Type of visit: Required - 1 Year

On 06/23/26, Licensing Program Analyst (LPA) M. Yang arrived at the facility unannounced to conduct the Annual Inspection. LPA met and toured facility with Administrator Douglas Rice. Facility has sufficient furnishings inside and outside for resident use. The facility was observed to be at a comfortable temperature, clean, and no passageway obstructions or fire hazards. Residents were observed seating in common areas. Facility is equipped with pull stations and fire sprinklers throughout the facility. Fire extinguisher was observed throughout the facility with a service date of: 09/09/2025. LPA toured kitchen. An adequate supply of perishable and non-perishable food was observed to be properly stored in freezer, refrigerator, and pantry. Rotten fruits were observed. Expired food was observed. Temperature observed for the refrigerator maintained at 32 degrees F and freezer with 0 degrees F. Medications are kept lock in medication cart in medication room. Chemicals stored and locked in house keeping’s room. A sample of resident bedrooms was toured. LPA observed a chemical bottle in R1’s bedroom. Medications were observed unlocked in R2’s bathroom. Bathrooms hot water temperature was tested at 109.7 degrees F in room 105, 110.2 degrees F in room 107. 110.8 degrees F in room 402 and 111.4 degrees F in room 507. LPA observed securely fastened grab bars in bathrooms. Nonskid strips in residents’ showers. A sample of residents' files was reviewed. Courtyard was toured and observed with available seating for residents with shade. Carbon monoxide was observed operational during visit. Technical Support Program (TSP) assistance was offered. Administrator will make a decision and reach out the department regarding acceptance. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6 see attached 809D. Exit interview was conducted. The requested documents are to be submitted to the department by 06/26/6: Lic 308, Lic 500, Lic 610E and current liability insurance. On 07/24/25, Licensing Program Analyst (LPA) M. Yang arrived at the facility unannounced to conduct the Annual Inspection. LPA met and toured facility with Administrator Douglas Rice. Facility has sufficient furnishings inside and outside for resident use. The facility was observed to be at a comfortable temperature, clean, and no passageway obstructions or fire hazards. Residents were observed seating in common areas. Facility is equipped with pull stations and fire sprinklers throughout the facility. Fire extinguisher was observed throughout the facility with a service date of: 09/12/2024. LPA toured kitchen. An adequate supply of perishable and non-perishable food was observed to be properly stored in freezer, refrigerator, and pantry. Temperature for refrigerator observed maintained at 34 degrees F. LPA observed exits to have a 15-second delay egress. Chemicals stored and locked in house keeping’s room. A sample of resident bedrooms was toured. LPA observed a chemical bottle in R1’s bedroom. Bathrooms hot water temperature was tested maintained within range between 108.8 to 111.0 degrees F. LPA observed securely fastened grab bars in bathrooms. Nonskid mat/ strips was not observed in room 102, room 202, and room 407. A sample of resident files were reviewed. Due to time constraints, LPAs will return at a later date to finish the inspection tool and issue citations. Exit interview was conducted. Allegation investigated: Staff do not respond to resident's call for assistance in a timely manner Facility alarm is in disrepair Facility does not have a certified Administrator

On 03/27/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced a subsequent complaint visit and deliver complaint findings on the above allegation. LPA introduced self, stated the purpose of the visit, and met with Benched Executive Director Angelina Rodriguez. Administrator Douglas Rice was called and unable to reach Administrator. During the course of the investigation, the Department conducted interviews, toured the facility, and reviewed records. Staff responded to call signal within a timely manner. Fire alarm was tested and observation operational during investigation. Administrator certificate is currently valid. Based on records reviewed, interviews conducted and observation, the preponderance of evidence standard has not been met, therefore, the above allegations are found to be UNSUBSTANTIATED. An exit interview was conducted. A copy of this report was provided to the Benched Executive Director, whose signature on this form confirms receipt of this report. Allegation investigated: Facility staff caused bruising to resident Facility staff handled resident in a rough manner Facility staff are forcing residents out of bed against their will

On 11/06/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver complaint findings on the above allegations. LPA stated the purpose of the visit and met with Memory Care Director Kelli Porter. During the course of the investigation, LPA toured the facility, reviewed records, and conducted interviews. R1 confirmed bruising was not caused by staff. Residents confirmed staff assists the residents as needed. Residents confirmed that staff do not force resident out of bed against their will. Residents sleeps in until the residents are ready to get up to get ready for the day. Based on interviews which were conducted, staff alleged caused bruising to resident, staff alleged handled resident in a resident in a rough manner and forcing resident out of bed against their will, the preponderance of evidence standard has not been met, therefore the above allegations are found to be UBSTANTIATED. An exit interview was conducted. A copy of this report was provided to the Memory Care Director, whose signature on this report confirms receipt of this report. Type of visit: Required - 1 Year

On 07/09/24, Licensing Program Analyst (LPA) M. Yang arrived at the facility unannounced to conduct the Annual Inspection. LPA met and toured facility with Administrator (A1) Angela Ohanian. Facility has sufficient furnishings inside and outside for resident use. The facility was observed to be at a comfortable temperature, clean, and no passageway obstructions or fire hazards. Residents were observed seating in common areas. Facility is equipped with pull stations and fire sprinklers throughout facility. Fire extinguisher was observed throughout the facility with a service date of: 09/15/2023. LPA toured kitchen. An adequate supply of perishable and non-perishable food was observed to be properly stored in freezer, refrigerator, and pantry. Temperature for refrigerator observed maintained at 40 degrees F and freezer maintained at -7 degrees F. LPA and Administrator observed janitor cart with cleaning chemicals unlock set aside in hall between the receptionist desks and facility bathroom. LPA observed exits to have a 15-second delay egress. Chemicals stored and locked in house keeping’s room. LPA toured a sample of resident bedrooms. LPA observed medications and tools unlock in resident’s room. Bathrooms hot water temperature was tested and within range between 109.6 to 113.3 degrees F. LPA observed securely fastened grab bars and non-skid surfaces in shower. A sample of resident and staff files were reviewed to have all the required documents. Medications were stored in a locked medication room in a medication cart. MARs and medications were reviewed. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit interview was conducted. The following documents are requested and submitted to Fresno CCL by: 07/15/24. The following updated forms were requested: Lic 308, Lic 500, Lic 610E. On 03/19/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver complaint finding on the above allegation. LPA stated the purpose of the visit and met with Angela Ohanian, Administrator. During the course of the investigation, the Department conducted interviews with staff and reviewed records. Based on records reviewed and interviews which were conducted, there was insufficient evidence to prove or disprove that the resident was hospitalized with unexplained traces of drug found. Therefore, the above allegation is found to be UNSUBSTANTIATED. An exit interview was conducted. Allegation investigated: Facility is overcharging a resident in care. Staff terminated resident's physical therapy services without obtaining consent.

On 01/02/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver findings on the above allegations. LPA introduced self, stated the purpose of the visit, and requested to meet with Administrator Angela Ohanian. LPA met with Business Office Petra Vargas. Administrator was called and unable to attend meeting. LPA delivered findings to Administrator via telephone. Administrator authorized Business Office to received and sign report. During the course of the investigation, the Department conducted interviews and reviewed records. Based on records reviewed and interviews conducted, R1’s Admission Agreement was not completed and not signed. R1’s physical therapy was discontinued on 06/07/23 by the resident’s Home Health Agency. However, R1 does not reside at the facility. R1 is a resident who resided in a memory care facility, therefore, the above allegations are UNFOUNDED, meaning they were false, could not have happened, and/or are without reasonable basis. We have therefore dismissed the complaint. An exit interview was conducted. A copy of this report was provided to the Business Office, whose signature on this form confirms receipt of this report.
