Licensing Program Analyst (LPA) Lavinia Muscan arrived unannounced on 12/08/25 to conduct the annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA and RCC toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. Grab bars were present at the toilet and in the shower. All exits were unobstructed. The disaster drill is current. The administrator's certificate is current. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked . LPA observed cleaning products and other toxins to be locked away. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguishers are ready for emergency use. In the areas toured, there were no health or safety violations observed. LPA reviewed resident (3) and staff files (2). All resident files contained the required paperwork. Staff file contained the required paperwork. Staff has current first aid and CPR training. Facility is current on fire drills. All required posting were observed. Staff training contained the required initial training. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by end of the month. Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility on August 19, 2025 to conduct a case management visit about a complaint that was filed at a sister facility regarding pests. LPA interviewed two (2) residents and one (1) staff. .The facility representative stated that the pest control company comes in monthly, and more often as needed. Department reviewed Pest Control dates for the past 3 months on 06/26/2025, 07/10/2025, and 08/07/2025. The next spraying is scheduled for 09/06/2025. It was stated that the Pest Control company just visited the facility over a week ago and sprayed the exterior and interior of the building. The pest control company is continuing to monitor any pest activity. At this time, this case in under review and department will follow up as needed. Allegation investigated: Staff did not issue a refund

On 6/17/24 Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to open and deliver complaint findings into the allegation listed above and met with Administrator Jennifer Hinch. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. The results of the investigation are as follows: The Department received a report stating that the Authorized Representative did not receive a refund after a resident’s death. However, the Department received confirmation and documentation on 6/14/24 that a check was sent to the address on file. The LPA finds this allegation to be UNFOUNDED - meaning that the allegation was false, could not have happened and/or is without a reasonable basis. Allegation investigated: Licensee failed to report to the Department that the licensee filed for Bankruptcy.

Licensing Program Analyst (LPA) Lavinia Muscan conducted an unannounced complaint visit and met with Administrator Lenore Alexius. It was alleged that the Licensee did not provide required notifications due to bankruptcy. The Licensee confirmed that they have filed for bankruptcy on May 21, 2024. The Licensee was informed that they are required to notify the department, the State Long-Term Care Ombudsman, all residents, and, if applicable, their legal representatives, in writing, within two business days of filing for bankruptcy. During today's visit a facility tour was conducted, and the food was inspected which identified an adequate food supply for the number of residents in the home. Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
