On 11/18/2025, Licensing Program Analyst (LPA) J. Duarte and J. Leffall, arrived unannounced to conduct required Annual inspection. LPAs introduced self, stated the purpose of the visit, and was greeted by staff Patricia Arreguin. LPA J. Duarte contacted Licensee, Jason Johnson via telephone and advised of today's visit. Licensee Jason stated that Administrator Diana Diaz will assist with this visit. Administrator Diana Diaz arrived shortly after and a tour was conducted. The facility was observed to be at a temperature of 74 degrees F, clean, in good repair, and no passageway obstructions were observed. The living room has sufficient seating for residents. The kitchen was toured and LPA observed a two-day supply of perishable and seven-day non-perishable food. The facility stores kitchen knives in a locked hallway closet. A fire extinguisher was observed in the kitchen, with a service date of 05/23/25. All bedrooms were toured and observed to have the required furniture and adequate lighting. Bathrooms were observed operational, with non-skid strips and grabbed bars observed for the showers and toilets. The hallway restroom hot water measured at 108.3 degrees F and the hot water to the restroom connected to the room measured at 107.4 degrees F. Chemicals were observed stored and locked in the garage. The facility has a dryer and washer in the garage. The facility has an additional garage and freezer in the garage with an additional food supply. Outside of the facility toured and observed to be free of debris. The carbon monoxide/smoke detector was observed operational during inspection. Medications observed kept locked in facility office. Medications were reviewed along with MARS and observed to be administered as prescribed. A first aid kit is also stored in a hallway closet. The last fire drill was conducted on 11/6/2025, per staff records. A sample of resident files and staff files were also reviewed and observed to have required documentation. This is an amended report. Continued from LIC 809. The following deficiency was observed during visit: R1's bedroom was observed to have a second bed that is not in use, obstructing the sliding glass door in the bedroom, which is an exit that leads to the back patio. An office meeting will be scheduled with the licensee to discuss the deficiency observed on today's inspection and citations will be issued if warranted. An exit Interview was conducted. Allegation investigated: Staff are interfering with residents right to select their own health care agency

On 03/18/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver complaint findings on the above allegation. LPA introduced self, stated the purpose of the visit, and met with Diana Diaz,Program manager. During the course of the investigation, the Department conducted interviews, toured the facility, and reviewed records. R1’s physician referred resident for referral and treatment planning for hospice. Based on records reviewed and interviews conducted, there was insufficient evidence to prove that staff interfered with the resident’s rights to select their own health care agency therefore, the above allegation is found to be UNSUBSTANTIATED. On 10/15/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct required Annual inspection. LPA introduce self, stated the purpose of the visit, and was greeted by Licensee/ Administrator Jason Johnson and Program Director Diana Diaz. House Supervisor Marina Solorzano arrived shortly. Licensee left during inspection. LPA toured facility with Program Director, and House Supervisor. Three residents observed during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. Fire extinguisher was observed with a service date of: 06/10/24. Temperature maintained for refrigerator at 35 degrees F and freezer at 0 degrees F. Medications observed locked in hall closet. All bedrooms were toured and observed to be required furniture and adequate lighting. Bathrooms were toured. Toilet observed functional and operational. Non-skid strips and grabbed bars were observed. Hot water temperature was tested 106.3 degrees F in hall bathroom and 109 degrees F in shared bathroom. Outside of facility toured and observed to be free of debris. Carbon monoxide and smoke detector observed operational during inspection. No deficiency cited during visit. Exit Interview conducted. Allegation investigated: Unlawful Eviction

Licensing Program Analyst (LPA) Shawna Doucette and Rachel Bruce conducted an unnannounced visit to investigate a complaint and deliver findings. LPA met with House Manager, and discussed the purpose of the visit. House Manager Diana Diaz responded to the facility to assist with the visit. LPA conducted interviews. Based on interviews R1 was not evicted. R1 required a higher level of care. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED. Licensing Program Analyst conducted an Annual Inspection visit. LPA Williams met with Program Director, Diana Diaz and discussed the purpose of the visit. Administrator, Jason Johnson arrived at a later time. LPA Williams toured the facility with Program Director. The kitchen was sanitary and in good repair. There were 2 days of perishable food and 7 days nonperishable food. The dining and living room had seats to accommodate all residents. The facility thermostat reflected approximately 75 degrees Fahrenheit (F). LPA Williams observed one bedroom. The bedroom had a bed, with required linens, night stand, dresser, chairs, and working light. There was space for clients to move around and the rooms were personalized. Two bathrooms were sanitary and in good repair. There were non-slip strip and grab bars available for resident use. Smoke detectors and carbon monoxide detectors were present and operational. First aid kit was present and had all required items. LPA observed medications, chemicals, and sharps to be locked and inaccessible to residents. *Continued on LIC 809C* The backyard had seating available for residents and there was an overhang with shaded area. There is no pool on the premises. LPA reviewed four client files and three employee files. All files had documentation that was requested. No deficiency was cited during this visit. LPA requested the following documents be provided to the Department: Liability Insurance, LIC 500 and LIC 308.
