Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Kelly Dulek arrived at the facility unannounced to conduct a required annual visit at 02:20PM. LPA was greeted by designee Christopher (Chris) Pastrana, who indicated the Administrator/Licensee is currently out of the country and unavailable for today's visit. Entrance interview conducted. The facility has not yet had any residents move into the facility. Licensee will update CCL when a resident is admitted. At 02:32PM, the LPA, along with designee, briefly toured the physical plant areas inside and outside to ensure there are no health and safety hazards and the facility is in compliance with Title 22 Regulations. The following was observed: GARAGE/OUTDOOR SPACE: The garage contains the laundry area, extra refrigerator and freezer, as well as storage space. The backyard patio contains a shaded area and is equipped with furniture for resident use. KITCHEN: Kitchen appliances appeared to be in operable condition. COMMON SPACES: Living room and dining room furniture was observed to be in good condition. The LPA observed the required postings upon entry. There is a locked cabinet for medication storage in the living room. The fire extinguisher was observed to be fully charged but no proof of purchase or annual service was available. LPA advised to ensure the fire extinguisher has been serviced within the last year and proof of purchase or service completed to be maintained at the facility. BEDROOMS: The facility consists of four (4) total bedrooms, of which three (3) are single occupancy and one (1) is designated as a shared room. LPA observed all resident bedrooms, which were furnished appropriately. Report Continued on LIC 809-C RESTROOMS: One (1) restroom is in the hallway, designated for shared resident use and 1 (one) is designated as a private restroom. Resident restrooms are clean and sanitary and in operating condition. INFECTION CONTROL/EMERGENCY DISASTER PLANNING: Not reviewed during today's visit. RECORD REVIEW: As no residents are in care, no resident files were reviewed during today's visit. MEDICATION REVIEW: As no residents are in care, no medications were observed during today's visit. The purpose of this LIC 809 is to document that corrections have been received. LPA Peraldi emailed CAB regarding proof of corrections. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Emily Peraldi conducted a pre-licensing visit to the above noted facility. At 10:15 a.m., the LPA met with Applicant, Lyka Lacson. This is a new facility. A dementia program was included in the plan of operation. A Hospice Waiver has been granted for five (5) residents. At 10:25 a.m., a physical plant tour was conducted inside and out. An approved fire clearance was received, clearing them for five (5) non-ambulatory residents. The facility is one story residential home. The facility has four (4) resident bedrooms, all rooms are private bedrooms, except for Resident room #4 which is shared. All resident bedrooms have a direct exit to the outside. All residents’ rooms are set up with beds, nightstands, lamps, chests of drawers, chairs and closet space. The beds are furnished with box springs, comfortable mattress and clean linen; which includes pillowcases and blanket (if needed). Lighting in the rooms appeared adequate. The bedrooms were large enough to allow for easy passage between the beds and furniture with a wheelchair or walker. In addition, no bedroom was used as a passageway to another room, bath or toilet. There are no staff rooms – ‘awake night staff only’ - is required. All rooms were free of odors. All window screens were clean and maintained in good repair. There are two (2) bathrooms in the facility. Resident bedroom #4 has a bathroom inside. The residents’ bathrooms have a shower with non-skid materials. Each shower has grab bars. The Applicant stated that grab bars for toilets will be made available. The hot water temperature was tested in the bathrooms and the kitchen and was found to not be within the range of 105*F and 120*F. At 10:27 a.m., the kitchen hot water temperature measured at 115.7*F. At 10:36 a.m., the bathroom hot water temperature measured between 115.2*F and 125.5*F. Water temperature was adjusted during the time of the visit, and the Applicant will send the LPA proof of water temperature measuring within required range. Continued on LIC 809-C. Resident and staff records will be stored in filing cabinets which are currently located in the garage. Medications are centrally stored in locked cabinets in the living area. The first aid supplies were complete, including a thermometer and a current version of a first aid manual. Kitchen knives are stored in a locked cabinet underneath the kitchen sink. The supply of dishes, utensils, pots, pans and drinkware is adequate. The freezer was maintained at zero degrees Fahrenheit (0*F) and the refrigerator was maintained at 37*F. The supply of nonperishable food is adequate. There are no pesticides (poisons) or toxins stored in any food storage area or preparation area with utensils. Appliances in the kitchen were clean and all appeared functional. Trash cans had tight fitting lids. LPA did observe ants in the kitchen area, and Applicant cleaned the area and stated that they will treat for ants and other vermin. The common areas were appropriately furnished, and the lighting was adequate, however additional seating is needed for the living areas inside and out. There are televisions and other entertainment equipment, games and/or activity supplies in the living room. There was sufficient space to accommodate both indoor and outdoor activities. Night lights were maintained in hallways and passageways to nonprivate bathrooms. There is a fireplace in the living room which is adequately screened. Alarms on all exterior doors were engaged at the time of visit and functional. In addition, the physical plant is consistent with the submitted facility sketch/floor plan. The facility had emergency lighting, which included flashlights and batteries. The facility has a furnace, which is able to heat rooms that residents occupy to a minimum of 68 degrees Fahrenheit; and, they have central air conditioning and are able to cool rooms to a comfortable range, not to exceed 85 degrees Fahrenheit. The facility smoke alarm system is hard wired. At 10:45 a.m., the smoke detector and carbon monoxide detectors were tested and functioned properly. The facility has a central entry point for symptom screening and temperature checks for staff and visitors. There is hand sanitizer available at the entrance of the facility. There is a fire extinguisher near the entrance. It was fully charged and purchased on 07/20/2025. The supply of extra bed and bath linens is adequate. Personal hygiene items (shampoos, soaps) were adequate and are stored in the garage. There is a functioning telephone on the premises. The emergency exiting plans/sketch are posted at the entrance of the facility. The emergency telephone numbers are posted near the entrance of the facility. Other required postings are also posted near the entrance of the facility. Continued on LIC 809-C. The exterior passageways were clean and clear of any obstructions. There is a covered patio area at the back of the house. There is a back gate leading to an alleyway. There are two (2) gates with a self-latching mechanism at each side of the house designated for emergency exits. There are no bodies of water on the premises at the present time. The garage is attached to the house and will be inaccessible to residents. Laundry units are located inside the garage. Additional cleaning supplies and household supplies are located inside the garage. Component III was conducted in conjunction with this pre-licensing visit. The following items must be corrected prior to licensure: - The hot water temperature to be maintained between 105-120 degrees F. - All toilets to have grab bars. - Showers to have non-skid mats or strips. - Kitchen area to be free of ants. - The Licensing Complaint Poster posted. - Additional furniture for living room and outside patio to accommodate five (5) residents. Submit proof of corrections to LPA Peraldi so that your application may be completed. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect the approval of your license.
