On March 27, 2026, Licensing Program Analyst (LPA) Eboni Bentley arrived unannounced for the purposes of conducting a required 1-Year annual visit using the CARE Inspection Tool. LPA Bentley was greeted and granted entry by staff, after stating the reason for the visit. Administrator (AD) Julie Cornejo was out of town, contacted by telephone, and granted permission for staff to sign today’s inspection report. The facility is licensed to operate forage range 60 and over, for six (6) non-ambulatory elderly residents, of which one (1) may be Bedridden. The building is a single story structure located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, two (2) staff bedrooms, three (3) bathrooms, living area, dining area, kitchen, an outdoor covered seating areas, and an attached two car garage. During the inspection, LPA Bentley toured inside and outside of the physical plant with staff. There were no bodies of water or obstructions on the premises. A comfortable temperature of 74 degrees F was maintained in the facility. LPA observed the facility to be appropriately furnished at the time of visit and free of clutter and debris. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked and available at the time of visit. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available. Bathrooms were found to be clean and operational. The water temperatures in bathrooms measured at 129.0 degrees F to 130.4 degrees F. A deficiency is being cited. Continues on LIC 809-C…. The facility last conducted an emergency safety drill on November 18, 2025. First aid kit is maintained and contains all the necessary elements The smoke alarms and carbon monoxide detectors were operable. A working telephone (714-916-0616) remains available, and the facility has a device that can be used for video teleconference purposes. Emergency food, emergency water, and emergency supplies were available. The facility has two (2) fire extinguishers that were charged, mounted, and serviced on March 11, 2026. Liability Insurance is effective July 20, 2025, and expires on July 20, 2026. During the visit, LPA conducted an audit of five (5) resident files (R1-R5), five (5) staff files (S1-S5), and medication and medication administration records. A total of four (4) resident and three (3) staff were interviewed. Based on observations, a deficiency was cited during the visit, per Title 22, Division 6, Chapter 8 of the California Code of Regulations. On February 4, 2025, Licensing Program Analyst (LPA) Eboni Bentley arrived at the facility unannounced, for the purpose of conducting a required one-year annual visit using the CARE Inspection Tool. LPA Bentley was greeted and granted entry by Caregiver (CG) Roel Atanacio, and explained the reason for the visit. Administrator (AD) Julie Way Conejo arrived a short time later and was present for today’s visit. AD Cornejo has an Administrator Certificate with an expiration date of June 3, 2026. This facility is a one-story building in a residential neighborhood, licensed to provide services to age range 60 and over, approved for six (6) Non-Ambulatory residents of which one (1) may be bedridden and has a hospice waiver for four (4) residents. Currently, there are five (5) residents on census of which two (2) are receiving hospice care. All five (5) residents were present during today’s visit. Around 9:00AM, LPA Bentley conducted a tour of the physical plant accompanied by AD Cornejo and the following was observed: All resident bedrooms and bathrooms were inspected. Each bedroom had adequate lighting, furniture, bedding, and storage for the residents’ personal belongings. Additional bed linens, comforters, and bath towels were available during the visit. Hot water temperatures in bathrooms were measured and observed between 105.2 degrees and 113.3 degrees Fahrenheit. The kitchen was clean and organized. All knives and sharp objects were kept locked in a drawer below the counter. A supply of perishable food items was observed in the refrigerator and nonperishable food items observed in the cabinets. The garage was partially used for storage and all walkways free of clutter. There was a refrigerator with a supply of perishable items inside. LPA Bentley also observed a washer and dryer, additional boxes of supplies, and emergency food and water. CONTINUE TO 809-C PAGE.... The backyard was clean and free of clutter and debris. There is one (1) side exit gate, clear and accessible and a shaded patio area with tables and seating observed. There are no bodies of water. A review of five (5) residents (R1-R5) service files and four (4) staff (S1-S4) personnel files was conducted. Three (3) residents and two (2) staff interviews were reviewed. A review of the Medication Records Administration (MAR) and medication was conducted, and records were found to be in compliance. The facility has liability insurance effective July 20, 2024 through July 20, 2025. A first aid kit was observed with all the required elements present. There are two (2) fully charged fire extinguishers, observed mounted with a last service date of March 19, 2024. Smoke detectors were tested and operational and the last fire and disaster drill was conducted on January 6, 2025. Three (3) deficiencies and two (2) Technical Violations were cited during today’s inspection visit. Type of visit: Case Management - Health Checks

On today's date, Licensing Program Analyst (LPA) Rosie Quiroz conducted an unannounced Case Management Health and Safety check visit on today's date. LPA Quiroz was greeted by Caregiver Rhoda Yturralde and discussed the purpose for the visit. House Manager (HM) Roel Atanacio arrived shortly after. LPA Quiroz toured the inside and exterior of the facility and no immediate health/safety hazards were observed. LPA Quiroz inspected resident bedrooms and interviewed with residents who stated they had no complaints. LPA Quiroz observed 12 residents playing bingo in the back yard area. (HM) Atanacio indicated "6 additional residents are visiting from other sister facilities. We started joining the facilities together for games and socialization time." LPA Quiroz inspected facility food supply and was observed to be in quantity to meet the regulatory requirements during this inspection. Staffing was observed as House Manager and five caregivers present during today's visit. No deficiencies are being cited in the areas observed during today's visit. On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz conducted an unannounced visit for the purpose of conducting a required Annual inspection. LPA was greeted and granted entry into the facility by Caregiver 1 (CG1). LPA Quiroz called Administrator (AD) Karmian Calangi and explained the nature of the visit. House Manager (HM) Roel Atanacio arrived during today's visit. This facility is licensed to provide services to age range 60 and over, approved for six (6) Non-Ambulatory Residents of which 1 (one) may be bedridden and has a hospice waiver for four (4) residents. There are (2) two residents receiving hospice services.(AD) Calangi has an Administrator Certificate with expiration date of 6/20/2025. LPA Rosie Quiroz along with (CG1) toured the interior and exterior of the facility. There are six (6) residents in care and there are no active COVID-19 cases in the facility at this time. During today's inspection tour, LPA Quiroz observed five of six residents in their bedroom resting and of one of six residents in the living room area watching television with staff supervision. LPA Quiroz interacted and interviewed with staff and residents during today's visit. Between 12:52pm-1:20pm while conducting inspection tour of kitchen, LPA Quiroz observed expired condiments and other food items, with expiration dates varying from 7/12/2023- 2/6/2024. This was verified with (CG1) (CG2) and (HM) Roel Atanacio. (See LIC 809-D) Between 1:30pm-1:40pm, while LPA Quiroz inspected resident's bedrooms and bathrooms. Water temperatures were recorded to be between 110.5-112.4 degrees Fahrenheit. LPA Quiroz inspected resident’s bedrooms and appeared to be clean. While inspecting Resident 6(R6s) bedroom area, LPA Quiroz observed medication in (R6s) bedroom area but not limited to Peptobismol, Zyrtec, Antacid tables, NyQuil and fluticasone nasal spray. This was verified with (CG1), (CG2) and (HM) Atanacio. (See LIC 809-D) Facility temperature in resident's bedrooms and throughout the facility was recorded to be within normal limits.CONTINUED ON NEXT PAGE... CONTINUED...LPA Quiroz observed the emergency and disaster and evacuation plan. Facility has a supply of emergency food, water and PPE in garage area and facility entrance readily available for staff and residents. LPA Quiroz toured the outside of the facility and observed seating and shaded area backyard for residents and visitor's enjoyment in backyard area. During today's visit, while LPA Quiroz provided Consultation on Title 22 and Infection control. Citation were issued during today’s visit. An exit interview was conducted with (HM) Roel Atanacio via telephone and with (CG1) at the facility.
