Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced 1 year required visit. LPA announced the purpose of the visit. LPA observed 2 staff and 6 residents. LPA met with Designated Administrator (DA) Iluminada Yap. During visit LPA toured the kitchen and observed locked cabinet for cleaning supplies and Sharp objects. LPA observed perishable food supply of at least two days and a non-perishable food supply of at least seven days. Refrigerator temperature measured with thermometer at 42 degrees F and freezer at 0 degrees F. LPA toured 4 resident bedrooms and observed room with bedding, dresser, and lighting. LPA toured 2 resident bathrooms, and 1 staff bathroom. Bathrooms water temperature measured with thermometer between 114-119 degrees F. LPA observed a locked cabinet located in kitchen where residents medication, resident records along with staff files are kept. During tour LPA observed the hallway had hand rails for residents to use. The facility has a staff bedroom. LPA toured the facility inside and out and observed walkways free and clear of obstruction. LPA observed ramp leading from hallway to backyard. LPA observed a locked shed being used as storage and not living quarters. LPA observed fire extinguisher was last serviced on 01/7/26. LPA reviewed the facility disaster drill log and the last disaster drill was conducted on 03/02/26. LPA reviewed resident records for 3 out of 6 residents. LPA reviewed 2 staff records. LPA and ADM reviewed the Centrally stored medication for 3 residents. DA tested the smoke detector and found the smoke detector to function properly when tested. No deficiency were cited as per California Code of Regulations Title 22. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Required 1 Year visit and met with Iluminada Yap, designated Administrator (DA). LPA announced the purpose of the visit. LPA observed 6 residents and 2 staff. During visit, LPA toured the facility inside and out. LPA toured the Kitchen area and observed a Pantry. LPA observed the kitchen area and observed locked cabinets for medications, sharp objects, and cleaning supplies. LPA observed perishable food supply of at least two days and a non-perishable food supply of at least seven days. Refrigerator temperature measured with thermometer at 44 degrees F. LPA toured 4 resident bedrooms. Each bedroom had available bedding and clothing storage areas as well as functioning lights. ADM tested the smoke detector in the hallway and found the smoke detector to function properly when tested. LPA toured 2 resident bathrooms. Each bathroom had available soap and paper towels and functioning lights. The bathroom shower was equipped with non skid mat and grab bars. The water temperatures in the bathroom sinks measured with thermometer at 113-119 degrees F. During tour LPA observed the hallway had hand rails for residents to use. The facility has a staff bedroom and staff bathroom. During tour LPA observed fruit trees, lime tree and apple tree, squash, and a locked shed in the backyard used as a storage and not as living quarters. The facility deck located in the backyard was updated. page 1 of 2 Page 2 of 2 LPA toured the outside area and found the exits to be clear of obstructions. LPA observed fire extinguisher was last serviced on 01/23/25. LPA reviewed the facility disaster drill log and last disaster drill was conducted on 07/02/25. LPA reviewed resident records for 3 out of 6 residents.. LPA reviewed 3 staff records. DA reviewed the Centrally stored medication for 3 residents. No deficiency were cited as per California Code of Regulations Title 22. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Required 1 Year visit and met with Administrator (ADM) Merle Laurel. During visit, LPA Marrufo toured the facility inside and out. LPA toured the kitchen area and food storage areas and observed there to be a perishable food supply of at least two days and a non-perishable food supply of at least seven days. LPA observed there to be a locked cabinet for storage of cleaning supplies and sharp objects. LPA observed the first aid kit and found it to be complete. LPA toured 4 out of 4 resident bedrooms. Each bedroom had available bedding and clothing storage areas as well as working lights. LPA tested the smoke detectors in the hallways, living room, and 4 out of 4 bedrooms. The smoke detector in the hallway and in 1 on the 4 resident rooms had a very faint sound when tested. The rest of the smoke detectors functioned properly when tested. 1 out of 1 carbon monoxide detectors functioned properly when tested. LPA toured 2 out of 2 resident bathrooms and observed each bathroom to have available soap and paper towels and working lights. The water temperatures in the bathroom sinks measured at 110 F and 111 F. The last recorded Emergency Disaster Drill was conducted on 07/01/2024. LPA toured the outside area and found the outdoor exits were clear of obstructions. LPA reviewed the Centrally Stored Medication and Destruction Records and resident records for 5 out of 5 residents and found them to be complete. LPA reviewed 3 out of 3 staff records and found them to be complete. No deficiencies were cited at this time as per California Code of Regulations Title 22. An Advisory Note was issued.
