Type of visit: Required - 1 Year
On 8/23/2026, Licensing Program Analyst (LPA) Mia Lankford conducted an unannounced required annual inspection at the above location. LPA met with Licensee Daryl Lee and explained the reason for the visit and was granted entry. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 117.0 degrees F. and warning label posted as well. Laundry facilities and a locked cabinet is present for storing laundry soap in the garage and other chemicals in the hallway. All outdoor and indoor passageways are free of obstruction. A locked area is provided for medications and sharp objects. LPA verified there is a telephone working at this location. Food Service- Food supply meets the of one week supply of nonperishable and 2-day supply of perishables food on hand. A menu is posted, foods are dated to assure safety. Food prep areas are clean and organized. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation, and the structure remains unchanged according to the approved floor plan. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Fire extinguishers are tested or replaced annually and were last done so on 4/3/2026. The facility conducting emergency disaster drills and last done on 06/03/2026. Corporation or LLC is active and in good standing.(Continued on Page 2) Cont... Licensing Program Analyst (LPA) Yolanda Delgado arrived unannounced to complete the annual inspection. Upon arrival LPA was greeted by facility staff and granted entry by Elizabeth Sanchez. LPA began inspection with introduction, visit purpose and provided the facility caregiver with LPA identification. Administrator Alejandra and Richard Contreras arrived during the inspection. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 114.0 degrees F. and warning label posted as well. Laundry facilities and a locked cabinet is present for storing laundry soap in the garage and other chemicals in the hallway. All outdoor and indoor passageways are free of obstruction. A locked area is provided for medications and sharp objects. LPA verified there is a telephone working at this location. Food Service- Food supply meets the of one week supply of nonperishable and 2-day supply of perishables food on hand. A menu is posted, foods are dated to assure safety. Food prep areas are clean and organized. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. LPA began inspection with introduction, visit purpose and provided the facility caregiver with business card. Resident record review began- Six (6) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. This facility is meeting documentation requirements. Employee records review began- Three (3) records were reviewed. LPA reviewed employee record for first aid certification, fingerprint clearance, personnel/job application, health screening and TB test results, criminal record statement, employee rights, training verification, and current administrator certification. CPR and requirements have been met. The facility employs enough staff to maintain cleanliness and meet the needs of the clients in care. Allegation investigated: Facility is in disrepair
Licensing Program Analyst (LPA) Lydia Martinez made an unannounced visit to the facility to gather additional information and deliver the findings on allegation listed above. LPA introduced herself to Administrator (AD) Alexandra Contreras and explained the purpose of today's visit. During the course of the investigation, LPA toured the facility and interviewed staff. Allegation that "facility is in disrepair"; residents are being showered with buckets due to the shower not working properly. The investigation revealed the following: During the tour of the facility, LPA observed a bucket full of water and shower head leaking inside bucket (picture taken). AD acknowledged shower valves and shower head need to be repaired/replaced. Hot water was tested at 95.4 degrees F. The rest of the facility appears clean, safe, and sanitary. Based on observation, the preponderance of evidence standard has been met. Therefore, the above allegation is found to be Substantiated. California Code of Regulations, (Title 22, Division 6, Chapter 8), is being cited. This requirement was not met as evidenced by: Based on observation, Licensee failed to ensure facility is in good repair. LPA observed leaking shower head, and shower handle valves need to be replaced per AD. Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Martinez conducted a Case Management visit in conjunction with a complaint investigation visit with control number (8-AS-20230907150614.) LPA was greeted and granted entry into the facility by Caregiver Amelita Gragassin. Administrator Alexandra Contreras arrived shortly after. LPA conducted a tour of the facility. LPA measured the hot water temperature in the resident bathroom which measured at 95.4 degrees F. Staff informed LPA that the hot water "takes a long time to heat up. LPA let water run for 5 minutes and hot water gradually reached a temperature of 98.4 with LPA holding the thermometer for 1 minute. Deficiency is being cited under California Code of Regulations, Title 22, Division 6, Chapter 8. LPA began inspection with introduction, visit purpose and provided the facility caregiver with LPA identification and business card. Resident record review began- Five (5) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. This facility is meeting documentation requirements. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 105.7 degrees F. Laundry facilities and a locked cabinet is present for storing laundry soap. Chemicals stored in locked cabinet. All outdoor and indoor passageways are free of obstruction. A locked area is provided for medications and sharp objects. LPA observed a pool that has no water and has a perimeter gate that is locked and secured. LPA verified there is a telephone working at this location. Food Service- Food supply meets the requirement of one week supply of nonperishable and 2 day supply of perishables food on hand. A menu is posted, foods are dated to assure safety. Food prep areas are clean and organized. LPA reviewed employee record for first aid certification, fingerprint clearance-one not cleared, personnel/job application, health screening and TB test results, criminal record statement, employee rights, training verification, and current administrator certification. CPR and requirements have been met. The facility employs enough staff to maintain cleanliness and meet the needs of the clients in care. Administrator certification is present and current. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation and the structure remains unchanged according to the approved floor plan. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Fire extinguishers are tested or replaced annually and were last done so on 03/11/2024. The facility is conducting emergency disaster drills. The last disaster drill was conducted on 06/1/2024. LPA allocated time to prepare this report for delivery. Based on the information received during this visit today, there are two (2) deficiencies and civil penalties being assessed for $500 is being cited per Title 22, Division 6 of The California Code of Regulations. This report was reviewed with Richard Contreras, LIC809D, LIC421BG $500 Civil Penalty, Appeal Rights and a copy provided to the facility representative at the time of the exit interview.
