Licensing Program Analyst (LPA) Angela Hood arrived at the facility unannounced and met with the Administrator, Kiki Smeu, to conduct a Required-1 Year Inspection. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. There are six (6) bedrooms and five (5) bathrooms for resident use. LPA observed bedrooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 110.9 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two-day perishable and (7) seven-day non-perishable food supply on hand. LPA observed the outdoor area and perimeter of the care home to be free of clutter and debris and there appeared to be no potential safety hazards to the residents in care. Smoke and carbon monoxide detectors are operational. Fire extinguisher and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed three (3) resident files and also reviewed two (2) staff files. As a result of this visit, no deficiencies were cited per California Code of Regulations, Title 22. Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced and met with Acting Administrator Ingrid Negru to conduct an annual inspection utilizing the inspection tool. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. LPA observed six (6) resident rooms, seven (7) resident bathrooms, kitchen, common areas and perimeter of care home. LPA observed rooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 109.6 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two day perishable and (7) seven day non-perishable food supply on hand. Smoke detectors and carbon monoxide detectors are operational in the care home. Fire extinguishers and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed six (6) resident files, three (3) staff files and two (2) resident medications. Facility has a current copy of certificate of liability insurance and LPA obtained a copy. As a result of this visit, no deficiencies were cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. On 04/09/24 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a required 1 year annual inspection. LPA met with Administrator Christine Smeu and explained the purpose of the visit. Administrator has a current certificate (#6047966740) with an expiration date of 05/13/2024. LPA and administrator toured the interior and exterior of the facility to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, six (6) residents bedrooms, two (2) full bathrooms, four (4) half bathrooms, kitchen, garage and backyard. LPA observed required furniture, and lighting throughout the residents' bedrooms and facility. LPA observed food supplies of non-perishables for a minimum of seven (7) days and perishable foods for a minimum of two (2) days. Toxins and cleaning supplies are locked and inaccessible to residents in care. Hot water temperature was measured at 114.6 degrees Fahrenheit at the kitchen sink, which is within the required range of 105 to 120 degrees. The temperature in the facility was 72 degrees. First aid kit was completed. LPA observed fire detectors and carbon monoxide detectors to be operable. LPA observed the fire extinguisher, located in kitchen, which was last inspected on 10/09/2023. LPA reviewed drill logs. LPA observed required Licensing posters posted throughout the facility. LPA conducted a file review of two (2) personnel and six (6) residents records. All records have the required documents. LPA conducted one (1) resident interview and one (1) staff interview. Medications are centrally stored, locked, and appear to be given per doctor order. LPA compared medications to those being given for two (2) residents and found no discrepancies. Facility is correctly using the Medication Administration Records (MAR). Type of visit: Case Management - Incident

On 10/04/2023, Licensing Program Analyst (LPA) Sarena Keosavang arrived at the facility unannounced to conduct a Case Management- Incident to obtain information regarding an incident that occurred at the facility on 09/21/2023. LPA met with administrator, Christine Smeu, and explained the purpose of the visit. The Case Management visit is in response to an incident report and Report of Suspected Dependent Adult/Elder Abuse (SOC 341) that was submitted to Community Care Licensing (CCLD). Incident report indicates, resident (R1) arrived at the facility after being discharged from the hospital with responsible party (RP). RP asked staff (S1) to assist R1 with transfer. S1 refused due to inability to lift. RP assisted R1 with transfer from bed to chair. RP notified administrator that RP witnessed S1 yelling at resident (R2) in care. The facility conducted an internal investigation and S1 was asked to take a leave of absence from direct patient care. Community Care Licensing and Long Term Care Ombudsman were notified. On 09/21/2023, LPA Keosavang received a telephone call from acting administrator, Ingrid Smeu. Acting administrator reported the incident via telephone. LPA advised to provide training to all staff. Training on elder and dependent adult abuse and communication considerations when caring for person with Dementia were completed by all staff. The facility submitted training for review on 09/28/2023. At this time, deficiencies are not being cited.
