Unannounced case management visit made out to this facility on 03/30/2026 by this Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility representative, Rachel Rae Santiago, who was briefly interviewed and was requested by this LPA to inform the facility designated Administrator, Rolando Santiago, that CCL was present at this time. The facility designated Administrator, Rolando Santiago, was present and briefly interviewed at this time. Current census was 3 residents. The purpose of this case management visit was to follow up on some potential issues that were recently brought to the attention of this LPA at this time: Wood Flooring in need of repair/replacement Tiles in common areas in need of repair/replacement Missing window screens-others in need of repair/replacement Facility perimeter fence The following deficiencies were observed and cited on the following LIC 809-D pursuant to Title 22 Rules and Regulations, Health and Safety Codes. Unannounced Annual visit made out to this facility on 02/26/2026 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility designated Administrator, Rolando Santiago, who was briefly interviewed at this time. This LPA was also met by the facility Program Manager, Rachel Rae Santiago, who was briefly interviewed at this time. Current census was 4 residents. This facility was also vendorized through Valley Mountain Regional Center (VMRC) to be able to accept and retain residents receiving their services at this time. It was learned that there weren't any residents under the care of hospice at this time. This facility does have an approved waiver to be able to accept and retain up to (6) residents under the care of hospice at any given time. It was learned that this facility has a program to be able to accept and retain dementia residents at any given time. It was learned that there was (1) resident diagnosed with dementia at this time. It was learned that there weren't any residents receiving services through home health at this time. It was learned that this facility does not have a bedridden fire clearance to be able to accept or retain any residents deemed to be bedridden at any given time. Tour of this facility was conducted. Dining area, living area, and all other areas intended for resident use were toured. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Linen closet, located in facility hallway closet, was reviewed and observed to contain a sufficient supply of towels, sheets, and bedding able to meet the needs of the residents at this time. Kitchen area was toured. Kitchen drawers and cabinets were opened and reviewed. Food supply for 2-day perishable and 7-day nonperishable quantities was reviewed to make sure that they were in compliance at all times. Pantry area was toured. Laundry area, located at the rear of this facility, was toured and observed to be made inaccessible to the residents at this time. A review of the bleach, detergent, and all other cleaning supplies was conducted to make sure that they were locked and made inaccessible to the residents at this time. Administrator certificate # 7023227740 for Rolando Santiago was observed to have an expiration date of 09/14/2026 and in compliance at this time. Medication cabinet, located in the facility office cabinet, was observed to be locked and made inaccessible to the residents at this time. First aid kit, located in the medication cabinet, was reviewed. This LPA observed that it did contain all of the required components at this time. Fire extinguishers located hanging on the wall throughout this facility were observed to have been annually inspected by the local fire extinguisher company, Butch Young Fire Equipment, with the inspection date of 01/08/2026 and in compliance at this time. Facility resident bedrooms were toured. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Facility resident restrooms were toured. Grab bars and non skid mats were observed to be present and in good repair at this time. Hot water temperatures were taken to make sure that they were within the allowed range of 105-120 degrees at all times. A tour of the facility exterior grounds was conducted. A review of the facility perimeter fence, side gates, and all other exits was conducted. A review of (5) facility personnel records was conducted and noted on the following LIC 859. A review of (4) facility resident records was conducted and noted on the following LIC 858. Type of visit: Case Management - Incident

LPA arrived unnannounced to the facility on 1/21/26 at 10:45am to follow up with a significant incident report involving a resident that had a bacteriological infection leading to siezures. LPA explained the purpose of the visit to Administror Rolondo Santiago. LPA reviewed most recent discharge order dated 12/19/25, instructs the client to complete antibiotic course, folow up with neurologist, follow up with primary care physician(pcp). 12/29/26, PCP(worried about MRSA, wont perscribe more than short course (IV in the hosptial is most effective), Enlarged prostate complicates inspection of the area, diabetes complicates symptoms that appear similar., Idd affects catheter candidacy), neuro scheduled today 1/21/26, urology scheduled for the future, 1/26/26. LPA reviewed MAR, the antibiotic course was finished 12/26/25, all medication dispersed no notes. Per Administrator interview about the PCP visit, Siezures mostly triggered by infection, headache, Focal siezure, confusion are the dominant symptoms. Staff are trained, via in-service to review medications(antibiotics), when staff are hired as part of the initial training(siezure, diabeties are reviewed). Ongoing measures taken by the facility are to Monitor and log hydration, Bowl Movements, siezures. LPA reviewed logs, they are filled out to the current date. Per staff interview, 2 staff verbally confirmed they aware of the signs and symptoms that should be noted in daily logs. Per client interview, the client appears stable. monitoring and going into the hospital as appropriate has brought down the number of infections requiring hosptialization in the past. client has a call button for immidiate distress. On 2-14-2025 at 10:10am, Licensing Program Analyst (LPA) Michael Bilger arrived at this facility unannounced to conduct an annual inspection visit. LPA met with the administrator Rolando Santiago and explained the purpose of the visit. LPA inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, living area, common TV area, and outside of the facility to ensure compliance with Title 22 regulations. Facility is a residential care facility for the elderly (RCFE) with a current census of 3. Facility has 5 bedrooms and 3 bathrooms for resident use. Facility has a dining area off the kitchen and a formal living room. LPA also conducted the inspection using the CARE tool. Facility currently provides care for 1 ambulatory residents, 2 non ambulatory residents, 0 hospice, and 0 bedridden. The facility has an approved infection control plan in place. Water temperature reads 105*F to 120*F in the bathroom and room temperature reads 70*F. LPA observed the facility to have adequate food supply. Resident rooms were sanitary and had the required furniture and furnishings. The facility common areas were clean and furnished. Smoke and carbon detectors were in good repair. Fire extinguisher was checked 1-29-25. Facility has an emergency food and water kit. All toxins and other dangerous items including sharp objects were locked and inaccessible to residents in care. Medication storage area was observed to be locked and inaccessible to residents in care. Medications were reviewed and contained accompanying regulatory required Physician’s orders. First aid kit was observed to have adequate supplies and accessible to staff. During this inspection 3 resident files and 5 staffing files were reviewed for regulatory compliance. All files contained required contents including staff training requirements. All staff noted on LIC 500 contained criminal background clearances. LPA completed 2 resident interviews and 2 staff interviews. {Cont. on 809C} Resident files reviewed contained all required contents including updated admission agreements, medical assessments, and updated appraisal forms as required. Facility’s surety bond is current and update to date per regulatory requirements. Facility does not contain any bodies of water. LPA observed personal rights and complaint information posted. Facility has appropriate internet access available for resident use. LPA observed facility to have sufficient equipment and supplies to meet activity program needs of residents in care. LPA reviewed facility’s disaster plan to ensure regulatory compliance. Facility conducts monthly fire drills. LPA requested an updated copy of LIC 308 and LIC 500. Per California Code of Regulations, Title 22, no deficiencies were observed during this visit. Licensing Program Analyst (LPA) Ruth Wallace conducted unannounced required 1 year annual inspection visit. LPA met with Administrator and explained purpose of visit. LPA and administrator inspected the physical plant including but not limited to the kitchen, dining room, client bedrooms; client bathrooms, laundry room, activity room, and outside courtyards. LPA observed sufficient furniture and lighting throughout the facility. LPA observed sufficient seven day non-perishable and two day perishable food supplies. LPA measured the hot water temperature in client's bathroom at degrees Fahrenheit which is within the required range of 105 to 120 degrees. Fire extinguishers last inspected on 2/8/2023. Smoke detectors are operational. LPA observed centrally stored medications are kept locked and inaccessible to clients. LPA reviewed and compared client medication vs. medication logs. First aid kit was checked and is complete. LPA observed carbon monoxide detectors in the facility. The facility conducts fire/disaster drills with residents on 12/26/2023. Licensee has been following restricted health care plan approved on 4/14/2023 for client - C2 (See LIC 811 Confidential Names). Valley Mountain Regional Center (VMRC) service coordinator has documented C2's continual refusal of medications for condition of diabetes on individual program plan last documented on 12/5/2023. Therefore no need for deficiency for not following restricted health care plan. Primary doctor, VMRC, and client are aware of the health risk factors involved. A review of staff records indicates that all facility staff or other individuals who require caregiver background checks are Fingerprint cleared and associated to the facility. LPA verified staff training for staff file reviews. LPA received the following updated documents on today's date: LIC 308 - Designation of Administrator, Copy of Surety Bond with Expiration Date, and Copy of Administrator Certificate. Per the California Code of Regulations, Title 22, Division 6, Chapter 6, no deficiencies were cited during this visit. Exit interview held with administrator.
