Type of visit: Required - 1 Year
On 08/25/2026 at 11:00AM, Licensing Program Analyst (LPA) Andrew Christy arrived unannounced to conduct the 1-Year Annual Required inspection. LPA met with Administrator, Alicia Johnston, and explained the purpose of the visit. The facility currently houses four (4) residents with a max capacity of six (6) residents. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, common area, and backyard. All outdoor and indoor passageways are kept free of obstruction. No bodies of water were observed. A comfortable indoor temperature is maintained at 70.0 degrees Fahrenheit. The hot water temperature in the residents’ shared bathroom was measured at 109.7 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of non-perishable and 2 day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 11/07/2025. At 12:15PM, LPA reviewed four (4) resident files and five (5) staff files, all found to be complete. The emergency disaster plan was last reviewed 06/18/2026. Quarterly emergency drills were last conducted 07/15/2026. A review of resident medications and the Medication Administration Record (MAR) found no outstanding errors. No deficiencies cited during visit. Type of visit: Post Licensing
On 10/24/2025 at 12:30PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conducted a post licensing inspection. LPA met with Administrator, Alicia Johnston and explained the purpose of the visit. LPA toured facility including but not limited to resident's bedrooms, bathrooms, living room, dining area, kitchen, garage, and outdoor area. LPA observed lighting in all rooms. Hot water was measured at 106.4 degrees F in the kitchen sink. Facility bathrooms have grab bars and non-skid mat/materials for the showers. LPA observed facility have one week of non-perishable and two day of perishable food supplies available. Smoke and carbon monoxide detectors were observed. Fire extinguisher was observed to be full and last serviced on 11/22/2024. First aid kit was complete. No bodies of water observed. The facility has a written emergency disaster plan. Last disaster drill was conducted on 10/15/2025. LPA reviewed 3 residents and 3 staff files starting at 1:00PM. Residents and staff files were complete. All staff are fingerprint cleared and associated to the facility. LPA reviewed a sample of resident's medications during inspection. No deficiencies are being cited on this date. Component II completion: Successful Facility Type: Residential Care Facility for the Elderly (RCFE) Application Type: Change in Ownership (CHOW) Capacity: 6 Census (if any clients in care): 3 COMP II Participants: Alicia Johnston, Applicant/Administrator Interview Method: Virtual interview (Microsoft Teams) On August 27, 2025, Applicant/Administrator participated in COMP II. Identification of the Applicant/ Administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, Applicant/Administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB Analyst confirmed Applicant/Administrator’s understanding of following areas: 1. On 8/21/2025 at 2:45PM, Licensing Program Analyst (LPA) G. Luk conducted a Pre-licensing Inspection. LPA met with Licensee/Applicant, Alicia Johnston and explained the purpose of the visit. LPA toured facility including but not limited to resident's bedrooms, bathrooms, living room, dining area, kitchen, garage, and outdoor area. LPA observed lighting in all rooms. Hot water was measured at 107.7 degrees F in the hallway bathroom. Facility bathrooms have grab bars and non-skid mat/materials for the showers. LPA observed facility have one week of non-perishable and two day of perishable food supplies available. Smoke and carbon monoxide detectors were observed. Fire extinguisher was observed to be full and last serviced on 11/22/2024. First aid kit was complete. No bodies of water observed. Last disaster drill was conducted on 7/15/2025. No issues noted during inspection. LPA observed that facility is ready to be licensed. This report will be submitted to the Centralized Application Bureau (CAB) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAB. Additional requirements may still be required.
