On April 8, 2026, at 2:00 PM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim met with Caregiver (CG)Nathaniel Franco Puzon. CG Puzon called Administrator (ADMIN) Jarren Manalo over the phone and LPA Kim explained the purpose of the visit. ADMIN Manalo stated that CG Puzon could sign on behalf of the facility. The facility is licensed to operate for six (6) non-ambulatory, of which one (1) may be bedridden and have a hospice waiver for six (6) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: six (6) resident bedrooms, one (1) staff bedroom, three (3) bathrooms, living area, dining area, office area, kitchen, outdoor covered patio, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with CG Puzon. There were no bodies of water or obstructions in the facility. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, Resident Room 5, and Resident Room 6. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured between 107.9 degrees F to 108.3 degrees F. A comfortable temperature of 75 degrees maintained in the facility. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food both were available and maintained properly. LPA Kim observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. Emergency food, emergency water, and emergency supplies are stored in the garage. A working telephone (714-844-2282) and an internet accessible device dedicated to the residents that can do video teleconference which both remain available. Last emergency drill was conducted on January 12, 2026. First aid kit is maintained and contains all the necessary elements. The facility has two (2) fire extinguishers which are located and mounted near the entrance and next to room 4 and both were last serviced on March 23, 2026. Evidence of liability insurance is effective June 15, 2025, and expires on June 15, 2026. LPA Kim conducted an audit of resident files (R1-R6), staff files (S1-S4), and medication and medication administration record. R1 did not have a pre-appraisal or current reappraisal on file during the visit. R2 and R3 had outdated appraisals in their files during the visit. R2's appraisal was from January 6, 2025 and R3's appraisal was from July 20, 2024. LPA Kim conducted two (2) resident interviews and one (1) staff interview. A deficiency was cited during the visit per Title 22 Division 6 Chapter 8 of the California Code of Regulations. R1 did not have a pre-appraisal or current reappraisal on file during the visit. R2 and R3 had outdated appraisals. R2's appraisal was from January 6, 2025 and R3's appraisal was from July 20, 2024. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Jessica Cho arrived announced for the purpose of conducting the Pre-Licensing visit for a Change of Ownership (CHOW). LPA conducted the visit with Applicant/Chief Executive Officer (CEO) William Manalo and Administrators (Admins) Jarren Manalo, BSN, RN and Rhadzivil Baylon. The initial application to operate a Residential Care Facility for the Elderly (RCFE) was received by the Department of Social Services on November 20, 2024 for age range 60 and over. LPA toured the facility interior and exterior with Applicant William and Administrators Jarren and Radzivil. The following was observed: Structure: The facility is a single story property in a residential neighborhood comprised of six resident bedrooms with three resident full bathrooms and one staff bedroom. LPAs observed all common areas which includes the living room, dining area, kitchen, and an attached two car garage which doubles as a laundry area. There is a secured shed in the patio to store supplies. The backyard has sufficient shaded seating area and one exit gate. Telephone Number: Facility land line number (714) 844-2282 was tested and remains available. Signal System: No signal system, however residents utilize call buttons to alert the staff. Bedrooms: The resident bedrooms had all required components, are spacious, and easily accommodates the residents’ furnishings. Bathrooms: Bathrooms were clean and operational. Grab bars were secure and bath slip mats were in place. Linens and Hygiene Supplies: Clean linens and hygiene supplies were observed to be fully stocked in the walk-in linen closet. Appliances: Stove burners, microwaves, refrigerator, freezer, and washer/dryer were inspected and operating. Resident and Staff Files: Resident and staff records will be maintained on site per administrators. Reading Material, Games, Equipment, & Materials: The facility maintains games and activities in the living room area. Emergency Phone Numbers/Exit Plan: Posted on the bulletin boards. Postings: The See Something, Say Something (PUB475) and the Ombudsman Posters were posted in the entry way in the correct size as well as the Rights of the Resident Councils, Resident's Rights, Theft & Loss Policy, Activity Schedule, and the Food Menu. A copy of the Admission Agreement is available upon request. Food Service and Menu: Supply of seven day non-perishable and two day perishables were observed in the kitchen and garage cabinet. The sample menu was posted on the bulletin board. The emergency food/water supply were also available in the garage. Smoke and Carbon Monoxide Detectors: The smoke detectors and carbon monoxide alert systems and the auditory devices on all exit doors were tested and found to be operational. Fire Extinguishers: Two fire extinguishers were mounted, fully charged, and serviced on September 13, 2024. Fire Clearance: Approved on December 18, 2024 for 5 non-ambulatory and 1 bedridden residents. Please note that only bedridden resident is permitted in bedroom 6 and the bedridden room door shall self-close at all times per inspection report. Toxins and Sharps: Cleaning supplies, toxins, and sharps were observed to be secured and inaccessible. Water Temperature: The water temperature in the three resident bathrooms measured at 116.9, 115.7, and 117.6 degrees Fahrenheit. Medications, First Aid Kit, & Manual: The First Aid Kit was checked and found to be in order. Component III: Component III was waived because Applicant is operating other licensed facilities and has fulfilled this requirement. The Pre-Licensing is now complete, and the facility is ready for licensure. The license will be granted upon completion of a final review and approval from the Licensing Program Manager and the Central Applications Bureau.
