On 10/20/2025, LPA Maria (Mita) Partoza, conducted an unannounced POC inspection visit and met with licensee/administrator Armando Silva. LPA stated the purpose of the visit. LPA toured the facility with LIC/ADM inside and outside to address the deficiencies cited during the annual inspection. Based on today's inspection,observation and record review the facility corrected and cleared the deficiencies that were cited. LPA issued the Letter of Deficiency Citations Cleared during today's visit. Based on observation - the back ramp that was missing planks were repaired and planks that poses a potential risk of breaking were replaced. Based on observation the boxes of perishable food at the side of the ramp were removed and cleared. The water temperature is monitored on a daily basis to ensure that temperature stays within the range of 105 to 120 degree F. LPA measured the water temperature in the bathroom and kitchen with a digital thermometer. Temperature measured at 110.1 degree F. Based on record review - 3 out of 3 staff CPR 1st aid training have been completed and 3 out of 3 staff have a valid 1st aid/cpr certificate that will expire on 10/16//2027. 3 Out of 3 staff was provided the required 10 hours of continuing education. ADM started the training for 3 out of 3 staff and administered on 10/20/2025 and completed on 10/27/2025. 3 out of 3 staff training for the dementia care is ongoing and will be completed on 10/31/2025. LIC/ADM stated that he/she will send certificate of completion to LPA by 10/31/2025. No deficiencies were cited during today's visit based on California Code of Regulation (CCR) Title 22. LIcensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced visit to continue the facility required 1-year inspection, met with the licensee/administrator (LIC/ADM) Armando Silva and stated the purpose of the visit. In continuing the record review, LPA reviewed 3 Out of 3 resident record, such as but not limited to the Centrally Stored Medication and Destruction Record (CSMDR), physician's report, pre-placement appraisal and assessment report, appraisal needs and services plan, physician's report and admission agreement and found records to be complete an up-to-date. Based on the observation, and record reviews from inspection conducted on 10/13/2025, citations are issued during today's visit based on the California Code of Regulations (CCR) Title 22. Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced required 1 year inspection visit and met with Administrator (ADM) Armando Silva, and stated the purpose of the visit. The facility is licensed to serve adults 60 years old and over 6 non-ambulatory, bedrooms #1 and #3 are approved for both non-ambulatory and bedridden individuals and bedroom #2 is approved for non-ambulatory only. The facility has a hospice waiver for 6. LPA was accompanied with ADM during inspection of the facility inside and outside. The facility exterior has a ramp with missing plank creating a gap between planks and is a tripping hazard. LPA observed boxes of fruits and vegetables exposed to the element and starting to dry up and rot, collecting dust and attracts insects. ADM stated that the boxes of fruits and vegetables are property of the lessor who stays in the converted garage. LPA observed 2 vehicle parked in the side driveway and ADM stated the vehicles belongs to the lessor. The facility has cemented/paved garden area that is uneven and may pose as a tripping hazard. LPA observed 2 sheds used for storing supplies and wheelchairs no longer in use. Inside the facility, LPA observed 3 resident bedroom all are shared and 1 staff room. 3 bathroom, laundry area, kitchen, dining and main living room. The residents bedroom have functioning lights, clean bed linens, storage for resident's personal belongings. Resident's bathroom are equipped with anti skid/slip mats and grab bars. The hallways and walkways inside the facility are free from obstruction and debris and is well lighted. LPA observed laundry supplies and chemicals are locked and not accessible to residents in care. In the kitchen, LPA observed that sharps and medications are locked and not accessible to residents. LPA observed 2 days of perishable food and 7 days of non-perishable food. LPA observed that the kitchen is clean and organized. The kitchen and bathroom water temperature measured at 139.9 degree F to 141.1 degree F. The facility is equipped with smoke and carbon monoxide alarm system and fire extinguisher was last inspected on 09/17/25. LPA reviewed 3 staff record. 2 Out of 3 staff record training are outdated, 2 out 3 staff LIC 501, requires to state that they are employed by the facility and not by the old licensee. 3 Out of 3 staff has CPR/First Aid that are expired. 3 Out of 3 staff record have cleared fingerprint and criminal background record. 3 Out 3 staff health screening are cleared. Due to time constraint, the annual inspection will be continued at a later date. The following deficiencies will be cited at a future date based on the initial observation of today's visit (10/13/2025), 87705 Care of Persons with Dementia, 87411 Personnel Requirement, 87412 Personnel Record, 87303 Maintenance and operation and 87555 General Food Service Requirements. Licensing Program Analyst (LPA) Steve Chang conducted an pre-licensing inspection visit and met with Administrator (ADM) Armando Silva. Two residents and two staff were observed in the facility. Living room, dining room, kitchen, 3 shared resident bedrooms, 1 staff live-in room, 2 resident restrooms, 1 staff restroom, and laundry room were inspected. Two day perishable food supplies and seven day nonperishable food supplies were observed sufficient. Room temperature was observed at 82 degree F. Hot water temperature was observed at 107 degree F. Bars and non-skid mats were observed in the restrooms. Resident bedrooms were observed with window screens. The temperature of the refrigerator was measured at 40 degree F and the temperature of the freezer was measured at 0 degree F. Fire extinguisher was serviced on 09/08/2024. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Smoke detectors was tested by ADM, and were working fine. Medications closet, knives closet were observed locked. Medications closet was observed locked. Knives closet and dish washing solution closet were observed locked. First aid box, flash lights and night lights were observed in the facility. LPA toured the backyard with ADM. Three storage rooms were observed at the backyard. One big room with 2 bedrooms, 1 living room, kitchen and garage inside was observed at the backyard. The big room at the backyard has a drive way and exit door to exit the property. The big room has no access to the facility and with separate route to exit the property. ADM stated the landlord lives in the big room. No obstruction blocks the exits. The last time the facility conducted the emergency drill is on 7/15/2024. Component III was conducted with ADM. No deficiency/citation noted today. Exit interview was conducted with ADM. Identification of the applicant and administrator was verified by confirming driver’s license number. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB.
