Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Licensee, Ritchie Espiridion, Continual Administrator's Certification expires 03/03/2028. There are currently 2 residents who reside at this home and there is 0 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. . First Aid kit is on site and complete. Staff 1 and staff 2 do not have signed LIC 503 Health Screening. Water temperature in resident hallway bathroom was tested at 131.9 degrees, and resident bedroom sink water measured to be 122 degrees. The facilities Emergency disaster plan is not updated or reviewed annually. Emergency disaster plan section identifying which staff is assigned to shutting off utilities, etc,. does not have specific staff names listed. Staff 1 and staff 2's LIC 501 Personnel record is not filled out completely. Staff 1 and staff 2 are living in bedroom listed on sketch to be used as resident bedroom. LPA observed cleaning supplies were stored in the cabinet located beneath the resident hallway bathroom sink. The cabinet was equipped with a child safety lock intended to restrict access to hazardous items. However, upon testing the mechanism, LPA was able to open the cabinet without difficulty, indicating that the lock was not effectively securing the contents The following deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Allegation investigated: Staff spoke to resident in an inappropriate manner. Staff handled resident in a physically inappropriate manner. Staff did not respond to resident's requests for assistance in a timely manner.
Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to investigate/ deliver findings on the allegations listed above. LPA met with facility Licensee Ritchie Espiridion, and explained the purpose of today's visit Regarding the allegation Staff spoke to resident in an inappropriate manner. LPA interviewed two facility residents, Resident 1's family members, and facility staff. LPA also reviewed Resident 1resident record. There is no evidence suggesting Resident 1 is being spoken to inappropriately to by facility staff. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Unsubstantiated Regarding the allegation Staff handled resident in a physically inappropriate manner. LPA interviewed facility residents, Resident 1's family members, and facility staff. LPA also reviewed Resident 1's resident record. There is no evidence suggesting facility staff is handling Resident 1 in a physically inappropriate manner. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Regarding the allegation Staff did not respond to resident's requests for assistance in a timely manner. LPA interviewed facility residents and there is no evidence suggesting the facility staff is not responding to the residents request for assistance. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. Type of visit: Post Licensing
Licensing Program Analyst (LPA) S. Hurt arrived for a post licensing inspection. LPA was met by Licensee Ritchie Espiridion. The facility has one resident who was present during this inspection. Licensee Ritchie Espiridion recently contacted LPA Hurt informing the facility has one resident. A complete tour of the facility was done. Medications are properly stored and secured in a locked cabinet. Food supply was checked and a 2 day supply of perishable and 7 day supply of non-perishable was checked. Water temperature was measured at 114 degrees F in the bathroom. Linens and resident's hygiene were observed. Fire extinguisher/smoke detectors and buildings and grounds were checked and observed to be operational. A sample of resident's and staff's files were checked. Resident's files have signed Admission Agreements and Personal Rights. Staff's files are fingerprinted clear and associated and have current CPR. No deficiency cited for this visit. Type of visit: Prelicensing
Licensing Program Analyst (LPA) Sarah Hurt conducted an announced visit to the facility for purpose of a Pre-Licensing evaluation. LPA arrived and was granted entry to the facility by Licensee Ritchie Espiridion. An initial application to operate a Residential Care Facility for the Elderly (RCFE) was submitted to the Central Applications Unit (CAU) on 09/23/2023 for a capacity of six adult residents. LPA Hurt observed the following: Structure: Facility is a one-story house with 4 resident bedrooms, 2 bathrooms, family / living room, dining area and kitchen. There is a 2-car garage attached in front of home. The resident bedrooms will accommodate residents' furnishings. Signal System: Central air/heating system installed with a central panel to control entire house. Bedrooms Residents: Bedrooms #1-4 will accommodate 6 clients (2 shared resident bedrooms, and 2 private bedrooms)Bathrooms: All bathrooms have a working toilet, wash basin, and shower. Linens and Hygiene Supplies: Adequate supply of linens is stored in hallway cabinet. Emergency Phone Numbers, Exit Plan, and Sample Menu: Will be posted and readily available for review in facility. Continued from 809C... Food Service: Adequate supply of 7-day non-perishable and 2 day perishables would be stored in the kitchen and pantry. Smoke and Carbon Monoxide Detectors: Smoke and carbon monoxide alert systems were hardwired and found operational. Fire Extinguisher: 1 Fully charged and located inside garage door Fire Clearance: Approved on 11/07/2023 Appliances: Electric four burner stove with oven, refrigerator/freezer and microwave which were clean and noted to be operational. Washer and dryer are located in the laundry area in the hallways and were clean and noted to be operational. Toxins: Will be locked away/ stored in locked cabinets in bathroom, and above washer and dryer. Water Temperature: Tested and recorded at 111 degrees (within regulation) Medications, First Aid Kit & Manual: First Aid kit with guide will be stored in locked cabinet next to kitchen area. Medication will be stored and locked in cabinet next to kitchen area. Resident and Staff Files: Records will be kept in locked kitchen area Reading Material, Games, Equipment, & Materials: The facility has materials that commensurate with their plan of operation. Continued onto 809C.. Continued from 809C .. The license will be granted upon completion of a final review and approval from the Licensing Program Manager and the Central Applications Unit. Applicant was reminded of the statute that requires notification to Licensing Program Analyst within 5 business days of admitting the first resident. This notification may be done by phone, mail, email or fax. At this time, facility has met all pre - licensing requirements of Title 22 division 6.
