Licensing Program Analyst (LPA) Becky Mann conducted an unannounced required 1-year visit to the facility. LPA met with Reynante Mangayao, Caregiver and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE). Licensed capacity of 6 with a current census of 5. LPA conducted an overall inspection of the facility, which included, but was not limited to the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature 75 degrees Fahrenheit. LPA inspected resident bedrooms; they are equipped with the required furniture such as beds, mattresses, night stands, storage space, chairs and sufficient lighting. Bathrooms were clean and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The hot water temperature tested 107 degrees Fahrenheit. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents. All the sharps are locked. There was a designated area for resident/staff files. Overall, the facility is clean, in good repair, and operating in a safe condition for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for the number of residents. Facility has a variety of food available for residents. Dishes, cups, and utensils were also stored properly. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPA reviewed (3) resident files for admission agreements, updated physician reports, and needs and services plans. Resident files are maintained and up to date. LPA reviewed (2) resident medications. Medications are labeled and administered as prescribed. Medications are kept locked and inaccessible to residents. LPA reviewed (2) staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings. Facility staff records are up to date. Based on the observations made during today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analysts (LPAs) Becky Mann and Renese Howell-Small made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA Mann and Howell-Small met with Administrator Milagros Azaula. The facility is an Residential Facility for the Elderly (RCFE). The current census is five (5) residents. The facility is a five (5) bedroom, two (2) bathroom home with a kitchen/dining area, living room, laundry area and a garage. LPAs was accompanied by Milagros Azuala, Administrator to conduct a general overall inspection, which included, but was not limited to the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). LPAs observed no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 78 degrees Fahrenheit. LPAs inspected resident bedrooms; they are equipped with required furniture such as: beds, mattresses, lamps and storage space with sufficient lightning. LPAs observed that bathrooms were clean, and appliances were operating appropriately. LPAs observed grab bars and non-skid mat in the resident bathrooms. LPAs tested the hot water temperatures in the bathroom to be between 109 and 111 degrees Fahrenheit. The facility is equipped with operating smoke detectors, carbon monoxide alarms and charged fire extinguisher. Posters such as personal rights, the CCLD complaint poster, Ombudsman poster and the disaster plan were posted in a common area. There was a designated storage space for resident/staff files. There is a cabinet with the resident’s medications locked. Food Service: Seven (7) days non-perishable and two (2) days perishable food supply observed at the facility. Care & Supervision: The facility has an administrator present in the facility. The facility has sufficient staff 24 hours, 7 days a week. Record Review: LPA reviewed three (3) resident files for admission agreements, updated physician reports, pre-placement appraisal (LIC603) and needs and services plans (LIC625). The files were complete with updated physician’s reports, admissions agreements, needs and services plan and preplacement appraisals. LPAs reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings with negative tuberculosis (TB) result. LPAs observed that two (2) out of three (3) staff do not have current trainings for the year of 2025. Technical violation issued. Three (3) resident medications were audited. Based on LPAs observation and record reviews, staff did not update Resident #1 (R1) and Resident #2 (R2) Medication Administration Record (MAR) after dispensing their medications per R1 and R2 physician's order. Deficiency cited. Based on the observations made during today’s visit, deficiencies were cited per Title 22, Division 6, Chapter 8 of the California Code of Regulations. On 04/11/2024 at 08:45 AM, Licensing Program Analyst (LPA) Melody Brown made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA Brown met with a staff and was granted entry to the facility. At the time of the visit there were three (3) staffs present, and five (5) residents present. Licensee/Administrator Milagros Azaula was contacted and informed of the visit. Licensee/Administrator Milagros Azaula arrived during the visit. LPA Brown explained the purpose of the visit to Licensee/Administrator Milagros Azaula. The facility is a five (5) bedroom, two (2) bathroom home with a kitchen/dining area, living room, laundry area and a garage. The facility is Residential Care Facility for the Elderly (RCFE). The facility is licensed for a capacity of six (6) residents of which six (6) can be non-ambulatory residents, one (1) may be bedridden. The facility has six (6) Hospice Waiver. The current census is five (5) residents. LPA Brown was accompanied by Staff #2 (S2) to conduct a general overall inspection, which included, but was not limited to the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). LPA Brown observed no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 77 degrees Fahrenheit. LPA Brown inspected resident bedrooms; they are equipped with required furniture such as: mattresses, lamps and storage space and with sufficient lightning. Moreover, LPA Brown observed that bathrooms were clean, and appliances were operating appropriately. LPA Brown observed grab bars and non-skid mat in the resident bathrooms. However, LPA Brown observed Resident #5 (R5) with half bed rail and no written order from R5's physician indicating the need for half bed rail for mobility. Deficiency will be issued. ***Continuation in LIC809C *** LPA Brown observed no appropriate signage of "No Smoking-Oxygen in Use" in Resident #3 (R3) room. Deficiency will be issued. LPA Brown measured and observed the water temperatures in the bathroom to be at 106 degrees Fahrenheit. The facility is equipped with operating smoke detectors, carbon monoxide alarms and charged fire extinguisher. Posters such as personal rights, the CCLD complaint poster, Ombudsman poster and the disaster plan were posted in a common area. There was a designated storage space for resident/staff files. There is a cabinet with the resident’s medications locked. However, LPA Brown observed residents medications pre-poured in a small container for the day, up to bedtime medication for each resident at the facility. LPA Brown explained that no medications shall be transferred between containers. Deficiency will be issued. Food Service: Seven (7) days non-perishable and two (2) days perishable food supply observed at the facility. Care & Supervision: The facility has an administrator present in the facility. LPA Brown observed no staff scheduled to work at night (NOC) shift due to the facility having dementia residents. Deficiency will be issued Record Review: LPA reviewed three (3) resident files for admission agreements, updated physician reports, pre-placement appraisal (LIC603) and needs and services plans (LIC625). The files were complete with updated physician’s reports, admissions agreements, needs and services plan and preplacement appraisals. LPA reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings with negative tuberculosis (TB) result. LPA Brown observed files were complete. However, three (3) Medications/Medication Administration Records (MARs) were audited and LPA Brown observed that staffs at the facility are not updating residents MAR when dispensing residents medications. Deficiency will be issued. Based on the observations made during today’s visit, deficiencies were cited per Title 22, Division 6, Chapter 8 of the California Code of Regulations. Type of visit: Collateral

Licensing Program Manager (LPM) Tricia Danielson and Licensing Program Analyst (LPA) Sara Martinez arrived unannounced to the facility to conduct a collateral visit regarding two (2) complaint investigations which are unrelated to this facility. LPM and LPA met with Milagros Azaula and explained the purpose of the visit. Complaint Control #18-AS-20201124140635 concern allegations of unlicensed care being provide at this address by a former occupant of this address and #18-AS-20201124133053 concern allegations made against the previous licensee who operated a facility at this address. During today's visit, LPM and LPA verified the current licensee purchased the business from Adelaida Legaspi in April 2021 and obtained a license to provide care and supervision on June 21, 2021. Azaula reported she has been in operation since that date. There are no concerns or deficiencies noted during today's visit.
