Type of visit: Required - 1 Year

On 8/13/2026, Licensing Program Analyst(s) (LPA) Jewel Baptiste conducted an unannounced annual visit at Love N’ Care Villa, using the CARE tool. LPA Baptiste met with DSP Cheryl LaCambra and explained the reason for the visit. Administrator Nicole Axalan arrived 20 minutes later and assisted with the visit. The facility is licensed to serve age range 60 and over. Approved for six (6) non-ambulatory residents of which one (1) may be bedridden in bedroom#3. Hospice waiver approved for six (6) residents. The garage is not approved for residents. The facility is a single-story home, located in a residential area. It consists of 4-resident bedrooms, 1 staff bedroom, 1 bathroom, 1 dining room, 1 living room, kitchen, a garage, a front yard and backyard. LPA Baptiste conducted a tour of the facility with Administrator Nicole and observed the following: Facility is clean and in good repair indoors and outdoors. Kitchen is clean, sharps were locked and inaccessible to residents. Fire extinguishers observed in kitchen, and laundry area. The sharps were locked under kitchen sink, and cleaning supplies were locked in a cabinet located in the laundry area. Sufficient food supplies were observed, at least 2 days of perishables and 7 days of non-perishables. (4) Resident's bedrooms were observed with sufficient lighting, furniture, and bedding supplies. (1) bathroom was observed in working conditions with grab bars, and skid mats. The water temperature was tested at 116.1, which is within the required 105-120 degrees F. The dining room and living room were observed with sufficient sitting area. Smoke/Carbon monoxide detectors were tested and in working condition. The garage contained storage supplies. Backyard were observed with covered sitting area. The back yard also contained a shed with PPE’s and storage supplies. (CONTINUED LIC 809C) The facility does not have any large body of water. Last emergency drill was conducted on 08/09/2026. LPA Baptiste reviewed medication and files for 6 residents. Each resident file reviewed has all the required documentation, except an update medical assessment. The Administrator was not aware of the regulation change. Staff files were reviewed for (3) staff and they have all the required documentation. An administrator certificate was reviewed for Nicole Axalan exp: 8/19/26.The Administrator is currently on the pending list for their certificate. Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during the visit. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Caregiver Cheryl Lacambra and explained reason for visit. Administrator Nicole Axalan arrived shortly. Facility is licensed to serve age range 60 and over. Approved for six (6) non-ambulatory residents, one (1) of which may be bedridden. Bedridden resident approved in bedroom in bedroom #3. Waiver granted for hospice care for two (2). Facility has an approved Dementia Care Plan. The facility is a single-story home located in a residential area. kitchen, dining area, living room, laundry room, 4 resident rooms, 1 caregiver room, 1 resident bathroom, 1 staff bathroom, detached garage, front yard, and backyard. There is sufficient indoor and outdoor activity space for residents. LPA toured the facility and observed the following: Each resident bedroom has the required furniture and bedding. Extra linen was observed in hallway storage cabinet. The Smoke detectors and carbon monoxide detectors were observed throughout the facility and are properly operating. The facility has two (2) fully charged fire extinguishers which is kept in kitchen and laundry area. Cleaning supplies and toxic substances were observed to be inaccessible in a locked cabinet in laundry area Knives were locked under kitchen sink. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 40 degrees F. Facility was observed to have sufficient supply of 2 days perishable & 7 days non-perishable foods. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms were measured between the required range of 105-120 degrees F. The resident bathrooms have the required grabs bars and non-skid mats. The facility does not have a swimming pool or bodies of water on the premises There is a shaded seating area for the residents outside with a garden. Passageways and exits are free of obstruction. SEE LIC 809C Four (4) staff files were reviewed and included criminal clearance record, and health screening with TB. Six (6) resident files were reviewed and included physician’s reports and appraisal needs and service plan. Fire/earthquake drill was conducted August 01, 2025. The medications are centrally stored and locked in a cabinet in kitchen. LPA reviewed medications for four (4) residents, and they are being administered as prescribed by the physician. Mar log is being used. No deficiency was observed during today’s visit. Jul 22, 2025Complaint investigation reportUnsubstantiated

Allegation investigated: Staff financially abused resident in care. Staff physically abused resident in care.

Licensing Program Analysts (LPAs) S Vaid and E Mallet conducted a findings delivery visit to the facility. LPAs Vaid and Mallett were met by Cherly LaCambra and notified the administrator via phone. Administrator Nicole Axalan arrived shortly after. LPA’s conducted a tour with caregiver and did nor observe any health and safety concerns. On 01/23/2025, Licensing Program Analyst (LPA) S Vaid conducted an initial 10-day complaint investigation visit for the above allegations. LPA met with Nicole Axalan, Administrator and purpose of the visit was discussed. Cheryl Lacambra assisted LPA with complaint. LPA did not observe any safety concerns during the facility tour. LPA Vaid requested, collected, and reviewed documents from R1's face sheet, physicians reports, admissions-health care directives, residential appraisal, and copies five current residents: face sheet, physician reports, admissions agreement, residential appraisals. Telephone contact information, hospice nurse, attorney information for 1 lawsuit settlement, and banker who opened account for R1. CONTINUED ON 9099C............ Unsubstantiated Regarding the allegation: Staff financially abused resident in care. It is alleged that the staff at the facility financial abused resident in care. Per the complaint, staff opened separate bank account for resident while family had fiduciary power of attorney. Four (4) out of four (4) staff interviewed deny this allegation. The investigation determined the resident was capable to managing their cash resources. Document review of the pre-placement information dated 6/01/2023, resident appraisal dated 6/01/2023, acknowledged by R1’s family and S4(former administrator) and LIC 602 Physician report for residential care facility for the elderly (RCFE) state R1 to manage own cash resources. According S1 and S3, they assisted R1 to the bank to open a new account on 6/26/2024. Upon R1’s death, S3 informed family and relinquished all R1’s personal and financial belonging to R1’s family, including California ID and debit card including pin number. Documents reviewed indicate the family only had Health Care power of attorney. Fiduciary power of attorney was not corroborated. R1 was able to manage their own cash resources. Five (5) out of six (6) residents interviewed could not corroborate this allegation. Facility residents have not been financially abused by the staff. Based upon record review and interviews conducted the findings indicate that, although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Regarding the allegation: Staff physically abused resident in care. It is alleged that the facility staff physically abused the resident in care. Four (4) out of four (4) staff interviewed deny this allegation. According to the staff they provide residents with living assistance, treat residents with respect and dignity, staff stated they have never physically abused any residents in their care, past nor present. Five (5) out of six (6) residents interviewed could not corroborate this allegation. Residents interviewed stated the facility staff attend to their needs and provide assistance when residents are in need. Two (2) out three (3) witnesses stated they have not observed physical abuse on R1’s body at any time during R1’s resident at the facility. Review of staffing records confirms staff person accused of physical abuse in the complaint worked at the facility between October 2022 and December 2022, R1 started their stay at the facility on 6/01/23. Based upon record review and interviews conducted the findings indicate that, although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the . Type of visit: Prelicensing

Licensing Program Analysts (LPA) Luis Mora conducted an announced pre-licensing visit at the facility using the CARE Tool. LPA met with Nicole Axalan (Licensee) and Evan Cunanan (Licensee) explained the reason for the visit. A change of ownership application was submitted to the Community Care Licensing Department (CCLD) for a Residential Care Facility for the Elderly (RCFE) to serve six (6) non-ambulatory residents, of which one (1) may be bedridden, in the age range of 60 and over. Facility is approved for two (2) hospice waivers. A tour of the single-story facility included: kitchen, dining area, living room, laundry room, 4 resident rooms, 1 caregiver room, 1 resident bathroom, 1 staff bathroom, detached garage, front yard, and backyard. LPA toured the facility with Nicole and Evan, and the following was observed: the front and backyard are well maintained and there are no pools or large bodies of water. There is a shaded seating area for the residents located in the backyard. Passageways and exits are free of obstruction. Auditory devices were seen on exit doors which are required for dementia residents and were operating at the time of the visit. The water temperature was tested in the residents’ bathroom and measured at 110.4 degrees F which is within the required 105 - 120 degrees F. The bathroom is clean and have the required grab bars in the shower and near the toilet for non-ambulatory residents. Showers also have non-skid materials. Resident bedrooms have the required furniture such as bed frames, dressers, lamps and chairs. Bedrooms also have enough closet space. Resident beds have the required linen and the linen is in good condition. There is extra clean linen and towels in the hallway cabinets. Smoke detectors combined with carbon monoxide were observed in each room and throughout the facility and are properly operating. There are 2 fire extinguishers located in the kitchen and laundry room which are fully charged. Kitchen appliances are clean and were operating at the time of the visit. Sharps are kept locked under the kitchen sink and are inaccessible to residents. Sufficient supply of 2 days perishable & 7 days non-perishable foods was observed in the kitchen. Cleaning supplies and toxins are locked in a cabinet in the laundry room and are inaccessible to residents. First Aid kit was fully stocked with current manual and it is kept in the hallway cabinets. (Continued to LIC 809-C) Residents medications are centrally stored in a locked cabinet in the kitchen. Residents and staff files are centrally stored in a locked cabinet in the dining area. LPA reviewed 6 residents’ medication and observed that medication properly documented and given as prescribed. LPA reviewed 6 resident files and 3 staff files. LPA observed administrator certificate for Nicole Axalan 6072466740 with an expiration date of 08/19/2026. No outstanding or pending items were observed by the LPA requiring additional pre-licensing visits. LPA will notify the assigned Centralized Applications Bureau (CAB) Analyst of the completed pre-licensing facility evaluation visit conducted, which included the Component III Orientation.
