from bedroom #2. All food, furniture, and personal items belonging to owner and/or tenants need to be removed from the refrigerator, kitchen and living room. All personal items/equipment belonging to owner shall be removed from garage. LPA Smith advised the Administrator to contact the LPA when corrections are made and to send pictures to verify that corrections were made. A conference call between Licensing Representatives, Administrator, and Trustee Lawyer Nancy Zamora was conducted at 11:02 am regarding issues preventing licensing approval today.
